 ADMINISTRATION
  
 2021 EXISTING BUILDING CODE. 801.3 COMPLIANCE. ALL NEW
 CONSTRUCTION ELEMENTS, COMPONENTS, SYSTEMS, AND SPACES
 SHALL COMPLY WITH THE REQUIREMENTS OF THE FLORIDA
 BUILDING CODE, BUILDING.
  
 1ST REVIEW
 DATE: TUES. FEB. 16TH/ 2021
 ACTION: DENIED
  
 1) PLEASE NOTE THE PERMIT APPLICATION UNDER THE
 DESCRIPTION OF WORK STATES THAT REPLACING CARPET WITH
 ENGINEERED HARDWOOD FLOOR. PLEASE NOTE THE SUBMITTED
 TEST REPORTS FOR EITHER PROFLEX 90 AND OR PROFLEX
 MAXXIM SIM-90 UNDERLAYMENT ARE FOR QUARRY TILE FLOORS.
 THE 2ND ISSUE WITH THE SUBMITTED TEST WERE TESTED ON A
 8 INCH CONCRETE SLAB. THE BUILDING 651 OKEECHOBEE BLVD.
 WAS BUILT WITH THE HABITAL FLOORS ALL WITH 7-INCH POST
 TENSION CONCRETE SLABS. YOU COULD USE TEST REPORTS
 TESTED ON A 7-INCH SLAB, IF TESTED WITH AN ENGINEERED
 FLOOR SYSTEM AND NO DROPPED CEILING. THE 3RD ISSUE WITH
 THE TEST REPORTS IS THE FLAME SPREAD REPORT AGAIN WAS
 TESTED WITH TILE NOT ENGINEERED WOOD FLOOR SYSTEM. WE
 WILL REQUIRE 6 INCH CONCRETE SLAB WITH THE UNDERLAYMENT
 AND ENGINEERED WOOD SYSTEM TESTED TO THE 3 TEST LISTED
 BELOW.
  
 FLOOR INSULATION (FBC-B 2020) FOR TILE, STONE, MARBLE,
 VINYL AND WOOD FLOORING ALL NEED TO HAVE HAD THE SOUND
 UNDERLAYMENT TESTED FLOOR ASSEMBLIES TO THE FOLLOWING
 STANDARDS AND MEET THE QUALIFICATIONS LISTED UNDER EACH
 OF THE STANDARDS FOR A 6 INCH CONCRETE FLOOR ASSEMBLY/
 NO DROPPED CEILING:
 PLEASE PROVIDE ALL THREE TEST REPORTS FOR TYPE I AND
 TYPE II BUILDINGS SHOWING COMPLIANCE WITH:
  
 1A) FBC-B 2020 1207.3 STRUCTURE-BORNE SOUND.
 FLOOR/CEILING ASSEMBLIES BETWEEN DWELLING UNITS OR
 BETWEEN A DWELLING UNIT AND A PUBLIC OR SERVICE AREA
 WITHIN THE STRUCTURE SHALL HAVE AN IMPACT INSULATION
 CLASS (IIC) RATING OF NOT LESS THAN 50 (45 IF FIELD
 TESTED) WHEN TESTED IN ACCORDANCE WITH ASTM E-492.
  
 1B) FBC-B 2020 1207.2 AIR-BORNE SOUND. WALLS,
 PARTITIONS AND FLOOR/CEILING ASSEMBLIES SEPARATING
 DWELLING UNITS FROM EACH OTHER OR FROM PUBLIC OR
 SERVICE AREAS SHALL HAVE A SOUND TRANSMISSION CLASS
 (STC) OF NOT LESS THAN 50 (45 IF FIELD TESTED) FOR
 AIR-BORNE NOISE WHEN TESTED IN ACCORDANCE WITH ASTM E
 90.
  
 1C) FLAME SPREAD- FBC-B 2020 603.1. EXCEPTION 2.
 INSULATION INSTALLED BETWEEN A FINISHED FLOOR AND SOLID
 DECKING WITHOUT INTERVENING AIRSPACE SHALL BE ALLOWED
 TO HAVE A FLAME SPREAD INDEX OF NOT MORE THAN 200.
  
 FLAME SPREAD INDEX. A COMPARATIVE MEASURE, EXPRESSED AS
 A DIMENSIONLESS NUMBER, DERIVED FROM VISUAL
 MEASUREMENTS OF THE SPREAD OF FLAME VERSUS TIME FOR A
 MATERIAL TESTED IN ACCORDANCE WITH ASTM E 84 OR UL 723.
 2) A TRANSMITTAL LETTER LISTING THE ORIGINAL REVIEW
 COMMENT NUMBER, WITH A DESCRIPTION OF THE REVISION
 MADE, IDENTIFYING THE SHEET OR SPECIFICATION PAGE WHERE
 THE CHANGES CAN BE FOUND WILL HELP TO EXPEDITE YOUR
