 NEW 2004 FLORIDA BUILDING CODE.
  
 2) HOWEVER, YOU MAY SUBMIT A STATE
 APPROVED PRODUCT,ALONG WITH THE STATE
 COVER SHEET/FL#/TEST REPORTS AND
 INSTALLATION REPORTS.
  
 3) YOU MAY ALSO APPLY FOR LOCAL PRODUCT
 APPROVAL WHICH MAY TAKE A FEW WEEKS FOR
 APPROVAL. SEE APPLICAATION PROVIDED IN
 PACKAGE. THIS APPLICATION SHOULD BE
 COMPLETED BY THE REQUIRED PERSONEL.
  
 4) 713.13 F.S.A NOTICE OF COMMENCEMENT
 SHALL BE RECORDED AT PALM BEACH COUNTY
 COURTHOUSE AND A COPY SUBMITTED TO THIS
 OFFICE BEFORE A PERMIT CAN BE ISSUED.
 BLANK FORMS ARE AVAILABLE FROM THIS
 OFFICE.
 NOTE: THE NOTICE OF COMMENCEMENT MUST BE
 RE-RECORDED IF THE DESCRIBED IMPROVEMENT
 OR CONSTRUCTION IS NOT COMMENCED WITHIN
