 THIS IS TO BE APPROVED BY THE BUILDING OFFICIAL.
  
 2. PLEASE PROVIDE A LETTER, OR CONTRACTORS FORM, SIGNED
 BY THE OWNER TO ACKNOWLEDGE HIS OR HER RESPONSIBILITY
 TO HAVE THE AWNING COVER REMOVED PRIOR TO ANY WIND
 EVENT WITH PROJECTED WIND SPEEDS OF 75 MPH OR GREATER.
