 1) THE ADDITION OF THE STRUCTURE TO THE SITE REQUIRES
 AN AMENDMENT TO THE APPROVED SITE PLAN THROUGH THE
 MINOR PLANNED DEVELOPMENT AMENDMENT PROCESS. AN
 APPLICATION FOR THE AMENDMENT IS REQUIRED TO BE
 SUBMITTED TO THE PLANNING DIVISION PRIOR TO THE
 APPROVAL OF THIS PERMIT.
  
 2) PROVIDE TWO COPIES OF A SURVEY SHOWING THE LIMITS OF
 WORK WHICH SHOWS THE DIMENSIONS TO THE PROPERTY LINE,
 THE EXISTING STRUCTURE TO THE SOUTH AND THE PARKING LOT
 CURB LINES.
