 INCLUDING BUILDER'S NAME.
  
 C) ARCHITECT SIGNING ENERGY CALCULATIONS NEED TO
 PROVIDE HIS SEALAS REQUIRED BY SEC. 481 FLORIDA
 STATUTE AND FLORIDA ADMINISTRATIVE CODE SEC.
 61G1-16.003.
  
 D) OWNER/AGENT TO SIGN AND DATE BOTTOM OF FORM.
  
 4- PROVIDE COMPLETE INFORMATION FOR STORM SHUTTERS,
 INCLUDING PRODUCT APPROVAL, KEY PLAN AND A COMPLETE
 INSTALLATION SCHEDULE. SEE ATTACHED SAMPLE OF
 INSTALLATION SCHEDULE.
 ***NOTE:DESIGNER OF RECORD TO REVIEW AND APPROVE
 PRODUCT APPROVAL PRIOR TO SUBMITTAL.
