 D) THE DESIGN OCCUPANT LOAD, SEE 1004.
 E) IF AN AUTOMATIC SPRINKLER SYSTEM IS
 PROVIDED, WHETHER THE SPRINKLER SYSTEM
 IS REQUIRED.
 F) ANY SPECIAL STIPULATIONS & CONDITIONS
 OF THE BUILDING PERMIT.

 PERMIT WILL BE ISSUED. NOTE: 713.13(2)
 IF THE WORK DESCRIBED IN THE NOTICE OF
 COMMENCEMENT IS NOT ACTUALLY COMMENCED
 WITHIN 90 DAYS AFTER THE RECORDING
 THEREOF, SUCH NOTICE IS NULL & VOID.
  
 3) NOTE:105.9 ASBESTOS. THE ENFORCING
 AGENCY SHALL REQUIRE EACH BUILDING
 PERMIT FOR THE DEMOLITION OR RENOVATION
 OF AN EXISTING STRUCTURE TO CONTAIN AN
 ASBESTOS NOTIFICATION STATEMENT WHICH
 INDICATES THE OWNER?S OR OPERATOR?S
 RESPONSIBILITY TO COMPLY WITH THE
 PROVISIONS OF SECTION 469.003, FLORIDA
 STATUTES, AND TO NOTIFY THE DEPARTMENT
 OF ENVIRONMENTAL PROTECTION OF HIS OR
 HER INTENTIONS TO REMOVE ASBESTOS, WHEN
 APPLICABLE, IN ACCORDANCE WITH STATE AND
 FEDERAL LAW.
  
 4)(2004 FBC) 106.1.1 INFORMATION ON
 CONSTRUCTION DOCUMENTS. CONSTRUCTION
 DOCUMENTS SHALL BE OF SUFFICIENT CLARITY
 TO INDICATE THE LOCATION, NATURE &
 EXTENT OF THE WORK PROPOSED & SHOW IN
 DETAIL THAT IT WILL CONFORM TO THE
 PROVISIONS OF THIS CODE AND REVELENT
 LAWS, ORDINANCES, AND RULES AND
 REGULATIONS AS DETERMINED BY THE
 BUILDING OFFICIAL.
  
 5) INDICATE CLEARLY ON THE DRAWINGS WHAT
 TYPE OF BUSINESS WILL BE AT THIS
 LOCATION AND THE SQUARE-FOOTAGE OF
 WAREHOUSE AND THE PATIENT/OFFICE AREA.
  
 6) WHAT TYPE OF MATERIALS WILL BE STORED
 IN THE WAREHOUSE AND WHAT AMOUNT WILL BE
 STORED. WILL THE BE FORK-LIFTS BE
 DRIVING AROUND IN THE WAREHOUSE?
  
 7) A MORE DETAIL DRAWING FOR THE HOLDING
 AREA FOR THE OXYGEN TANK IS REQUIRED
 SHOWING HOW THE TANKS WILL BE PREVENTED
 FROM TIPPING OVER. ALSO THE SIZE AND
 TYPE OF TANKS SHALL BE INDICATED ON THE
 PLANS.
  
 8) WHAT IS THE REASON FOR HAVING A
 PATIENT SERVICE TECH OUT IN THE
 WAREHOUSE AREA.
  
 9) THE PLANS INDICATE THE OCCUPANCY AS :
 SMALL ASSEMBLY HOWEVER, LOOKING AT THE
 DRAWINGS AND READING THE DEFINITION OF
 ASSEMBLY, THESE PLANS DO NOT FALL UNDER
 THAT TYPE OF OCCUPANCY. SEE FBC SECTION
 "303 AND 304" FOR THE CORRECT TYPE OF
 OCCUPANCY.
  
 10) SEE FBC.SEC 311 FOR THE TYPE OF
 STORAGE WHICH IS REQUIRED FOR THE
 BUILDING. STATE CLEARLY THE OCCUPANT
 LOAD OF THE BUILDING.
  
 11) SHEET 2.1 MENTION "STUDIO" WHERE IS
 THE STUDIO ON THE PLANS?
  
 12) THOSE PRODUCT APPROVALS WHICH ARE
 REGULATED BY DCA RULE 9B-72 SHALL BE
 REVIEWED AND APPROVED IN WRITING BY THE
 DESIGNER OF RECORD PRIOR TO SUBMITTAL
 FOR JURISDICTIONAL APPROVAL.WPB
 AMMENDMENT TO FBC 106.3.3
 PRODUCT APPROVALS SUBMITTED WITH PERMIT
 APPLICATION AFTER OCTOBER 1, 2003 ARE
 REQUIRED TO COMPLY WITH THE FLORIDA
 PRODUCT APPROVAL SYSTEM. FOR INFORMATION
 PLEASE SEE THE STATE WEBSITE AT
 WWW.FLORIDABUILDING.ORG. PRODUCTS WITH
 STATEWIDE APPROVAL ARE REQUIRED TO BE
 SUBMITTED WITH A COVER SHEET THAT LISTS
 THE PRODUCT IDENTITY NUMBER FROM THE
 STATE. IF THE PRODUCT DOES NOT HAVE
 STATEWIDE APPROVAL, SUBMIT AN APPLICA-
 TION FOR LOCAL PRODUCT APPROVAL OR SITE
 SPECIFIC FORM PER RULE 9B-72. SEE
 ATTACHMENT. WWW.FLORIDABUILDING.ORG. THE
 NEW ROLL UP DOOR NEEDS PRODUCT APPROVAL.
  
 13) 11-4.1.6(2) DUTY TO PROVIDE AN
 ACCESSIBLE PATH OF TRAVEL TO ALTERED
 AREAS SHALL BE DEEMED DISPROPORTIONATE
 TO THE OVERALL ALTERATION WHEN THE COST
 EXCEEDS 20 % OF THE COST OF THE ALTERA-
 TION TO THE PRIMARY FUNCTION AREA.
