 9. UPON THE ARRIVAL OF THE EXPLOSIVES, THE SITE SHALL

 UNAUTHORIZED HANDLING OR POTENTIAL THEFT OF THESE
 MATERIALS. OVERNIGHT SECURITY SHALL BE
 PROVIDED AT ALL TIMES WHEN THERE ARE NO WORKERS PRESENT

 DOCUMENTATION THAT STORAGE, HANDLING AND USE OF
 EXPLOSIVES WILL BE PERFORMED IN ACCORDANCE WITH NFPA
 495, EXPLOSIVE MATERIALS CODE.
 4. SITE PLAN WHICH INDICATES THE FINAL PREDICTED
 POSITION AND HEIGHT OF RUBBLE.
 5. SITE PLAN WHICH INDICATES THE PROPOSED MITIGATION OF
 FLYING DEBRIS FROM BOTH THE DETONATION OF EXPLOSIVES
 AND THE COLLAPSE OF THE BUILDING WITH RESPECT TO
 SURROUNDING PROPERTIES. SHOW THE TYPE AND LOCATIONS OF
 ALL PROPOSED PROTECTIVE SCREENS, MATS, AND BOXES.
 6. THE ANTICIPATED GROUND LEVEL PEAK PARTICLE VELOCITY
 FROM THE BUILDING FALL AT THE CLOSEST STRUCTURES. SHOW
 ALL CALCULATIONS OR HISTORICAL DATA. INCLUDE BOTH
 VERTICAL AND HORIZONTAL PARTICLE VELOCITY DATA.
 7. THE ANTICIPATED EFFECT OF THE EXISTING SITE
 GEOLOGICAL CONDITIONS ON THE TRANSMISSION OF VIBRATION
 TO THE NORTH AND SOUTH ALONG THE EXISTING ROCK

 3. MITIGATION PLAN FOR THE EVENT OF A PARTIAL COLLAPSE
 OF THE STRUCTURE.
 4. EVACUATION PLAN, INCLUDING A MAP OF THE EVACUATION
 AREA, WHICH ENCOMPASSES A SAFE AREA FOR COLLAPSE OF 1.5
 TIMES THE HEIGHT OF THE STRUCTURE. THE EVACUATION PLAN
 MUST ALSO ADDRESS A PARTIAL COLLAPSE.
 5. MAP WHICH IDENTIFIES AND ADDRESSES THE LIMITS OF THE
 ANTICIPATED DUST CLOUD HAZARD CREATED BY THIS IMPLOSION
 (ASSUMING NO PREVAILING WIND).

 INCLUDING COLLAPSE DUE TO WINDSTORM OR OTHER ACT OF
 NATURE DURING THIS TIME PERIOD.
 C. MOST RECENT FINANCIAL STATEMENTS OF CONTRACTOR AND
 BLASTING CONTRACTOR, EVIDENCING ABILITY TO MEET THE
 DEDUCTIBLES OR SELF-INSURED RETENTION REQUIREMENTS OF
 THE POLICIES.
 D. INSURANCE POLICIES OR RIDERS, SPECIFIC TO THIS
 PROJECT, WHICH INDEMNIFIES, BOTH INDIVIDUALLY AND IN
 TOTALITY, THE CITY OF WEST PALM BEACH, THE MAYOR AND
 CITY COMMISSIONERS, AND ALL CITY STAFF ASSOCIATED WITH
 REVIEW AND ISSUANCE OF THE DEMOLITION PERMIT FROM ANY

 EQUAL TO THE VALUE OF ALL SURROUNDING STRUCTURES FOR A
 RADIUS OF 600 FEET.
 3. BOND FOR COSTS. POSTING OF A BOND, IN AN AMOUNT TO
 BE DETERMINED, THROUGH A FLORIDA SURETY TO COVER ALL
 ADDITIONAL COSTS TO THE CITY FOR THIS PROJECT,
 INCLUDING THE COSTS TO RETAIN A DEMOLITION CONSULTANT
 AND ADDITIONAL COSTS FOR POLICE, FIRE AND OTHER PUBLIC
 SAFETY SUPPORT.
 4. EXPERIENCE. PROVIDE THE FOLLOWING:
 A. DOCUMENTATION THAT THE BLASTING SUBCONTRACTOR HAS
 SUCCESSFULLY COMPLETED PROJECTS OF SIMILAR SIZE, SCOPE
 AND COMPLEXITY, WHICH INCLUDES THE PROJECT LOCATIONS
 AND JURISDICTIONAL INFORMATION.
 B. LIST OF THE LAST FIVE IMPLOSION PROJECTS BY
 DEMOLITION CONTRACTOR THAT HAVE BEEN PERFORMED IN THE
 STATE OF FLORIDA, ALONG WITH CLIENT, MUNICIPALITY AND
 CONTACT INFORMATION.
 5. SAFETY HISTORY. PROVIDE A LIST OF EMPLOYEES AND
 WORKERS COMPENSATION STATUS, INCLUDING EMR RATINGS AND
 OSHA 300 LOGS FOR THE PAST FIVE YEARS.

 BUREAU OF ALCOHOL, TOBACCO, AND FIREARMS.
  
 3. SOUND FIRE PREVENTION METHODS AND TECHNIQUES ARE TO
 BE UTILIZED PRIOR TO, DURING, AND AFTER THE IMPLOSION.
  
 4. NO OPEN FLAMES, HEAT, SPARKS, OR FRICTION SHALL BE
 ALLOWED AT THE SITE UPON ARRIVAL OF THE EXPLOSIVES.
  
 5. "NO SMOKING" AND "NO TRESPASSING" SIGNS SHALL BE
 INSTALLED IN SUFFICIENT QUANTITIES AT THE SITE TO
 PREVENT SMOKING OR TRESPASSING AT THE SITE.
  
 6. UPON THE ARRIVAL OF THE EXPLOSIVES, THE SITE SHALL
 BE PROPERLY SECURED BY MEMBERS OF THE WEST PALM BEACH
 POLICE AND FIRE RESCUE PERSONNEL. ALSO IF DESIRED
 PRIVATE SECURITY CAN BE USED IN ADDITION TO THE
 PREVIOUS MENTIONED CITY DEPARTMENTS.
  
 7. IN SEVERAL SECTIONS OF THE DOCUMENTS, THE BUILDING
 IS CALLED THE "MODERN". THE EXISTING TOWER WAS NOT
 NAMED THE MODERN.
  
 8. IN THE SITE SPECIFIC WORK PLAN, SECTION 1.0
 REFERENCE IS MADE TO THE WORD "BRIDGE".
  
 9.. IN THE SITE SPECIFIC WORK PLAN, SECTION 6.0
 REFERENCE IS MADE TO THE "MINNEAPOLIS FIRE TEAM".
  
 10. IN THE SITE SPECIFIC WORK PLAN, SECTION 8.1.
 REFERENCES "BURNS FROM CUTTING OPERATIONS". ALL CUTTING
 AND WELDING OPERATIONS SHALL BE PERFORMED PRIOR TO THE
 LOADING OF EXPLOSIVES WITHIN THE BUILDING AND SHALL
 COMPLY WITH NFPA 51B, THE STANDARD FOR CUTTING AND
 WELDING OPERATIONS.
  
 11. IN THE SITE SPECIFIC WORK PLAN IT SHALL BE ADDED
 THAT ALL EMERGENCIES SHALL BE REPORTED TO THE EMERGENCY
 SERVICE ORGANIZATIONS (ESO'S) FIRE AND POLICE
 DEPARTMENTS BY DIALING 911.
  
 12. ON THE PERMIT AND CLEARANCES REQUIRED PAGE SECTION
 B, "EXPLOSIVES SHALL BE BROUGHT TO THE SITE NO EARLIER
 THAN 10:00 A.M.. FINAL COORDINATION MEETINGS WILL BE
 REQUIRED WITH THE POLICE AND FIRE DEPARTMENTS WITH
 REGARDS TO THE EXPLOSIVE ARRIVAL, SECURITY AND OTHER
 MATTERS. THIS MEETING WILL BE CLOSED FOR SECURITY
 PURPOSES.
  
 13. THE MATERIAL SAFETY DATA SHEETS (MSDS) SHALL BE
 MADE AVAILABLE AT THE COMMAND POST AND ALSO TO THE
 OPERATIONAL COMMANDERS OF THE EMERGENCY SERVICE
 ORGANIZATIONS (ESO'S) FIRE AND POLICE DEPARTMENTS
 OPERATING AT THE IMPLOSION EVENT.
  
 14. THE SCHEDULE CHART INDICATES TEST SHOTS BUT OTHER
 INFORMATION INDICATES NO TEST SHOTS ARE PLANNED. PLEASE
 COORDINATE INFORMATION.
  
 15. THE SAME SCHEDULING CHART INDICATES THE LOADING OF
 EXPLOSIVES WILL OCCUR BETWEEN JANUARY 25, 2010 THROUGH
 JANUARY 28, 2010. AGAIN THIS INFORMATION NEEDS TO BE
 VETTED WITH POLICE AND FIRE OFFICIALS SO PROPER
 COORDINATION CAN OCCUR.
  
 16. THIS WRITER COULD NOT LOCATE A PERMIT APPLICATION
 FROM A LICENSED GLAZING CONTRACTOR SHOULD IT BECOME
 NECESSARY TO REPAIR OR REPLACE ANY DAMAGE THAT HAS
 OCCURRED TO ANY WINDOW OR DOOR GLAZING FROM THE
 IMPLOSION TO ANY ADJACENT BUILDINGS.
  
 17. THE STATE OF FLORIDA, DEPARTMENT OF FINANCIAL
 SERVICES, DIVISION OF STATE FIRE MARSHALS, LICENSE HAS
 EXPIRED. PRIOR TO THE IMPLOSION, A RENEWED LICENSE IS
 REQUIRED.
  
 18. ALL DEBRIS FROM THE IMPLOSION SHALL BE REMOVED FROM
 THE SITE IN AN APPROVED MANNER AND BE DISPOSED OF IN
 ACCORDANCE WITH STATE AND LOCAL REQUIREMENTS.
  
 19. ONLY AUTHORIZED PERSONS ARE ALLOWED TO ENTER THE
 BUILDING WHILE THE EXPLOSIVES ARE BEING LOADED OR AFTER
 THEY HAVE BEEN LOADED INTO THE BUILDING.
  
 20. NO ONE IS TO ENTER THE SITE FOLLOWING THE IMPLOSION
 UNTIL THE SITE HAS BEEN CLEARED FOR ANY MISFIRES BY THE
 BLASTER IN CHARGE OR THE EXPLOSIVE DEMOLITION
 TECHNICIAN.
  
 21. SHOULD A FIRE OCCUR AND THE POSSIBILITY EXISTS THAT
 EXPLOSIVE MATERIAL MAY BE INVOLVED NO ATTEMPTS SHOULD
 BE MADE TO EXTINGUISH THE FIRE.
  
 22. NO TOOLS THAT PRODUCE SPARKS SHALL BE UTILIZED
 WITHIN THE VICINITY OF ANY EXPLOSIVE MATERIALS. ONLY
 NON-SPARKING TOOLS SHALL BE USED.
  
 23. NO PERSON SHALL HANDLE, USE OR TRANSPORT EXPLOSIVES
 INTO THE BUILDING THAT HAVE NOT BEEN PROPERLY TRAINED
 IN THEIR USE.
  
 24. WHEN THE EXPLOSIVES ARRIVE ON SITE THEY SHALL BE
 CLOSELY SUPERVISED TO PREVENT THE UNAUTHORIZED HANDLING
 OR POTENTIAL THEFT OF THESE MATERIALS. THE SITE WILL BE
 CONSIDERED A "NO TRESPASS" SITE EXCEPT FOR REQUIRED
 WORKERS AND EMERGENCY SERVICE ORGANIZATION (ESO)

 25. A SUITABLE SYSTEM SHALL BE IN PLACE TO ENSURE THE
