 1) SCOPE OF WORK DESCRIBES THIS AS A PARTIAL DEMO. WHAT
 PART IS THE PARTIAL? PROVIDE PLANS TO MAKE CLEAR, HOW
 MUCH OF THE BUILDING WILL BE DEMOLISHED AND HOW MUCH
 SAVED. IF IT IS ONLY A PARTIAL HOW WILL THE SAVED END
 BE SECURED, RECONSTRUCTED?
  
 2) THERE ARE TWO BATHROOMS IN THE BUILDING PER OUR
 RECORDS. WE WILL NEED A SEWER LATERAL CAPPING PERMIT
 PULLED, LATERAL CAPPED, AND PASSED #703 INSPECTION
 PRIOR TO DEMO.
  
 3) NO PAPERWORK CONFIRMATION FROM UTILITIES THAT THE
 THEY RECEIVED A REQUEST FROM YOU TO PULL THE WATER
 METER OR METERS. THIS FORM CAN BE FOUND ON LINE OR IN
 OUR OFFICE BEHND THE FRONT COUNTER. IT WILL NEED TO BE
 SENT TO UTILITIES. THEY WILL PROVIDE CONFIRMATION BACK
 TO YOU AND THAT SHOULD BE INCLUDED IN THE DEMO
 APPLICATION.
  
 4) A COPY OF THE N.O.C. SHOULD BE INCLUDED.
  
 5) A DEBRIS DISPOSAL FORM FILLED OUT AND COPY INCLUDED
 IN APPLICATION.
  
 5) THE DEMO APPLICATION WILL NEED TO BE ROUTED TO
 ENGINEERING AND HISTORIC. I WILL SEND IT TO ENGINEERING
 FROM HERE.
