 1. PLEASE CHECK TO SEE IF FDEP FORMS FOR PROPOSED
 SANITARY SEWER ARE REQUIRED.
 2. COORDINATE WITH MARK HURLEY, FIELD CUSTOMER SERVICE
 SUPERVISOR WITH CITY OF WPB PUBLIC UTILITIES, REGARDING
 PLACEMENT OF BACKFLOW DEVICES AND WATER METERS. CONTACT

 3. FOR FORMAL ENGINEERING SUBMITTAL, SFWMD ERP AND
 NPDES NOI REQUIREMENTS WILL BE ENFORCED.
 - AN NOI IS REQUIRED FOR PROJECTS THAT:
 - 1) DISTURB ONE OF MORE ACRES OF LAND OR DISTURB LESS
 THAN ON ACRE OF LAND BUT ARE PART OF A COMMON PLAN OF
 DEVELOPMENT OR SALE; AND
 - 2) DISCHARGE STORMWATER TO SURFACE WATERS OF THE
 STATE OR TO SURFACE WATERS OF THE STATE THROUGH A
 MUNICIPAL SEPARATE STORM SEWER SYSTEM (MS4).
 - THE SFWMD ERP CAN BE OBTAINED THROUGH A 10-2 SELF
 CERTIFICATION THROUGH FDEP OR AN ERP APPLICATION
 THROUGH SFWMD. EITHER WAY, THE DEVELOPER/ENGINEER SHALL
 SUBMIT A COPY OF THE ERP PERMIT OR FDEP ACKNOWLEDGEMENT
 OF THE 10-2 CERTIFICATION. IF THE ENGINEERING PLANS DO
 NOT INCLUDE AN EROSION CONTROL PLAN, THEN THE
 CONTRACTOR SHOULD SUBMIT ONE PRIOR TO RECEIVING THE
 ENGINEERING PERMIT.
 4. FOR FORMAL ENGINEERING SUBMITTAL, PROVIDE PROFILES
 OF WATER AND SEWER AND SHOW ANY CROSSINGS/CONFLICTS.
 5. FOR FORMAL ENGINEERING SUBMITTAL, WHEN SUBMITTING
 APPROVED MATERIALS LIST, PLEASE PROVIDE MATERIAL/CUT
 SHEETS FOR ALL PROPOSED BACKFLOW DEVICES SHOWING
 SPECIFIC MODEL NUMBER AND SPECIFYING USC APPROVED
 LEAD-FREE DESIGN.
 6. PLEASE RESPOND TO ALL COMMENTS IN WRITING AND
 INCLUDE PROJECT NAME, ADDRESS AND PERMIT NUMBER
 (Z-XXXXXXXX).

 8) PROPOSED ISLAND CURB CONFLICTS WITH 12 INCH WATER
 MAIN MAINTENANCE. NEED SEVERAL FEET CLEARANCE SO WATER
 MAN CAN BE EXCAVATED.
  
 RESPONSE: 12? WATER MAIN IS BEING RELOCATED WITHIN THE
 14? WIDE LANE. WATER WILL HAVE MINIMUM COVER, SO
 EXCAVATION SHOULD NOT BE AN ISSUE.
  
 STILL SEE CONFLICT WITH EXISTING 12 INCH WATER MAIN AND
 NEW PROPOSED CURBING OF EAST ISLAND. PLEASE RESOLVE SO
 WATER MAIN CAN BE EXCAVATED VERTICALLY WITHOUT HAVING
 TO REMOVE CURBING.
  
 9) CITY WPB WATER MAINS, FORCE MAINS, FIRE HYDRANTS,
 WATER SERVICES AND METER BOXES SHOULD HAVE EXISTING OR
 PROPOSED UTILITY EASEMENTS SHOWN AND LABELED ON WATER
 PLAN. BACKFLOW ASSEMBLIES MAY NOT BE LOCATED IN UTILITY
 EASEMENTS.
  
 RESPONSE: PROPOSED EASEMENTS HAVE BEEN SHOWN OVER THE
 PROPOSED WATER MAIN
  
 PROPOSED EASEMENT BOUNDARIES ARE NOT SUFFICIENT. NEED
 UTILITY EASEMENTS FOR ALL PROPOSED AND EXISTING CITY
 WPB UTILITY INFRASTRUCTURE.

 ARE LOCATED IN LANDSCAPE AND NOT HARDSCAPE. PLEASE SHOW
 WATER METER AND BACKFLOW ASSEMBLY ON LANDSCAPE PLAN,
 AND ADD NOTE ON LANDSCAPE PLAN TO PROVIDE 18? CLEARANCE
 DISTANCE BETWEEN BUSHES AND BACKFLOW ASSEMBLY OR WATER
 METER BOX IF LOCATED IN LANDSCAPE PLANTING AREA.
