 POLICY REGARDING EXTENSION OF WATER OR SEWER SERVICE
 FACILITIES OF THE CITY:
 THE WATER AND WASTEWATER SERVICE AGREEMENTS AND THE
 CONSTRUCTION PERMITS ADDRESS RESERVATION OF CAPACITY
 BASED ON USAGE, METER SIZE AND FEES, DESIGN STANDARDS,
 BACKFLOW PREVENTION, FIRE SERVICE AND IRRIGATION.
 4. AS OF DECEMBER 20, 2015, THE CITY OF WEST PALM BEACH
 CROSS-CONNECTION CONTROL MANUAL HAS BEEN REVISED,
 STATING "ALL BACKFLOW DEVICES MUST BE LEAD-FREE". FOR

 5. PLEASE SUBMIT A FORMAL AS-BUILT REQUEST FOR ALL CITY
 OF WEST PALM BEACH-OWNED UTILITIES WITHIN THE PUBLIC
 RIGHT OF WAY FOR THE ENTIRE EXTENT OF THE LIMITS OF
 YOUR PROJECT. IDENTIFY TYPE OF UTILITY, LOCATION, PIPE
 MATERIAL, AGE AND SIZE. PLEASE CONTACT JANICE TARBILL
 IN THE WPB ENGINEERING SERVICES DEPARTMENT AT

 ASSISTANCE.
 6. PLANS SHALL SHOW PROPOSED LOCATIONS AND DIMENSIONS
 OF ALL WATER, STORMWATER, AND SANITARY AND SEWER LINES,
 INCLUDING STUBS TO EACH PROPERTY LINE FOR WATER AND
 SANITARY SEWERS, THE MAKE AND MODEL OF ALL VALVES,
 METERS, AND OTHERS DEVICES TO BE INSTALLED ON THE
 SYSTEM, CATCH BASINS, CULVERTS, DITCHES INCLUDING
 GRADES, PIPES SIZES, MATERIALS, ELEVATIONS,
 ASSUMPTIONS, CALCULATIONS, INVERT ELEVATIONS FOR ALL
 INLETS AND MANHOLES AND PROFILES OF SANITARY SEWER
 LINES. ALL AVAILABLE ELEVATION SHALL BE SHOWN ON THE
 PROFILE VIEW. DIRECTION OF FLOW SHALL BE SHOWN ON PLAN
 VIEW ON PLAN VIEW FOR ALL SANITARY AND STORM SEWERS.
 PLEASE SHOW SLOPE % ON ALL SANITARY LINES AND CALL OUT
 MATERIAL. ADDITIONALLY, PLAN VIEW SHALL SHOW ALL
 EXISTING AND PROPOSED UNDERGROUND UTILITY LAYOUT,
 INCLUDING FIBER OPTIC, ELECTRIC, GAS, AND CABLEVISION &
 TELECOMMUNICATION FACILITIES LOCATED ON THE
 RIGHT-OF-WAY. PROVIDE PROFILE AND CLEARANCES AT ALL
 UTILITY CROSSINGS.
 7. CUT AND CAP UTILITIES NOT IN USE, AND REMOVE ANY
 ABANDONED UTILITY LINES LARGER THAN 2 INCHES FROM CITY
 ROW.
 8. PLEASE SUBMIT COMPLETED LOS ANALYSIS SUMMARY TABLE
 DIRECTLY TO ENGINEERING (SHEET 33 OF CITY OF WPB
 DEVELOPMENT APPLICATION).
 9. UPDATE CALCULATIONS ON PLANS OF EXISTING AND
 PROPOSED DEMANDS FOR WATER & SEWER (AVERAGE, MAXIMUM
 AND PEAK) AND FIRE FLOW DEMANDS. CALCULATIONS SHALL BE
 PER FAC 64E-8 FOR WATER AND PER FAC 64-E.008 FOR SEWER.
 10. NEW CONSTRUCTION OF WATER/SEWER SHALL COMPLY WITH
 LATEST CITY STANDARD DETAILS. PLEASE UPDATE PLANS
 ACCORDINGLY.

 POINT, BUT THE CIVIL PLAN COVER SHEET SAYS ESPERANTE
 STREETSCAPE WITH A LARGER AREA ENCOMPASSING SEVERAL
 PROPERTIES ON THE PROJECT LOCATION MAP. SEVERAL SHEETS
 OF CIVIL PLAN SET ALSO MISSING OR OMITTED. IF THIS
 PROJECT IS JUST FOR 777 S FLAGLER, THEN PLEASE SUBMIT
 ALL SHEETS SHOWING 777 S FLAGLER PROPERTY.
  
 -PGD PLAN ADJACENT LAKEVIEW AVE SHOWS LIGHT POLE IN
 SIDEWALK THAT IS CALLED OUT FOR REMOVE RELOCATE ON DEMO
 PLAN.
  
 -PGD PLAN SHOWS LIGHT POLE REMAINING IN SIDEWALK JUST
 WEST OF DRIVEWAY ADJACENT LAKEVIEW AVE THAT IS NOT
 CALLED OUT ON DEMO PLAN. IS THAT CORRECT OR AS

 -PLEASE UPDATE TD4 TO CLEARLY SHOW WHAT IS HAPPENING TO
 THE TREES ADJACENT LAKEVIEW AVE. NO SYMBOL SHOWN FROM
 DISPOSITION LEGEND.
  
 -THERE IS A DOUBLE SIDED STREET SIGN ADJACENT SOUTHWEST
 CORNER OF BUILDING AND LAKEVIEW AVE. PLANS SHOULD
 ADDRESS WHAT IS HAPPENING TO IT. IF VIEW BLOCKED BY
 TREE, THEN CONSIDER ADJUSTING LOCATION IN LANDSCAPING
 AREA.
  
 -THERE IS A STREET SIGN ON NORTH SIDE OF TREE 35. WILL
 IT BE VIEWABLE TO VEHICLES WITH PROPOSED LIGHT POLE
 INSTALLED ON LL4?
  
 -SHOW FPL MANHOLE IN FRONT OF STAIRWAY ADJACENT
 LAKEVIEW AVE ON HARDSCAPE PLAN AND MAKE SURE DOES NOT
 CONFLICT WITH PROPOSED LIGHT POLE ON LL4.
  
 -HARDSCAPE PLAN IS SHOWING LIGHT POLE AND STREET SIGN
 IN PROPOSED SIDEWALK ADJACENT LAKEVIEW, BUT THEY ARE
 GETTING REMOVED OR RELOCATED.
  
 -SHOW AND LABEL ANY EXISTING OR PROPOSED SIDEWALK
 EASEMENTS. (SOME MAY BE REQUIRED DUE TO PROPOSED
 IMPROVEMENTS)
  
 -ADD A LEGEND ON PGD PLAN SHOWING WHAT THE DIFFERENT
 SHADED AREAS ARE.
  
 -THERE IS A WATER WELL VAULT SHOWED IN PROPOSED
 SIDEWALK. IF INTENDED TO BE WALKED ON, RECOMMEND
 CHECKING FURTHER TO MAKE SURE THAT THE VAULT IS IN
 SUITABLE CONDITION AND THAT IT CAN BE ADJUSTED TO MATCH
 PROPOSED FINISHED GRADE.
  
 -PGD PLAN SHOWS DOORS OPENING INTO AREAS THAT LOOK LIKE
 LANDSCAPING ON THE NORTH SIDE OF BUILDING. IS THAT
 CORRECT; AND WILL THERE BE ELEVATION DIFFERENCE BETWEEN
 DOOR AND LANDSCAPING GRADE?
  
 -ARE ANY ADDITIONAL SEWER CLEANOUTS NEEDED? IF SO, SHOW
 ON UTILITY PLAN.
  
 -FIRELINE IS NOT COMPLIANT WITH BACKFLOW PROTECTION.
 NEEDS TO BE UPGRADED TO ABOVE-GROUND DETECTOR ASSEMBLY
 WHICH WOULD BE LOCATED ON PRIVATE PROPERTY. RECOMMEND
 INCLUDING IN THIS PROJECT AS UNDERGROUND PIPING MAY
 NEED REPLACEMENT, VAULT STRUCTURE WOULD NEED REMOVAL,
 AND WILL TAKE UP SPACE ON SITE.
  
 -WILL WATER DEMAND INCREASE? IF SO, ADD WATER DEMAND
 CALCULATIONS ON UTILITY PLAN AND ENSURE THAT EXISTING
 WATER SERVICE IS SUFFICIENT.
  
 -PLANTING PLAN IS SHOWING TWO STRM PIPES RUNNING ACROSS
 TRINITY PL THAT ARE NOT SHOWN ON CIVIL PLANS. PLEASE
 VERIFY AND MAKE SURE ALL PLAN SETS MATCH.
  
 -LANDSCAPING PLANS SHOW LIGHT POLE IN PROPOSED SIDEWALK
 ALONG FLAGLER AT EAST SIDE OF BUILDING. PLEASE ADD
 CIVIL PLAN SHEETS FOR THAT AREA AND LABEL IF LIGHT POLE
 IS INTENDED TO REMAIN.
  
 -PLANS DO NOT SHOW EXISTING SIGN POLES ALONG FLAGLER.
  
 -PROPOSED SEWER LATERAL IN RIGHT OF WAY SHOULD BE AT
 LEAST 6 INCH SIZE.
  
 -DEMO PLAN SAYS FDC NEAR NORTHEAST AREA OF PROPERTY
 WILL REMAIN, BUT UTILITY PLAN MAKES IT LOOK LIKE
 MOVING. PLEASE CLARIFY ON PLANS.
  
 -PLANS SHOW STOP SIGN AT END OF CROSSWALK AT TRINITY
 AND CHASE INTERSECTION IN MIDDLE OF PEDESTRIAN TRAFFIC.
 IS IT POSSIBLE TO RELOCATE SO NOT IN PEDESTRIAN
 TRAFFIC?
  
 -PGD PLAN DOES NOT SHOW TRENCH DRAIN AT GARAGE ENTRANCE
 ON CHASE.
  
 -LH3 SHOWS AN FDC IN SIDEWALK.
  
 -PLANS SHOW ALL OF CHASE STREET WITH PAVERS, BUT DEMO
 PLAN ONLY SHOWS PORTION OF CHASE ST. BEING AFFECTED.
