 VALUE HAS BEEN REVISED TO $36,381.ADDITIONAL PERMIT

 4.SUBMIT TWO COPIES OF ENERGY CALCS PER 2004 FBC.
  
 5.PRODUCT APPROVALS REQUIRED FOR ROOF, DOORS,
 WINDOWS, IMPACT PROTECTION AND LINTELS.
  
 6.ALL PRODUCT APPROVALS SUBMITTED WITH QUALITY
 ASSURANCE SHALL HAVE THE FOLLOWING STATE APPROVAL
 ATTACHED.
  
 PRODUCT APPROVALS SUBMITTED WITH PERMIT APPLICATION
 AFTER OCTOBER 1, 2003 ARE REQUIRED TO COMPLY WITH THE
 FLORIDA PRODUCT APPROVAL SYSTEM. FOR INFORMATION PLEASE
 SEE THE STATE WEBSITE AT WWW.FLORIDABUILDING.ORG.
 PRODUCTS WITH STATEWIDE APPROVAL ARE REQUIRED TO BE
 SUBMITTED WITH A STATE PRODUCT APPROVAL SHEETS THAT
 LISTS THE PRODUCT IDENTITY NUMBER FROM THE STATE. IF
 THE PRODUCT DOES NOT HAVE STATEWIDE APPROVAL, SUBMIT AN
 APPLICATION FOR LOCAL PRODUCT APPROVAL OR SITE SPECIFIC
 FORM PER RULE 9B-72. SEE ATTACHMENT.

 7.THOSE PRODUCT APPROVALS WHICH ARE REGULATED BY DCA
 RULE 9B-72 SHALL BE REVIEWED AND APPROVED IN WRITING BY
 THE DESIGNER OF RECORD PRIOR TO SUBMITTAL FOR
 JURISDICTIONAL APPROVAL.WPB AMENDMENT TO FBC 106.3.3.
  
  
 8.SUBMIT TWO SETS OF TRUSS DRAWINGS FOR REVIEW PER
 2004 FBC R-802.10.1TRUSS DRAWING REQUIRED TO BE
 APPROVED PRIOR TO INSTALLATION.
  
 9.A DESIGN PROFESSIONAL OR AN OWNER MUST ELECT ONE OR
 A COMBINATION OF LEVELS OF ALTERATION PURSUANT TO
 SECTIONS 303, 304 AND 305 OF THIS CODE.SHOW HOW PLANS

 CONVEYANCE OR OTHER APPROVED POINT OF COLLECTION SO AS
 TO NOT CREATE A HAZARD. LOTS SHALL BE GRADED SO AS TO
 DRAIN SURFACE WATER AWAY FROM FOUNDATION WALLS. THE
 GRADE AWAY FROM FOUNDATION WALLS SHALL FALL A MINIMUM
 OF 6 INCHESWITHIN THE FIRST 10 FEET.
  
 11.SHOW USE OF EXISTING ROOMS ADJACENT TO ROOM
 ADDITION.
  
 12.SHOW WINDOW AND DOOR PRESSURES.
  
 13.SHOW SIZE AND LOCATION OF ATTIC ACCESS.R- 807.1

 REMOVE & REPLACE ANY PAGES AS NECESSARY. SUBMIT ONE
 COPY OF OLD PAGES FOR REFERENCE . A TRANSMITTAL LETTER
 LISTING THE ORIGINAL REVIEW COMMENT NUMBER, WITH A
 DESCRIPTION OF THE REVISION MADE, IDENTIFYING THE SHEET
 OR SPECIFICATION PAGE WHERE THE CHANGES CAN BE FOUND
