  IF A HYDRANT METER OR BACKFLOW PREVENTER IS DESIRED,

 FAX THE COMPLETED "DEMOLITION APPLICATION" FORM TO

 2. SEWER LATERAL CAPPING PERMIT IS REQUIRED (SUBMIT
 APPLICATION TO BUILDING DIVISION); SCHEDULE A FINAL
 PLUMBING #703. PLEASE NOTIFY US VIA EMAIL AFTER FINAL
 INSPECTION HAS PASSED). THE PERMIT HAS BEEN ISSUED BUT
 NOT INSPECTED.
  
 3. EXTERMINATION LETTER - LETTER FROM A LICENSED PEST
 CONTROL COMPANY STATING THAT DEMOLITION ADDRESS HAS
 BEEN INSPECTED AND/OR TREATED FOR RODENTS.
  
 4. SUBMIT A LETTER INDICATING METHOD OF DUST CONTROL.
 IF ON SITE WATER WILL BE USED, A BACKFLOW PREVENTER IS

 EXISTING BUILDINGS MAY NOT BE DESTROYED OR REMOVED
 UNLESS THEY ARE TO BE REPLACED ACCORDING TO THESE URBAN
 REGULATIONS AND ONLY AFTER A BUILDING PERMIT WHICH
 AFFECTS THE VERTICAL CONSTRUCTION FOR THE NEW BUILDING
 HAS BEEN OBTAINED BY THE PROPERTY OWNER OR DEVELOPER.
 PERMITS WHICH AFFECT VERTICAL CONSTRUCTION INCLUDE THE
 FOUNDATION, VIBRO-FLOTATION, SOIL MIXING, PRESSURE
 GROUTING AND FULL BUILDING PERMITS.
  
 1. PLEASE OBTAIN OR PROVIDE BUILDING PERMIT RELATED TO
 THE VERTICAL CONSTRUCTION OF THE NEW STRUCTURE ON SITE.
