 PRINTED NAME AND SIGNATURE PER 2017 WPB AMENDMENTS TO
 THE FLORIDA BUILDING CODE, CHAPTER 1, SECTION 107.2.1.

 AND ACCOMPANYING DATA SHALL BEAR THE NAME AND SIGNATURE
