 1. PROVIDE PARKING STALL DETAILS, TO SCALE, AND AT THE
 PROPOSED ANGLE, SHOWING COMPLIANCE WITH SECTION
 94-485(J) OF THE ZONING AND LAND DEVELOPMENT
 REGULATIONS.
  
 2. PROVIDE WITH AND DEPTH DIMENSIONS OF THE PARKING
 STALLS, AND AISLE WIDTH DIMENSIONS OF THE PARKING LOT.
  
 3. PROVIDE WHEEL STOP DETAILS, TO SCALE, SHOWING
 COMPLIANCE WITH SECTION 94-443(C)(5)(B) OF THE ZONING
 AND LAND DEVELOPMENT REGULATIONS.
  
 4. IF WHEEL STOPS WILL NOT BE USED, AN ADDITIONAL DEPTH
 OF 2 FEET SHALL BE ADDED TO THE LANDSCAPE ISLANDS
 AND/OR BUFFERS TO ALLOW VEHICLES TO OVERHANG INTO THE
 LANDSCAPED AREA.NO PLANT MATERIALS OVER 6 INCHES IN
 HEIGHT SHALL BE PLANTED WITHIN THE 2 FOOT CAR OVERHANG.
 PLEASE NOTE THAT THIS OPTION WILL REQUIRE A MINOR
 AMENDMENT TO THE GOOD SAMARITAN HOSPITAL COMMUNITY
 SERVICE PLANNED DEVELOPMENT (CSPD) PRIOR TO OBTAINING A
 BUILDING PERMIT.
  
 5. PURSUANT TO SECTION 94-443(C)(5)(A), LANDSCAPED
 AREAS SHALL BE SEPARATED FROM VEHICULAR USE AREAS BY
 NONMOUNTABLE, CONCRETE CURBING OF THE TYPE
 CHARACTERIZED AS ?TYPE D? OR CURBING OF COMPARABLE
 DURABILITY.PROVIDE AND IDENTIFY THE TYPE OF CURBING
 AROUND ALL TERMINAL LANDSCAPED ISLANDS.
  
 6. A SITE VISIT COMPLETED ON APRIL 28, 2008, SHOWED
 THAT A NUMBER OF TREES, PALMS AND HEDGES WERE REMOVED
 FROM THAT PORTION OF THE PARKING LOT THAT IS THE
 SUBJECT OF THIS PERMIT APPLICATION.THE PLANNING AND
 ZONING DEPARTMENT HAS NO RECORDS THAT A TREE ALTERATION
 PERMIT WAS EVER APPROVED FOR THIS SITE.THUS, ALL
 MISSING PLANT MATERIALS SHALL BE REPLACED.PLEASE
 CONTACT ROBERT KUSSNER, LANDSCAPE PLANNER, AT (561)
 822-1462 FOR DETAILS.
  
 7. PURSUANT TO SECTION 94-443(C)(3)(A), TERMINAL
 LANDSCAPED ISLANDS SHALL NOT BE LESS THAN 5 FEET IN
 WIDTH AND THE LENGTH OF THE ADJACENT PARKING STALL.
 PLEASE NOTE THAT THIS CHANGE IN THE SITE PLAN WILL
 REQUIRE A MINOR AMENDMENT APPLICATION.THE MINOR
 AMENDMENT APPLICATION FEE IS $300, AND IS REVIEWED
 ADMINISTRATIVELY (STAFF LEVEL).PLEASE CONTACT THE
 PLANNING AND ZONING DEPARTMENT FOR DETAILS.
  
 8. PURSUANT TO SECTION 94-443(C)(2), ADDITIONAL SHADE
 TREES (GREEN BUTTONWOODS) SHALL BE PLANTED ALONG THE
 EAST PROPERY LINE.
