 BEING REVIEWED AT THIS TIME. OTHER DETAILS WILL BE
 CROSSED OFF PRIOR TO ISSUANCE UNLESS SCOPE OF WORK ON
 THE PERMIT APPLICATION IS REVISED. PLEASE REMOVE SHEETS
