| AND LOCATION OF NEW CONSTRUCTION AND EXISTING |
| STRUCTURES ON THE SITE, DISTANCES FROM LOT LINES, THE |
| ESTABLISHED STREET GRADES AND THE PROPOSED FINISHED |
| GRADES AND, AS APPLICABLE, FLOOD HAZARD AREAS, |
| FLOODWAYS, AND DESIGN FLOOD ELEVATIONS; AND IT SHALL BE |
