 (IT SPECIFICALLY STATES THAT UNDERGROUND UTILITIES AND
 IMPROVEMENTS WERE NOT LOCATED IN NOTE #3). THE PROPOSED
 POLE HAS A 10 FOOT DEEP UNDERGROUND BURIAL. THE
 INFORMATION HAS NOT BEEN PROVIDED THAT THIS WILL NOT
 IMPACT EXISTING UTILITIES (ALL OF WHICH ARE SHOWN LESS
 THAN 10 FEET IN DEPTH). THE CROSS SECTION PLANS NEED TO
 SHOW THE POLE FOUNDATION. POT HOLES ARE REQUIRED IF
 PROPOSED POLE IS WITHIN TEN FEET OF EXISTING WATER,
 SEWER OR STORM WATER FACILITIES.
  
 B. THE ENGINEERING PLAN MUST BE BASED OFF OF THIS
 SURVEY INFORMATION.
  
 2) PUBLIC UTILITY LETTERS, TELEPHONE, NATURAL GAS,
 ELECTRIC SERVICE, AND TELEVISION CABLE - PUBLIC UTILITY
 LETTERS NEED TO BE PROVIDED BY ANY PROVIDER WITHIN FIVE
 (5) FEET OF THE PROPOSED POLE (AND THE BORE AND BORE
 PITS IF INCLUDED ON THIS APPLICATION). PLEASE NOTE THAT
 AS STATED PREVIOUSLY, THE UNDERGROUND INFORMATION WAS
 NOT PROVIDED ON THE SURVEY. THE ENGINEERING PLAN STATES
 THAT THE CONTRACTOR IS TO FIELD VERIFY UNDERGROUND
 UTILITIES; HOWEVER, THE CITY CODE REQUIRES THAT THIS BE
 PROVIDED PRIOR TO THE CITY APPROVAL. IF THERE ARE NO
 UNDERGROUND UTILITIES IN THE AREA, THIS NEEDS TO BE
 CLEARLY STATED ON THE PLANS AND HOW THIS INFORMATION

 INSTALLED (I.E. ANTICIPATED CONSTRUCTION METHODS AND/OR
 TECHNIQUES), IDENTIFYING ANY DEVIATION FROM THE CITY'S
 ENGINEERING STANDARDS. PLANS SHALL SHOW THE LIMITS
 SIDEWALK/PAVER RESTORATION.
  
 3) SURVEY OF UNDERGROUND FACILITIES - THE BASE OF THE
 POLE IS BEING INSTALLED UNDERGROUND SO THIS IS REQUIRED

 A. PROVIDE A SURVEY THAT IDENTIFIES EXISTING
 UNDERGROUND COMMUNICATION AND UTILITY FACILITIES,
 INCLUDING WATER, SEWER, STORM WATER, ELECTRIC,
 TELEPHONE, GAS, ETC., EXISTING IN THE AREAS TO WHICH
 THE PERMIT APPLICATION APPLIES, AND EXTENDING 50 FEET
 BEYOND SAID AREAS WITHIN THE CITY IF AVAILABLE (SUCH
 INFORMATION MAY BE PROVIDED WITHOUT CERTIFICATION AS TO
 CORRECTNESS, TO THE EXTENT OBTAINED FROM OTHER PERSONS
 WITH FACILITIES IN THE PUBLIC RIGHTS-OF-WAY). SUCH
 SURVEY MUST SHOW VALVE BOX LOCATIONS AND WATER METER

 BEING INSTALLED ABOVEGROUND SO THIS IS REQUIRED (NO
 SURVEY PROVIDED).
  
 A. IDENTIFY ALL ABOVEGROUND FEATURES TO INCLUDE TREES.
  
 B. IDENTIFY ALL ABOVEGROUND COMMUNICATION FACILITIES IN
 THE AREA TO INCLUDE AN INVENTORY OF THE REGISTRANT'S
 EXISTING FACILITIES - NO INFORMATION ON EXISTING
 FACILITIES WAS PROVIDED. SHOW THIS INFORMATION ON THE
 PLANS AND PROVIDE A NARRATIVE. IF THERE ARE NO
 FACILITIES, THIS SHALL BE NOTED ON THE SURVEY.
  
 C. YOU ARE REQUIRED TO PROVIDE INFORMATION REGARDING
 EXISTING UTILITY POLES (INCLUDING LIGHT POLES) WITHIN
 50 FEET OF YOUR PROPOSED POLE. THE POLE HEIGHT AND BASE
 DIAMETER SHALL BE INCLUDED. PLEASE NOTE: THE
 SURROUNDING POLE HEIGHT WILL BE USED TO VERIFY IF THE
 SUPPORT POLE INCLUDED WITH THIS APPLICATION, WHICH IS
 30+ FEET (ABOVE GRADE) IN HEIGHT, COMPLIES WITH THE
 HEIGHT STANDARDS LISTED IN SECTION 78-411(G)(1) OF THE
 CITY CODE.
  
 D. THE SURVEY NEEDS TO PROVIDE THE SIZE OF THE PROPOSED
 SUPPORT STRUCTURE (POLE) BASE DIAMETER AND THE HEIGHT
 OF THE POLE.
  
 E. THE ENGINEERING PLAN MUST BE BASED OFF OF THIS
 SURVEY INFORMATION.
  
 5) CONCEALMENT METHODS DESCRIPTIONS - NO CONCEALMENT
 INFORMATION WAS PROVIDED. PLEASE NOTE THAT ATTACHING
 THE METER SOCKET, THE CONDUIT, THE EQUIPMENT ENCLOSURE,
 THE SMALL CELL ANTENNA, ETC. TO THE EXTERIOR OF THE
 POLE AS SHOWN ON THE PROVIDED DRAWINGS DOES NOT MEET
 THE CITY CONCEALMENT STANDARDS OUTLINED IN SECTION
 78-411 OF THE CITY CODE. PROVIDE REVISED PLANS AND
 NARRATIVE ON HOW THE INSTALLATION COMPLIES WITH THE
 CITY'S CONCEALMENT REQUIREMENT. ALL POLE DIMENSIONS
 MUST BE PROVIDED.
  
 6) RADIO FREQUENCY INTERFERENCE - NO INFORMATION WAS
 PROVIDED THAT THE PROPOSED FACILITIES WILL NOT CAUSE
 RADIO FREQUENCY INTERFERENCE WITH THE CITY'S PUBLIC
 SAFETY COMMUNICATIONS EQUIPMENT. PROVIDE A STATEMENT
 REGARDING RADIO FREQUENCY INTERFERENCE.
  
 7) EVIDENCE OF INSURANCE COVERAGE - PROVIDE EVIDENCE OF
 THE INSURANCE COVERAGE REQUIRED UNDER SECTION 78-406 OF

 APPLICATION LISTS CELLCO PARTNERSHIP, A DELAWARE
 PARTNERSHIP (VERIZON WIRELESS) AS THE REGISTRANT, THIS
 IS WHOSE CERTIFICATE OF LIABILITY INSURANCE NEEDS TO BE
 PROVIDED.
  
 B. PLEASE REMEMBER THAT THE CERTIFICATE OF LIABILITY
 INSURANCE NEEDS TO NAME THE CITY OF WEST PALM BEACH,
 ITS COMMISSIONERS, OFFICERS, EMPLOYEES AND AGENTS AS
 ADDITIONAL INSURED AS WELL AS CHECK THE "ADDITIONAL
 INSURED" COLUMN.
  
 C. ONCE THE CORRECT CERTIFICATE OF LIABILITY INSURANCE

 NINE MONTHS AFTER THE DATE THE APPLICATION IS APPROVED
 IS REQUIRED. PROVIDE THE REQUIRED ATTESTATION.
  
 9) PUBLIC UTILITY LETTERS, TELEPHONE, NATURAL GAS,
 ELECTRIC SERVICE, AND TELEVISION CABLE - THE PUBLIC
 UTILITY LETTERS WERE NOT PROVIDED.
  
 10) ELECTRONIC FORMAT OF SUBMITTAL - THE SUBMITTAL WAS
