 THE NOTICE OF COMMENCEMENT SHALL BE
 RECORDED AT PALM BEACH COUNTY COURTHOUSE
 AND A COPY SUBMITTED TO THIS OFFICE
 BEFORE A PERMIT CAN BE ISSUED.BLANK
 FORMS ARE AVAILABLE FROM THIS OFFICE.
  
 2) INFORMATIVE:
 COMMENT RESPONSE DATED 6/15/04 IS NOTED.
 IMPACT FEES. THE PLANS SHALL BE TAKEN
 TO PALM BEACH COUNTY BUILDING DEPARTMENT
 FOR IMPACT FEE ASSESSMENT. THEY SHALL BE
 STAMPED AT THAT OFFICE AND A COPY OF THE
 PAID RECEIPT SUBMITTED TO THE CITY OF
 WEST PALM BEACH DEPT OF CONSTRUCTION
 SERVICES BEFORE A PERMIT CAN BE ISSUED.

 THE SIGNED AND SEALED SURVEY DRAWING AND
 INSERT ONE COPY INTO EACH SET OF PLANS.

 PERMIT CAN BE ISSUED, SHEET 2 OF THE
 SUBMITTED ENERGY CODE COMPLIANCE FORMS
 SHALL BE SIGNED BY THE BUILDING OWNER,
 OWNER'S ARCHITECT OR OTHER AUTHORIZED
 AGENT LEGALLY DESIGNATED BY THE OWNER.
 -ALSO, ADD THE OWNER'S NAME IN THE
 SPACE PROVIDED ON SHEET 1 OF THE FORM,
 -AND CORRECT THE OCCUPANCY CLASS TO
 'FACTORY/WAREHOUSE' (NOT 'ASSEMBLY') AS
 PER 'BUILDING DATA' ON SHEET SP-1.

 APPROVED OPENING PROTECTIVES (MIN 3/4 HR
 RATING, SEE FBC TABLE 705.1.2).AMEND
 THE PLANS TO DEMONSTRATE COMPLIANCE.

 SHELL BUILDING ONLY.THE BUILDING, AS
 CURRENTLY PROPOSED SHALL NOT BE
 OCCUPIED WITHOUT TENANT BUILD OUT TO
 ENSURE THE REQUIRED SANITARY PROVISIONS
 ARE MADE.

 TILT WALL PANELS.SUBMIT SHOP DRAWINGS
 PREPARED BY A FLORIDA REGISTERED
 ENGINEER AND REVIEWD BY THE ENGINEER OF
 RECORD, AS REFERENCED ON SHEET S-9.
  
 15) SUBMITTAL OF STATEWIDE PRODUCT
 APPROVAL COVER SHEETS IS NOTED BUT, LIKE
 THAT SUBMITTED FOR THE LOUVER DOOR, EACH
 ONE SHALL INCLUDE THE SUPPORTING
 EVALUATION/TEST REPORT DOCUMENT (E.G.
 MIAMI-DADE NOA) GIVING THE MAXIMUM
 TESTED SIZE, DESIGN PRESSURES AND
 INSTALLATION INSTRUCTIONS.THIS APPLIES
 TO THE SUBMITTED PRODUCT APPROVALS FOR
 BUILT-UP ROOFING, STOREFRONT WINDOWS,
 EXTERIOR DOORS AND SECTIONAL DOORS.
 REFER TO THE SUBMITTED PRODUCT APPROVAL
 FOR THE LOUVER DOOR WHICH INCLUDES THE
 REQUIRED SUPPORTING DOCUMENT.IF NOT
 FOUND ON THE DEPARTMENT OF COMMUNITY

 DADE PRODUCT CONTROL NOTICES OF

 18) SUBMIT THREE COPIES OF EACH PRODUCT
 APPROVAL.ONE COPY IS FOR THE RESIDENT
 INSPECTOR.
  
 19) REMOVE 'VOID' SHEETS FROM PLAN SETS.
 TO EXPEDITE REVIEW OF A RESUBMITTAL
 WHICH REQUIRES NEW PLANS IN ORDER TO
 ADDRESS THE ABOVE COMMENTS, THE OLD
 SHEETS SHALL BE REMOVED AND THE NEW
 SHEETS INSERTED.ONE COPY OF EACH OLD
 SHEET SHOULD BE STAPLED/CLIPPED TO THE
 REMAINING OLD SHEETS, TO FORM A SET OF
 'OLD/VOID SHEETS', FOR COMPARISON OF
 REVISIONS WHEN REVIEWING THE RESUBMITTED
 PLANS.
