 NOT A REVISION. PERMIT TYPE WAS CHANGED FROM REVISION
 TO RESIDENTIAL-MISCELLANEOUS PERMIT. CONTRACTOR NEEDS
 TO COMPLETE AND SUBMIT NEW PERMIT APPLICATION FOR THIS
 SCOPE OF WORK. SEC. 105.1 CITY AMENDMENTS.
  
 2- NEW PERMIT APPLICATION TO INCLUDE THE VALUE FOR THIS
 ADDITIONAL SCOPE OF WORK. SEC. 109.3 CITY AMENDMENTS.

 B)CLEARLY SPECIFY POST SPACING AND ANCHORING, AND

 5- PROVIDE THE NAME AND SIGNATURE OF THE PERSON
 RESPONSIBLE FOR THE DESIGN OF THE PROPOSED RETAINING
