 COPY OF THE APPROVED THRESHOLD INSPECTION PLAN AS WELL
 AS A COPY OF THE APPROVED INSPECTOR AGREEMENT. BOTH ARE
 TO BE APPROVED BY KEN CONRAD PRIOR TO PERMIT ISSUANCE.

 IS THE EOR FOR THE PROJECT FAC61G15-31.
  
 8. PLEASE NOTE THAT THE ELEVATOR SHAFT PERMIT IS
 EXPIRED, 06090238. THIS ISSUE DOES NOT AFFECT THIS
 PERMIT APPLICATION BUT MAY AFFECT THE OTHER
 APPLICATIONS PENDING AS THE ELEVATOR IS THE ACCESSIBLE
