 PLEASE ADDRESS THE ITEMS NOTED BELOW:
  
 NOTE: THIS STRUCTURE IS CONSIDERED A THRESHOLD BUILDING
 PER FS 553.71 12) "THRESHOLD BUILDING" MEANS ANY
 BUILDING WHICH IS GREATER THAN THREE STORIES OR 50 FEET
 IN HEIGHT, OR WHICH HAS AN ASSEMBLY OCCUPANCY
 CLASSIFICATION AS DEFINED IN THE FLORIDA BUILDING CODE
 WHICH EXCEEDS 5,000 SQUARE FEET IN AREA AND AN OCCUPANT
 CONTENT OF GREATER THAN 500 PERSONS.
  
 PLEASE READ AND FILL OUT THE REQUIRED DOCUMENTS IN THE
 "GUIDELINES AND POLICIES FOR PROJECTS CONCERNING
 THRESHOLD/SPECIAL INSPECTORS AND RESIDENT INSPECTORS".
 YOU CAN E MAIL ME AND ASK FOR A COPY.
  
 1) ANY REPAIRS TO THRESHOLD BUILDINGS ARE REQUIRED TO
 COMPLY WITH FS 553.79. THRESHOLD BLDG. REQUIRED
 STATEMENT: W.P.B. AMENDMENT 110.8.4.4 ALL PLANS FOR THE
 BUILDING WHICH ARE REQUIRED TO BE SIGNED AND SEALED BY
 THE ARCHITECT OR ENGINEER OF RECORD CONTAIN A STATEMENT
 THAT, TO THE BEST OF THE ARCHITECT'S OR ENGINEER'S
 KNOWLEDGE, THE PLANS AND SPECIFICATIONS COMPLY WITH THE
 APPLICABLE MINIMUM BUILDING CODES AND THE APPLICABLE
 FIRE-SAFETY STANDARDS AS DETERMINED BY THE LOCAL
 AUTHORITY IN ACCORDANCE WITH THIS SECTION AND CHAPTER
 633 FLORIDA STATUTES.
  
 2) THRESHOLD INSPECTOR: WPB AMENDMENT 110.8.1 W.P.B.
 CONSTRUCTION SERVICES DEPARTMENT REQUEST FOR THRESHOLD
 BUILDINGS A SPECIAL INSPECTOR AS REQUIRED BY 553.79(5)
 FLORIDA STATUTES TO THE MINIMUM INSPECTIONS REQUIRED BY
 THIS CODE. THE THRESHOLD INSPECTION FORMS ARE REQUIRED
 TO BE FILED OUT BY ALL PARTIES WITH INTEREST, NOTARIZED
 AND RETURNED TO THIS OFFICE, AND REVIEWED AND APPROVED
