 PLANS AND REVISE AS REQUIRED.
  
 B) COMPLETE ALL INFORMATION ON TOP OF FORM.
 INCLUDING BUILDER INFORMATION.
  
 C) OWNER/AGENT TO SIGN AND DATE AT BOTTOM OF FORM.
