 OF 20 OF NOA 02-0212.07 AND PROVIDE THE REQUIRED RAS
 117 CALCULATIONS. INFORMATION SUBMITTED ARE NOT THE RAS
 117 CALCULATIONS.
  
 B) CALCULATED DESIGN PRESSURES ARE TOO LOW COMPARE
 TO TABLE 1609.6.2.1(2) OF 2004 FBC.ENGINEER TO
 PROVIDE ACTUAL CALCULATIONS SHOWING CALCULATED DESIGN
 PRESSURES. SEC. 106.1.2 CITY AMENDMENTS TO FBC 2004.
 ***NOTE: ENGINEER TO CHECK ROOF PLAN.THIS HOUSE HAS
 PITCH ROOF AND FLAT ROOF SYSTEMS, THEREFORE DESIGN
 PRESSURES ARE DIFFERENT FOR EACH ROOF TYPE.ALSO,
 CORRECT FLAT ROOF AREA NEED TO BE CONSIDER FOR DESIGN
 PRESSURES CALCULATIONS.
  
 C) ENGINEER TO IDENTIFY SPECIFIC APPROVED FLAT ROOF
 ASSEMBLY AND SPECIFIC FASTENING OPTIONS FOR WHICH HE IS
 PROVIDING ENHANCED FASTENING CALCULATIONS.SEE NOA
 SUBMITTED. COORDINATE WITH CONTRACTOR FOR SPECIFIC NAIL
 TYPE TO BE USED.
  
 D) ENGINEER TO PROVIDE ENGINEERING FIRM'S
 CERTIFICATE OF AUTHORIZATION AS REQUIRED BY FLORIDA
 ADMINISTRATIVE CODE SEC. 61G15--23.001.
  
 E) CITY OF WEST PALM BEACH ADDRESS IS INCORRECT.
 REVISE AS REQUIRED.
  
 2-CORRECT NOA FOR ROOF TILE WAS REQUESTED ON PREVIOUS
 CHECKLIST.SUCH NOA WAS NOT PROVIDED.THE ONLY
 INFORMATION SUBMITTED WAS A COPY OF FLORIDA PRODUCT
 APPROVAL FL282-R1.THIS PRODUCT APPROVAL IS FOR
 ANOTHER TILE MANUFACTURER AND THERE WAS NO NOA
 SUBMITTED WITH IT. PROVIDE 2 COPIES OF COMPLETE AND
 CORRECT INFORMATION.
  
 3- ROOF TILE FASTENING WAS NOT PROVIDED AS REQUESTED ON
 PREVIOUS CHECKLIST.IF USING POLYFOAM, THEN PROVIDE
 PRODUCT APPROVAL AS REQUIRED BY DCA RULE 9B-72.IS
 USING SCREWS OR NAILS CLEARLY IDENTIFY WHICH TYPE AND
 NUMBER TO VERIFY COMPLIANCE WITH THE PRODUCT APPROVAL
 AND DESIGN PRESSURES.
  
 4- NOTICE OF COMMENCEMENTSUBMITTEDEXPIRED ON
 12/24/2006. NEED TO PROVIDE NEW NOTICE OF COMMENCEMENT
 AS REQUIRED BY SEC. 713.13 FLORIDA STATUTES.
  
 5- PERMIT APPLICATION HAS THE GENERAL CONTRACTOR AS THE
 OWNER.GENERAL CONTRACTOR IS NOT THE OWNER.THESE ARE
 PUBLIC RECORDS AND THEY NEED TO HAVE CORRECT OWNER'S
 INFORMATION.PLEASE PROVIDE NEW PERMIT APPLICATION
