 1) PROVIDE A SURVEY, PREPARED BY A FLORIDA LICENSED
 SURVEYOR, THAT SHOWS THE RIGHT-OF-WAY LINE, THE
 SIDEWALK, EDGE OF PAVEMENT WHERE NO SIDEWALK EXISTS,
 AND ANY TREES WITHIN THE RIGHT-OF-WAY ALONG THE ENTIRE
 FIBER CONDUIT PATH SO THAT STAFF CAN VERIFY THAT THE
 BORE, OPEN TRENCH AND ANY REQUIRED BORE PIT AND
 HANDHOLES ARE WITHIN THE RIGHT-OF-WAY AND NOT ON
 PRIVATE PROPERTY.
  
 2) THE TUBULAR MARKERS ARE NOT PERMITTED. MARKERS MAY
 ONLY BE INSTALLED WITHIN THE HANDHOLES OR OTHERWISE
 BURIED.
  
 3) THE BORE ADJACENT TO HANDHOLE 00+00 APPEARS TO CROSS
 PRIVATE PROPERTY, WHICH IS NOT PERMITTED UNLESS THE
 SURVEY CLEARLY SHOWS A GENERAL UTILITY EASEMENT AT THE
 LOCATION.
  
 4) PROVIDE A DIMENSION LINE THAT VERIFIES THAT THE EDGE
 OF THE TRENCH ADJACENT TO 00+00 IS NO CLOSER THAN THREE
 FEET FROM THE TRUNK OF THE EXISTING PALM TREE AT THE
 BROADWAY AND 37TH STREET INTERSECTION.
  
 5) THE ENGINEER PLAN IS NOT ACCURATE AT BORE PIT 03+80.
 IF FAILS TO SHOW THE TREE IN THE BULB OUT. THE PLAN
 NEEDS TO BE BASED ON THE SURVEY REQUIRED IN COMMENT #1.
 THE CLOSEST EDGE OF THE EXCAVATION FOR BORE PIT 03+80
 MUST SHOW A DIMENSION OF AT LEAST THREE FEET FROM THE
 TRUNK OF THE ADJACENT PALM TREE AND TEN FEET FROM THE
 TRUNK OF THE ADJACENT TREE. THE BORE MACHINE MAY NOT
 STAGE ADJACENT TO EITHER TREE.
  
 6) THE CLOSEST EDGE OF THE EXCAVATION FOR HANDHOLE
 05+80 MUST SHOW A DIMENSION OF AT LEAST THREE FEET FROM
 THE TRUNK OF THE ADJACENT PALM TREE. THE BORE MACHINE
 MAY NOT STAGE ADJACENT TO THE PALM TREE.
  
 7) HANDHOLE 14+88 NEEDS TO BE MOVED OUT OF THE BULBOUT.
 THIS BULBOUT IS RESERVED FOR LANDSCAPE ONLY. NOTE: THE
 POLE ASSOCIATED WITH THIS LOCATION (DOWNTOWN WPB 30)
 HAS BEEN RELOCATED OUT OF THE BULB OUT AS WELL.
  
 8) THE CLOSEST EDGE OF THE EXCAVATION FOR HANDHOLE
 20+64 MUST SHOW A DIMENSION OF AT LEAST THREE FEET FROM
 THE TRUNK OF THE ADJACENT PALM TREE. THE BORE MACHINE
 MAY NOT STAGE ADJACENT TO THE PALM TREE.
  
 9) THE BORE DEPTH IS SUFFICIENT TO AVOID TREE ROOTS.
