 SEALED BY DERYL ZIMMERER, PE DO NOT APPEAR TO INCLUDE A
 TITLE BLOCK COMPLIANT WITH FS471 AND FAC61G15-23.002.
  
 IF THE ENGINEER IS PRACTICING ENGINEERING SERVICES
 THROUGH THE COMPANY WHICH DOES HAVE A TITLE BLOCK ON
 THE PAGES (AND IS REFERRED TO AS THE COMPANY PREPARING
 THE PLANS IN THE LETTER FROM EOR), A CERTIFICATE OF
 AUTHORIZATION IS REQUIRED AND THAT NUMBER IS REQUIRED
 TO BE INCLUDED IN THE TITLE BLOCK.
  
 IF THE ENGINEER IS PRACTICING ENGINEERING AS AN
 INDIVIDUAL AND NOT THROUGH CECO CONCRETE CONSTRUCTION,
 A TITLE BLOCK FOR THE ENGINEER IS REQUIRED.
  
 2. THE ENGINEER'S REVIEW LETTER REFERS TO SHORING &
 RESHORING PLANS PREPARED BY CECO CONCRETE CONSTRUCTION,
 LLC. I WAS UNABLE TO LOCATE AN ENGINEERING LICENSE FOR
 CECO CONCRETE CONSTRUCTION, LLC; IT DOES NOT APPEAR
 THAT THIS COMPANY IS LICENSED TO PREPARE
 SHORING/RESHORING ENGINEERING. PLEASE REVISE LETTER OR
 PROVIDE THE LICENSE NUMBER FOR CECO CONCRETE
 CONSTRUCTION.
