 PLEASE PROVIDE A WRITTEN RESPONSE TO THE FOLLOWING
 REVIEW COMMENTS:
  
 1. AN APPROVED TRAFFIC CONCURRENCY DETERMINATION SHALL
 BE PROVIDED FROM PALM BEACH COUNTY.
  
 2. THERE SHOULD ONLY BE ONE SITE PLAN FOR THE OVERALL
 PLANNED DEVELOPMENT, WHICH SHOULD CONTAIN ALL OF THE
 REQUIRED INFORMATION AS STATED IN THE SITE PLAN
 CHECKLIST AND WHICH WAS SHOWN ON THE PREVIOUSLY
 APPROVED SITE PLAN. THE PREVIOUS PLAN SIMPLY SHOULD
 HAVE BEEN UPDATED TO REFLECT THE PROPOSED AMENDMENT AND
 NEW CONSTRUCTION?AND THERE IS NO NEED TO DISTINGUISH
 EXISTING VS. PROPOSED (DO NOT SHOW EXISTING IN LIGHT
 GREY). THE SAME HOLDS TRUE FOR THE LANDSCAPE PLANS.
  
 3. THE SITE PLAN SHALL INDICATE THE SETBACK PROVIDED
 FOR THE BUILDING, AS MEASURED AT THE CLOSEST POINT
 BETWEEN THE BUILDING AND ALL SURROUNDING PROPERTY
 LINES.
  
 4. WHY THE RAILING ALONG PALM BEACH LAKES BOULEVARD?
 STAFF WOULD LIKE TO DISCUSS OPTIONS TO SEE IF THERE IS
 A WAY TO REMOVE THIS WHILE STILL MEETING ALL CODE
 REQUIREMENTS. WORST CASE, IF THE RAILING DOES HAVE TO
 REMAIN, A DETAIL SHALL BE PROVIDED INDICATING STYLE,
 DIMENSIONS, ETC.
  
 5. WOULDN?T IT BE MORE EFFECTIVE TO HAVE THE ALL (OR
 SOME) OF THE HANDICAP PARKING ON THE SOUTH END OF THE
 BUILDING, CLOSER TO THE MEDICAL OFFICE, SO THAT USERS
 ARE NOT REQUIRED TO CROSS THE DRIVE AISLE?
  
 6. AFTER DISCUSSIONS WITH THE CITY?S SURVEYOR,
 SUBDIVISION OF THE PROPERTY WILL REQUIRE THAT IT BE
 REPLATTED. THIS WOULD STILL GO THROUGH THE ?MINOR?
 PROCESS, BUT WOULD REQUIRE THE PREPARATION OF A NEW
 PLAT.
  
 7. AS THE PARKING SPACES ACROSS THE DRIVE AISLE ARE
 ONLY 16.5 FEET IN LENGTH, THERE IS SUPPOSED TO BE A
 2-FOOT OVERHANG OVER THE SIDEWALK THAT NEEDS TO REMAIN
 UNOBSTRUCTED. IT APPEARS THAT THE PROPOSED ADA SIGNS
 ENCROACH INTO THIS OVERHANG.
  
 8. IT IS SUGGESTED THAT THE LOADING ZONE BE SHIFTED
 SOUTHEAST TO ALLOW FOR A LANDSCAPE ISLAND BETWEEN THE
 LOADING ZONE AND THE ADJACENT SIDEWALK; THE ADJACENT
 PARKING SPACE CAN BE ELIMINATED AS THE OVERALL SITE
 EXCEEDS THE MINIMUM # OF SPACES REQUIRED.
  
 9. ADDRESS LIST AND RADIUS MAP SHALL BE PROVIDED FOR
 ALL PROPERTY OWNERS WITHIN 500 FEET OF THE ENTIRE
 PLANNED DEVELOPMENT. THIS INFORMATION IS TO BE OBTAINED
 FROM THE PALM BEACH COUNTY PROPERTY APPRAISER?S OFFICE.
 UPON SUBMITTAL TO THE PLANNING DIVISION, THE LIST AND
 MAP SHALL BE ACCOMPANIED BY THE MAILING LIST AFFIDAVIT,
 LOCATED ON PAGE 18 OF THE CITY?S DEVELOPMENT
 APPLICATION.
  
 9.1. IF THERE ARE MORE THAN 50 PROPERTY OWNERS WITHIN
 500 FEET, THE MAILER WILL BE SUPPLIED TO THE APPLICANT
 PRIOR TO PLANNING BOARD AND IT WILL BE THE APPLICANT?S
 RESPONSIBILITY TO SEND THEM OUT BEFORE THE APPLICABLE
 DEADLINE, AS WELL AS PROVIDE THE MAILER AFFIDAVIT
 LOCATED ON PAGE 12 OF THE DEVELOPMENT APPLICATION.
  
 10. UTILITY LETTERS PROVIDED ONLY INDICATE THEIR
 AVAILABILITY TO SERVE THE PROJECT. AS INDICATED IN THE
 CITY?S DEVELOPMENT APPLICATION, UTILITY LETTERS ARE TO
 INDICATE THAT ?THE PROJECT DOES NOT ENCROACH INTO THEIR
 EASEMENTS OR OTHERWISE INTERFERE WITH THE PROVISIONS OF
 THEIR SERVICES?.
  
 11. PLEASE IDENTIFY THE LOCATION OF THE PROPERTY LINE
 ON THE PHOTOMETRIC PLANS.
  
 12. DETAIL NEEDS TO BE PROVIDED FOR THE DUMPSTER
 ENCLOSURE. DIMENSIONS AND SCREENING SHALL CONFORM TO
 THE CITY?S MINIMUM REQUIREMENTS. PLEASE CONTACT ME FOR
 A COPY OF THE APPROVED DETAIL.
  
 13. BICYCLE PARKING SHALL BE PROVIDED. THE STYLE OF THE
 RACK AS WELL AS THE LOCATION AND SPACING SHALL CONFORM
 TO THE CITY?S MINIMUM REQUIREMENTS. PLEASE CONTACT ME
 FOR A COPY OF THE APPROVED DETAIL/SPECIFICATIONS.
  
 14. A 5 FOOT LANDSCAPE BUFFER SHALL BE PROVIDED BETWEEN
 THE PARKING SPACE ADJACENT TO THE LOADING ZONE AND THE
 OUTERMOST BOUNDARY OF THE PLANNED DEVELOPMENT.
  
 15. STAFF WOULD LIKE TO HAVE A DISCUSSION REGARDING THE
 CONFIGURATION OF THE PARKING, LOADING, ETC. NORTH OF
 THE RESTAURANT.
  
 16. ACCORDING TO THE FLOOR PLAN, NOT ALL INGRESS/EGRESS
 POINTS INTO THE RESTAURANT SPACE ARE SHOWN ON THE SITE
 PLAN. PLEASE ENSURE THAT ALL PLANS MATCH.

 PLEASE NOTE THE FOLLOWING WHEN RESUBMITTING:
  
 A) WRITTEN RESPONSES TO COMMENTS MADE BY ALL
 DEPARTMENTS SHALL BE PROVIDED UPON RESUBMISSION.
  
 B) 15 SETS OF REVISED PLANS AND/OR DOCUMENTS SHALL BE
 PROVIDED, ALONG WITH ONE (1) REDUCED COPY (11IN X
 17IN), AND AN ELECTRONIC COPY OF ALL PLANS.
  
 C) PLEASE NOTE THAT RE-SUBMITTALS MAY BE SUBJECT TO
 ADDITIONAL COMMENTS.
  
 D) IF NO RESPONSE IS RECEIVED WITHIN 60 DAYS OF THE
 ISSUANCE OF THESE COMMENTS, THE APPLICATION WILL BE
 CONSIDERED WITHDRAWN AND ANY FURTHER ACTION WILL
 REQUIRE SUBMISSION OF A NEW DEVELOPMENT APPLICATION.
  
 E) PLEASE NOTE THAT YOU WILL BE PERMITTED ONE (1)
 RESUBMITTAL AT NO ADDITIONAL COST. IF PREVIOUSLY-ISSUED
 COMMENTS CONTINUE TO NOT BE SUFFICIENTLY ADDRESSED, THE
 APPLICANT WILL BE ACCESSED A RESUBMITTAL FEE. SUCH FEE
 WILL BE 20% OF THE ORIGINAL APPLICATION FEE.
  
 F) IN ORDER TO BE CONSIDERED FOR THE NOVEMBER 19, 2019,
 PLANNING BOARD AGENDA, ALL COMMENTS SHALL BE
 SATISFACTORILY ADDRESSED. PLEASE SUBMIT ALL REVISED
 DOCUMENTS/PLANS NO LATER THAN SEPTEMBER 27, 2019.
  
 G) TO BE PLACED ON THE NOVEMBER 19, 2019, PLANNING
 BOARD AGENDA, SIGNS SHALL BE POSTED ON THE PROPERTY NO
 LATER THAN NOVEMBER 4, 2019. SIGNAGE SHALL CONFORM, AND
 BE INSTALLED ACCORDING TO THE REQUIREMENTS OF THE
 CITY?S ZONING AND LAND DEVELOPMENT REGULATIONS; SUCH
 REQUIREMENTS ARE ALSO PROVIDED IN THE DEVELOPMENT
 APPLICATION. A SIGNED AND NOTARIZED AFFIDAVIT SHALL BE
 PROVIDED BY THE DEADLINE REFERENCED ABOVE.
  
 H) IF AFTER THE CASE HAS BEEN ADVERTISED THE APPLICANT
 DESIRES TO REQUEST A CONTINUANCE, A FEE OF $50 PLUS THE
 COST OF ADVERTISING WILL BE CHARGED.
