 A. SHEET 27 DOES NOT SHOW THE RIGHT-OF-WAY LINE IN THE
 SAME LOCATION AS THE SURVEY BOTH ALONG THE M CANAL AND
 2130 ECHO LAKE DRIVE. ONCE THE RIGHT-OF-WAY LINE IS
 CORRECTLY SHOWN, IT APPEARS THAT IT WILL CLEARLY VERIFY
 THAT THE FIBER INSTALLATION AND THE HANDHOLE ARE WITHIN
 THE CONGRESS AVENUE RIGHT-OF-WAY, WHICH WILL BE SUBJECT
 TO PALM BEACH COUNTY RIGHT-OF-WAY APPROVAL ONLY.
  
 B. 4441 BEACON CIRCLE IS PRIVATE PROPERTY. THE HANDHOLE
 AND FIBER INSTALLATION MAY ONLY OCCUR ON PRIVATE
 PROPERTY WITHIN A UTILITY EASEMENT (AND THEN IS NOT
 SUBJECT TO THE RIGHT-OF-WAY PERMIT APPLICATION). NO
 VERIFICATION OF THE UTILITY EASEMENT SHOWN ON THE
 ENGINEERING PLAN WAS PROVIDED ON THE SURVEY. IF THERE

 INSTALLED WITHIN THE LOMBARD STREET PARCEL. IF THERE IS
 NO UTILITY EASEMENT HERE, THEN THE INSTALL MUST BE DONE

 2) SURVEY -

 RIGHT-OF-WAY (AND THUS SUBJECT TO PALM BEACH COUNTY
 PERMITTING) OR THE CITY'S CANAL RIGHT-OF-WAY AND
 SUBJECT TO THIS APPLICATION.
  
 B. 4441 BEACON CIRCLE IS PRIVATE PROPERTY. THE
 ENGINEERING PLAN SHOWS THAT THE HANDHOLE AND FIBER
 INSTALLATION IS ON THIS PRIVATE PROPERTY WITHIN A
 UTILITY EASEMENT. THE SURVEY FAILS TO SHOW A UTILITY

 VERIFICATION WAS PROVIDED THAT YOU HAVE PERMISSION TO
 ENCROACH ON CITY PROPERTY.
  
 B. 2130 ECHO LAKE DRIVE AND 4441 NORTH CONGRESS AVENUE

 3) TIMETABLE-? YOU ARE REQUIRED TO PROVIDE THE
 ESTIMATED TIMETABLE FOR CONSTRUCTION OF THE PROJECT OR
 EACH PHASE THEREOF.
  
 4) EVIDENCE OF INSURANCE COVERAGE - PROVIDE EVIDENCE OF
 THE INSURANCE COVERAGE REQUIRED UNDER SECTION 78-406 OF
 THE CITY CODE.
  
 5) POLE STYLE - PLEASE NOTE THAT THE FUTURE POLES ARE
 REQUIRED TO MEET THE CITY CONCEALMENT STANDARDS
 OUTLINED IN SECTION 78-411 OF THE CITY CODE.
  
 6) ELECTRONIC FORMAT OF SUBMITTAL - A COPY OF ALL OF
 THE SUBMITTAL MATERIAL MUST BE SUBMITTED IN ELECTRONIC
