 1.) PER PAGE 2 OF THE 2021 ZONING BOARD OF APPEALS
 (ZBA) APPLICATION, ALL ITEMS SHALL BE SUBMITTED IN HARD
 COPY FORMAT AND ELECTRONIC FORMAT. A DISC/FLASH DRIVE
 CONTAINING THE ELECTRONIC FILES OF ALL PLANS AND
 DOCUMENTS WERE NOT INCLUDED IN THE SUBMITTAL PACKET.
 PLEASE PROVIDE.
  
 2.) COMPLETED PROPERTY OWNER'S CONSENT FORM IS MISSING
 FROM THE SUBMITTAL (REFERENCE PAGE 15 OF THE ZBA
 APPLICATION). THE FORM MUST BE SIGNED BY AN AUTHORIZED
 PERSON OF ATB MANAGEMENT, LLC (PROPERTY OWNER) AS
 PROVIDED ON SUNBIZ, AND AUTHORIZING CARLOS RAMIREZ
 PERMISSION TO ACT AS AGENT FOR THE ZBA PROCESS.
  
 3.) JUSTIFICATION STATEMENT IS INADEQUATE.
 JUSTIFICATION STATEMENT MUST CLEARLY STATE THE PURPOSE
 OF THE APPLICATION, HOW THE PROJECT DOES NOT MEET THE
 STANDARDS OF THE ZONING AND LAND DEVELOPMENT
 REGULATIONS (ZLDRS) WHICH REQUIRE THE APPLICANT TO SEEK
 THE CLASS B AND/OR VARIANCES.
  
 4.) STANDARDS FOR CODE COMPLIANCE IS MISSING FROM THE
 APPLICATION SUBMITTAL. THE FOLLOWING STANDARDS SHALL BE
 ADDRESSED AND SUBMITTED AS PART OF THE ZBA APPLICATION
 (REFERENCE PAGE 7 OF THE ZBA APPLICATION):
 * SEC. 94-36(E)(3) - GENERAL USE STANDARDS
 * SEC. 94-36(E)(4) - SPECIFIC USE STANDARDS
 * SEC. 94-36(E)(5) - RESIDENTIAL DISTRICT STANDARDS
 * SEC. 94-303 - ACCESSORY APARTMENTS PERMITTED WITH
 EXTRA REQUIREMENTS
 * SEC. 94-273(A)(2) - WAIVER STANDARDS
  
 5.) PER PAGE 4 OF THE ZBA APPLICATION, AN ADDRESS LIST,
 RADIUS MAP SHALL BE PROVIDED BY THE PALM BEACH COUNTY
 PROPERTY APPRAISER'S OFFICE. ONCE THE ADDRESS LIST AND
 RADIUS MAP HAS BEEN OBTAINED, THE ADDRESS LIST
 AFFIDAVIT PROVIDED ON PAGE 10) SHALL BE COMPLETED AND
 NOTARIZED. IF THERE ARE UNDER 50 ADDRESSES, THE
 ENVELOPES SHALL ALSO BE PROVIDED TO STAFF. THE
 REQUIREMENT FOR THE ADDRESS LIST, RADIUS MAP AND SIGNED
 AND NOTARIZED AFFIDAVIT HAS NOT BEEN PROVIDED WITH THE
 APPLICATION.
  
 6.) ASIDE FROM THE ZBA APPLICATION REQUEST(S), THE SITE
 PLAN MUST DEMONSTRATE FULL COMPLIANCE WITH THE CITY'S
 ZONING AND LAND DEVELOPMENT REGULATIONS (ZLDRS);
 OTHERWISE, ADDITIONAL VARIANCES, ETC. MAY BE REQUIRED.
 NOT ALL DIMENSIONS HAVE BEEN PROVIDED IN THE SITE PLAN
 TO DEMONSTRATE FULL COMPLIANCE. PLEASE REVISE THE SITE
 PLAN TO PROVIDE ALL SETBACK DIMENSIONS THAT MEET THE
 MINIMUM REQUIREMENTS OF THE ZLDRS.
  
 7.) THE MINIMUM 10 FT. CORNER SIDE SETBACK HAS NOT BEEN
 MET (9.7 FT. PROVIDED IN THE SITE PLAN). THEREFORE, A
 VARIANCE MUST BE REQUESTED IN ADDITION TO THE CLASS B
 SPECIAL USE PERMIT FOR THE ACCESSORY DWELLING UNIT. THE
 REQUIREMENT AND FEES FOR THE VARIANCE PROCESS IS
 PROVIDED IN THE ZBA APPLICATION; A HARDSHIP MUST BE
 DEMONSTRATED WITH THE VARIANCE. ALTERNATIVELY, CAN THE
 CORNER SIDE SETBACK BE ADJUSTED TO MEET THE MINIMUM
 CORNER SIDE SETBACK REQUIREMENT?
  
 8.) PER PAGE 10 OF THE ZBA APPLICATION, PICTURES FROM
 ALL ANGLES OF THE SUBJECT PROPERTY AND LETTERS OF
 SUPPORT FROM THE SURROUNDING NEIGHBORS SHALL BE
 PROVIDED.
  
 9.) SUBMITTED HARD COPY PLAN IN 8.5 INCHES X 11 INCHES
 IS OF POOR QUALITY AND IS VERY DIFFICULT TO READ. IT
 MAY BE HELPFUL TO PRINT THE PLANS FROM THE ORIGINAL
 DRAWING DOCUMENT FOR CLARITY.
