 1)A RECORDED COPY OF THE NOTICE OF
 COMMENCEMENT MUST BE SUBMITTED BEFORE A
 PERMIT CAN BE ISSUED.
  
 2) THE STRUCTURE IS LOCATED IN AN "AO"
 FLOOD ZONE. AN ELEVATION CERTIFICATE
 WILL BE REQUIRED.
  
 3) ADDITIONAL PERMIT FEES ARE DUE. THE
 CALCULATED VALUE OF THE STRUCTURE IS
 $1,001,140.00. A WESTWARD EXPANSION FEE
 OF $550.00 PER UNIT IS ALSO REQUIRED.
  
 4) SITE PLANS ARE REQUIRED WHICH
 SPECIFY THE DISTANCE TO ASSUMED PROPERTY
 LINES BETWEEN ADJACENT BUILDINGS LOCATED
 ON THE SAME LOT. THIS IS REQUIRED IN
 ORDER TO DETERMINE THE PERCENTAGE OF
 WALL AREA THAT IS ALLOWED AS OPENINGS BY
 FBC TABLE 600.
  
 5) THE SCALE SPECIFIED IN THE INDEX ON
 A1 FOR THE DRAWINGS ON SHEET A4 IS
 INCORRECT. THE CORRECT SCALE IS 1/8".
  
 6) THE CONSTRUCTION TYPE SPECIFIED ON A1
 IS FROM THE 2004 FBC. THESE PLANS WERE
 REVIEWED UNDER THE 2001 FBC AND THE
 STRUCTURAL PLANS ARE ON THE 2001 FBC.
 PLEASE REVISE SHEETS A1 AND A9.
  
 7) BASED ON TYPE VI CONSTRUCTION,
 UNPROTECTED AND UNSPRINKLERED, 2001 FBC
 TABLE 500 LIMITS THE AREA PER FLOOR TO
 7000 SQFT. USING THE FORMULA FOR AREA
 INCREASES ALLOWED BY SECTION 503.3, AND
 THE FRONT AND REAR BUILDING PERIMETERS
 WHICH 30FT WIDE HORIZONTAL SEPARATIONS
 THE ALLOWABLE AREAS ARE EXCEEDED AT THE
 SECOND FLOOR. IF THE PORTIONS OF THE
 LEFT AND RIGHT SIDE PERIMETERS THAT HAVE
 30FT TO THE ASSUMED PROPERTY LINE
 BETWEEN BUILDINGS (SEE SECTION 505) WERE
 USED IN THE CALCULATIONS THE AREA
 INCREASE ALLOWED MAY COMPLY. HOWEVER, NO
 SITE PLAN INDICATING ASSUMED PROPERTY
 LINES WAS SUBMITTED. SUBMIT AREA
 INCREASE CALCULATIONS.
  
 8)CONDOMINIUM V APARTMENTS.IF THE
 DWELLING UNITS ARE TO BE SOLD AS
 CONDOMINIUMS, A 'DECLARATION OF
 CONDOMINIUM' SHALL BE SUBMITTED TO THE
 STATE FOR APPROVAL, AND THEN RECORDED
 WITH THE CLERK OF THE COUNTY COURT.A
 RECORDED COPY SHALL BE SUBMITTED TO THE

 11) PLEASE REVISE THE STANDARDS
 REFERENCED UNDER DESIGN CRITERIA ON S5
 TO THE EDITIONS LISTED IN CHAPTER 35 OF
 THE 2001 FBC. (ACI 318-02 AND 530.1-02)
  
 12) SPECIFY METHOD OF TERMITE PROTECTION
 AS REQUIRED BY FBC 1816.
  
 13) PRODUCT APPROVALS SUBMITTED WITH
 PERMIT APPLICATION AFTER OCTOBER 1,
 2003 ARE REQUIRED TO COMPLY WITH THE
 FLORIDA PRODUCT APPROVAL SYSTEM. FOR
 INFORMATION PLEASE SEE THE STATE WEBSITE
 AT WWW.FLORIDABUILDING.ORG. PRODUCTS
 WITH STATEWIDE APPROVAL ARE REQUIRED TO
 BE SUBMITTED WITH A COVER SHEET THAT
 LISTS THE PRODUCT IDENTITY NUMBER FROM
 THE STATE. IF THE PRODUCT DOES NOT HAVE
 STATEWIDE APPROVAL, SUBMIT AN APPLICA-
 TION FOR LOCAL PRODUCT APPROVAL OR SITE
 SPECIFIC FORM PER RULE 9B-72.
  
 14) SPECIFY HOW IMPACT PROTECTION WILL
 BE PROVIDED FOR THE GLAZED OPENINGS AS
 REQUIRED BY FBC 1606.1.4. ITEM #13 OF
 THE CITY OF WPB IMPLEMENTATION STANDARD
 FOR HURRICANE MISSLE IMPACT PROTECTION
 REQUIRES PRE-STORM PREPARATION PLANS TO
 BE SUBMITTED FOR REVIEW IF USING STORM
 SHUTTERS, IN ADDITION TO A KEY PLAN,
 INSTALLATION SCHEDULE AND PRODUCT
 APPROVALS. STORM PANELS ARE NOT ALLOWED
 ABOVE THE FIRST FLOOR FOR THIS TYPE OF
 STRUCTURE AND OCCUPANCY.
  
 15) SUBMIT THE EVALUATION REPORT AND
 INSTALLATION INSTRUCTIONS FROM NATIONAL
 ACCREDITATION AND MANAGEMENT INSTITUTE
 FOR THE THERMA-TRU DOORS WITH FL # 1170.
 THE INFORMATION THAT WAS SUBMITTED TO
 THE STATE FOR STATE PRODUCT APPROVAL IS
 REQUIRED TO BE SUBMITTED WITH THE STATE
 COVER SHEET.
  
 16) SUBMIT PRODUCT APPROVAL FOR THE
 SMALL (8' X 7') OVERHEAD GARAGE DOOR
 WHICH IS LESS THAN THE MINIMUM SIZE
 ALLOWED BY THE PRODUCT APPROVAL FOR THE
 LARGE DOOR THAT WAS SUBMITTED.
  
 17) SUBMIT THE SBCCI REPORT (2139A) THAT
 IS LISTED ON THE FL PRODUCT APPROVAL
 WEBSITE FOR THE COLLIER GARAGE DOORS.
  
 18) SUBMIT PRODUCT APPROVALS FOR THE
 TRUSS CONNECTORS.
  
 19) PROVIDE A STRUCTURAL DETAIL ON THE
 PLANS FOR THE CONCRETE STAIRS. SPECIFY
 STEEL REINFORCEMENT FOR THE STAIRS,
 LANDINGS AND AT THE BASE OF THE STAIRS.
  
 20) SUBMIT SIGNED AND SEALED TRUSS
 DRAWINGS AND LAYOUTS THAT HAVE REVIEWED
 AND APPROVED BY THE DESIGNER OF RECORD
 FOR THE STRUCTURE.
  
 21) SUBMIT A TECHNICAL BULLETIN OR
 REPORT FROM USP THAT ALLOWS TRUSS HANGER
 JUS26-2 TO BE ATTACHED TO MASONRY WITH
 TAPCON FASTENERS AND SPECIFYING THE
 ALLOWABLE LOADS FOR THE APPLICATION.
  
 22) PROVIDE SHOP DRAWINGS WITH
 STRUCTURAL DETAILS FOR THE TYPICAL
 INTERIOR GUARDRAIL DETAIL AS
 REFERENCED BY THE SECTION ON A12.
  
 23) SUBMIT SHOP DRAWINGS FOR THE
 EXTERIOR GUARDRAILS. INDICATING HOW THEY
 WILL BE ATTACHED TO THE STRUCTURE.
  
 24) THE ENGINEER IS REQUIRED TO SIGN HIS
 NAME TO THE PLANS HE SEALS PER FAC
 61G15-23.002. PLEASE SUBMIT A NOTARIZED
 LETTER SIGNED, SEALED AND DATED BY THE
 ENGINEER ATTESTING HIS LEGAL SIGNATURE.
  
 25) THE 5' BI-PASS DOOR AT HER WALK-IN
 CLOSET (#7 IN SCHEDULE) DOES NOT COMPLY
 WITH FAIR HOUSING REQUIREMENTS. USEABLE
 DOORS WITHIN THE DWELLING UNIT MUST HAVE
 A CLEAR OPENING OF AT LEAST 31-5/8"
 CLEAR.
  
 26) THE STRUCTURAL PLANS DO NOT SHOW THE
 ELEVATOR RECESSES ON THE FOUNDATION PLAN

 DETAILS FOR THESE AREAS.

 INFILL FLOOR FRAMING AT THE ELEVATOR
 RECESSES AND OPENINGS WHEN THE CLOSET
