 TRAFFIC PERFORMANCE STANDARDS LETTER BASED ON THE
 SQUARE FOOTAGE PROVIDED IN THE MAY 24, 2019 REVISED
 TRAFFIC IMPACT EVALUATION WAS NOT SUBMITTED.
  
 4) *** REPEAT COMMENT: THE LEGAL DESCRIPTION PROVIDED
 WITH THE SURVEY OF THE ENTIRE CAMPUS, CREATED BY DAVID
 LINDLEY (UPDATED ON JUNE 19, 2019 BASED ON CITY
 SURVEYOR REVIEW OF THE PREVIOUSLY PROVIDED INCORRECT
 LEGAL DESCRIPTION), DOES NOT MATCH THE LEGAL
 DESCRIPTION IN THE UNITY OF TITLE. THE LEGAL
 DESCRIPTION PROVIDED FOR THE UNITY OF TITLE (WHICH
 COVERS THE ENTIRE AREA FOR THE EXPANDED JFK NORTH
 CAMPUS CSPD) LISTS THE ENTIRE PARCEL AS HAVING 26.82
 ACRES, YET THE NEW SURVEY LISTS THE ENTIRE PARCEL AS
 HAVING 27.03 ACRES. STAFF WILL BE USING THE INFORMATION
 FROM THE JUNE 19, 2019 SURVEY AS THE ACREAGE FOR THE
 ENTIRE CSPD. THE DATA TABLES ON THE SITE AND LANDSCAPE
 PLANS NEED TO BASE THE DEVELOPMENT CALCULATIONS ON THE
 ACCURATE PARCEL ACREAGE OF 27.03 ACRES.
  
 5) *** NEW COMMENT: DUE TO THE EXPRESSED DESIRE TO
 INCLUDE THE REGULATIONS FOR THE FUTURE SUBDIVISION OF
 THE CSPD, IN ACCORDANCE WITH THE ZONING AND LAND
 DEVELOPMENT REGULATIONS, THE SETBACKS FOR THE CSPD ARE:
  
 25 FEET FOR THE FRONT - 45TH STREET
 25 FEET FOR THE CORNER SIDE - CONGRESS AVENUE
 15 FEET FOR THE SIDES - WEST PROPERTY LINE AND ALONG
 THE 2151 45TH STREET PARCEL
 15 FEET FOR THE REAR - NORTH PROPERTY LINE
  
 THESE SETBACK LINES NEED TO BE CORRECTED ON THE OVERALL
 SITE PLAN.
  
 6) REVISED COMMENT: THE MAY 24, 2019 JUSTIFICATION
 STATEMENT LISTS DEVELOPMENT REGULATIONS BASED ON
 WAIVERS TO PROVISIONS OF THE ZONING AND LAND
 DEVELOPMENT REGULATIONS TO PERMIT THE FUTURE
 SUBDIVISION OF THE PROPERTY WITHIN THE CSPD. NOTE: THE
 CITY WILL NOT RELEASE THE UNITY OF TITLE UNTIL THE
 APPLICATION FOR THE MINOR SUBDIVISION PLAT HAS BEEN
 SUBMITTED, REVIEWED, APPROVED AND THE PLAT IS READY FOR
 RECORDATION. STAFF HAS REVIEWED THE PROPOSED WAIVERS
 AND HAS THE FOLLOWING COMMENTS:
  
 A. INTERNAL ROW WITH MORE THAN 10 PARKING SPACES -
 EXISTING ROWS EXCEEDING 10 SPACES ARE SHOWN ON THE
 OVERALL SITE PLAN AND MAY REMAIN (EXISTING CONDITION
 WAIVER); HOWEVER, NEW OR RECONFIGURED PARKING AREAS
 MUST COMPLY WITH THE ZLDR IN PLACE AT THE TIME OF
 PERMITTING FOR THE PARKING AREA.
  
 B. LANDSCAPE BUFFER WIDTH - NO INTERNAL LANDSCAPE
 BUFFERS ARE REQUIRED FOR ANY INDIVIDUAL PARCELS THAT
 MIGHT BE CREATED VIA SUBDIVISION, EXCEPT WHERE THE
 PROPERTY LINE OF SUCH SUBDIVIDED LOT IS CONCURRENT WITH
 A CSPD PERIMETER LOT LINE. IN THAT CASE A MINIMUM FIVE
 (5) FOOT WIDE LANDSCAPE BUFFER IS REQUIRED.
  
 C. LANDSCAPE BUFFER MATERIAL - LANDSCAPE MATERIAL IS
 REQUIRED IN ANY REQUIRED LANDSCAPE BUFFER. ALL MISSING
 MATERIAL IN THE PERIMETER BUFFERS IS TO BE INSTALLED AS
 PART OF THE CSPD EXPANSION.
  
 D. FOUNDATION LANDSCAPE - STAFF WILL NOT SUPPORT ANY
 FOUNDATION PLANTING WAIVER.
  
 E. SITE IMPROVEMENTS - NEW PARCEL LINES MAY NOT CREATE
 SETBACK CONFLICTS WHERE THEY DO NOT CURRENTLY EXIST.
 NOTE: STAFF WILL SUPPORT A 5 FOOT SETBACK FOR NEW
 INTERIOR PARCEL LINES, PROVIDED THAT THE ALL FLORIDA
 BUILDING CODE REQUIREMENTS ARE MET.
  
 F. MONUMENT SIGNS - SAME AS OVERALL SITE, MAY NOT BE
 REBUILT IN THE EASEMENT.
  
 G. PARKING - CROSS PARKING IS PERMITTED WITHIN THE JFK
 NORTH COMMUNITY SERVICE PLANNED DEVELOPMENT. AN
 INDIVIDUAL PARCEL IS NOT REQUIRED TO PROVIDE ITS ENTIRE
 PARKING REQUIREMENT ON-SITE; HOWEVER, THE ENTIRE JFK
 NORTH CSPD IS REQUIRED TO MEET MINIMUM PARKING
 REQUIREMENTS. CROSS PARKING AND ACCESS AGREEMENTS MUST
 BE RECORDED IN THE PUBLIC RECORD.
  
 H. FAR - STAFF DOES NOT OBJECT TO ANY SPECIFIC PARCEL
 EXCEEDING 1.0 FAR; HOWEVER, NO INDIVIDUAL PARCEL SHALL
 NOT EXCEED 1.5 FAR.
  
 I. ALL NEW PARCELS WILL NEED TO FRONT ON A DEDICATED
 RIGHT-OF-WAY.
  
 7) *** NEW COMMENT: IN ORDER TO MITIGATE THE IMPACTS OF
 NOT PROVIDING INTERIOR PARCEL LANDSCAPE BUFFERS THE
 EXISTING PERIMETER LANDSCAPE BUFFERS MUST BE ENHANCED.
 HEDGES, CONFORMING TO THE ZLDR, MUST BE INSTALLED WHERE
 MISSING. TREES, CONFORMING TO THE ZLDR MUST BE
 INSTALLED UNLESS THE FPL EASEMENT PROHIBITS THE
 PLACEMENT OF THE TREES.
  
 8) *** NEW COMMENT: THE LANDSCAPE PLANS NEED TO BE
 REVISED TO SHOW THAT ALL THE EXISTING TERMINAL AND
 INTERIOR PARKING ISLANDS CONTAIN TREES IN ACCORDANCE
 WITH THE ZLDR AND EVIDENCE (STUMPS) THAT TREES HAVE
 BEEN REMOVED.
  
 9) *** NEW COMMENT: PARKING TABLE: THE LISTED NUMBER OF
 REQUIRED PARKING SPACES IS MORE THAN ARE REQUIRED. IT
 APPEARS THAT THE 2% ADA SPACES WERE CALCULATED IN
 ADDITION TO THE TOTAL NUMBER OF REQUIRED PARKING
 SPACES. PLEASE NOTE THAT THE CITY REQUIRES THAT THE
 NUMBER OF REQUIRED ADA SPACES (IN THIS CASE 2% OF THE
 TOTAL) TO BE PROVIDED AS A SUBSET OF THE TOTAL NUMBER
 OF REQUIRED SPACES, NOT IN ADDITION TO THE NUMBER OF
 REQUIRED SPACES. SO FOR JFK NORTH, 676 PARKING SPACES
 ARE REQUIRED (BASED ON BEDS, DOCTORS AND OFFICE SQUARE
 FOOTAGE). OF THE 676 REQUIRED PARKING SPACES, 14 ARE
 REQUIRED TO BE ADA PARKING SPACES. THE PARKING
 REQUIREMENTS PORTION OF THE TABLE NEEDS TO BE UPDATED
 ACCORDINGLY. NOTE: THE TABLE SHOWS THAT THE SITE HAS
 EXCESS PARKING PROVIDED.
  
 10) *** BICYCLE PARKING
  
 A. REVISED COMMENT: BASED ON THE 676 REQUIRED PARKING
 SPACES, ONLY 34 BICYCLE PARKING SPACES ARE REQUIRED.
 UPDATE THE TABLE ACCORDINGLY.
  
 B. REVISED COMMENT: THE TABLE LISTS 36 BICYCLE PARKING
 SPACES AS PROVIDED, YET THE PLANS LIST 40 BICYCLE
 PARKING SPACES.
  
 C. REPEAT COMMENT: ALL NEW BICYCLE RACKS MUST INCLUDE
 THE DIMENSIONS ON THE PLANS (SHEETS C002 AND C003). NO
 DIMENSIONS WERE INCLUDED FOR THE NEW BICYCLE PARKING
 PAD AT MEDICAL OFFICE BUILDING 1.
  
 D. NEW COMMENT: THE NEW BICYCLE PARKING PAD NEAR THE ER
 ENTRANCE SHOWS FIVE RACKS (10 PARKING SPACES) BUT LISTS
 EIGHT PARKING SPACES. NOTE: SEE COMMENT #10A - MANY OF
 THESE ARE NOT REQUIRED TO MEET CODE MINIMUM.
  
 11) *** REPEAT COMMENT (BASED ON LANDSCAPE PLANNER SITE
 VISIT): THE ADDITION TO THE NORTHERNMOST BEHAVIORAL
 HEALTH BUILDING HAS RESULTED IN THE RECONSTRUCTION OF
 THE CURRENT ACCESS DRIVE, INCLUDING A NEW DRAINAGE
 LINE, NORTH OF THE BUILDING. THE PLANS AS PRESENTED
 CAUSE THE REMOVAL OF MATURE TREES 311, 314 AND 315.
 THESE TREES MUST BE PROTECTED.
  
 A. SHIFT THE DRAINAGE LINE TO THE SOUTH AND DESIGN SO
 THAT THE CURB DRAIN AVOIDS TREE NUMBER 311.
 B. THE ROADWAY, CURB AND SIDEWALK SHOULD EITHER BE 1)
 SHIFTED SOUTH, 2) NARROW THE PAVEMENT WIDTH FROM 25
 FEET TO 20 FEET, 3) NARROW THE SIDEWALK TO SIX FEET, OR
 4) A COMBINATION OF THE ABOVE IN ORDER TO PERMIT TREES
 311, 314 AND 315 TO REMAIN IN PLACE.
  
 12) *** REPEAT COMMENT (FROM LANDSCAPE REVIEWER
 COMMENT): THE TWO TERMINAL ISLANDS IN THE NEW PARKING
 AREA SOUTH OF THE SOUTHERN BEHAVIORAL HEALTH BUILDING
 EXPANSION THAT WERE BUILT OVER THE EXISTING SEWER LINE
 MUST BE WIDENED TO ACCOMMODATE THE REQUIRED TERMINAL
 ISLAND TREES. THE SITE CONTAINS EXCESS PARKING SPACES,
 SO THE ISLANDS CAN BE WIDENED BY REMOVING THE ADJACENT
 PARKING SPACES. NOTE: THE PROVIDED LINE OF THE PARKING
 TABLE MUST BE UPDATED ACCORDINGLY.
  
 13) *** REPEAT COMMENT (FROM CITY'S TRANSPORTATION
 ENGINEERING DIVISION COMMENT): THE SIDEWALK CONNECTION
 TO THE WEST OF THE SOUTHERN BEHAVIORAL HEALTH BUILDING
 NEEDS TO FOLLOW THE OPTION #3 MODEL PROPOSED BY THE
 APPLICANT. HOWEVER, IT MUST NOT ONLY MEANDER AROUND
 EXISTING TREE 378, BUT ALSO AVOID EXISTING TREE 364 AS
 IT TIES INTO THE SIDEWALK ALONG THE WEST SIDE OF THE
 BUILDING.
  
 14) *** NEW COMMENT: DUE TO THE ADDITIONAL DETAIL
 PROVIDED BY THESE PLANS, STAFF HAS NOTED THAT THE
 REVISED OXYGEN TANK ENCLOSE (EXPANDED AS PART OF THE ER
 EXPANSION PROJECT) ENCROACHES INTO THE DRIVE AISLE
 AROUND THE BUILDING AND REDUCES IT TO SUBSTANDARD
 TWO-WAY DIMENSION. IT NEEDS TO HAVE AT LEAST 18 FEET OF
 TRAVEL LANE (20 FEET IS PREFERRED).
  
 15) WAIVERS REQUIRED BASED ON EXISTING CONDITIONS
 IDENTIFIED BY STAFF THAT STAFF CAN SUPPORT:
  
 A. INSTANCES OF PARKING ROWS WITH MORE THAN 10 SPACES.
 - FIVE PARKING ROWS CONTAIN MORE THAN 10 PARKING
 SPACES. ANY RECONSTRUCTION OF THESE PARKING AREAS WILL
 NEED TO MEET ZLDR REQUIREMENTS.
 B. AREAS WHERE THE MINIMUM FIVE FOOT LANDSCAPE BUFFER
 AND MATERIAL IS NOT PROVIDED.
 - TWO LOCATIONS AT THE SOUTHWEST CORNER OF THE SITE
 (DRIVEWAY AND PARKING SPACE)
 - NORTH OF THE NORTHERN CONGRESS AVENUE ENTRANCE,
 BETWEEN THE TWO PARKING BAYS AND CONGRESS AVENUE.
 BUFFER WIDTH IS AS SHOWN ON SITE AND LANDSCAPE PLANS.
 - ALONG 2145 45TH STREET
 - ALONG A PORTION OF THE WEST PROPERTY LINE, WEST OF
 THE MAIN HOSPITAL BUILDING
  
 THESE AREAS ARE SHOWN ON THE SITE AND LANDSCAPE PLANS.
 NOTE: IF ANY OF THE AREAS REFERENCED IN COMMENTS #15A
 AND #15B ARE RECONSTRUCTED, THEY MUST COMPLY WITH THE
 ZLDR IN PLACE AT THE TIME OF PERMITTING FOR THE
 RECONSTRUCTION.
  
 16) IF REBUILT, ANY SIGNS CURRENTLY LOCATED WITHIN AN
