 PLEASE ADDRESS THE ITEMS NOTED BELOW:
  
 1) FBC 107.2.1 - INFORMATION ON CONSTRUCTION DOCUMENTS.
 ALL INFORMATION, DRAWINGS, SPECIFICATIONS AND
 ACCOMPANYING DATA SHALL BEAR THE NAME AND SIGNATURE OF
 THE PERSON RESPONSIBLE FOR THE DESIGN.
  
 2) THE VALUE STATED ON THE APPLICATION APPEARS TO BE
 LOW. SUBMIT A COPY OF THE SIGNED CONTRACT TO VERIFY THE
 COSTS. FBC 109.3 BUILDING PERMIT VALUATION. FOR
 PERMITTING PURPOSES, VALUATION OF BUILDINGS AND SYSTEMS
 SHALL BE TOTAL REPLACEMENT VALUE TO INCLUDE STRUCTURAL,
 ELECTRIC, PLUMBING, MECHANICAL, INTERIOR FINISH,
 RELATIVE SITE WORK, ARCHITECTURAL AND DESIGN FEES,
 MARKETING COSTS, OVERHEAD AND PROFIT; EXCLUDING ONLY
 LAND VALUE. VALUATION REFERENCES MAY INCLUDE THE LATEST
 PUBLISHED DATA OF NATIONAL CONSTRUCTION COST ANALYSIS
 SERVICES (MARSHALL-SWIFT, MEANS, ETC.), AS PUBLISHED BY
 INTERNATIONAL CODE CONGRESS. FINAL BUILDING PERMIT
 VALUATION SHALL BE SET BY THE BUILDING OFFICIAL.
  
 3) SUBMIT TWO SETS OF THE TEST REPORT DOCUMENTS FOR THE
 SOUND INSULATION THAT WILL BE USED UNDER THE FLOOR
 TILE.
 PROVIDE THE FOLLOWING FOR FLOORING SUBMITTALS.
 DESCRIPTION OF EXISTING SITE CONDITIONS:
 (A) THICKNESS OF SLAB (B) FLOOR/CEILING ASSEMBLY BELOW
 (EXPOSED CONCRETE, SUSPENDED GYPSUM OR ACOUSTICAL ).
 (B) PROPOSED SOUND UNDERLAYMENT.
 (C) TEST REPORTS FOR THE PROPOSED UNDERLAYMENT BASED ON
 THE EXISTING SITE CONDITIONS.
 (D) TEST REPORTS MAY BE BASED ON THE FOLLOWING ASTM'S:
 E-84, E-90 & E-492.
 (E) ALL DOCUMENTS SHALL BE SUBMITTED IN DUPLICATE.
