 - THE COMMENTS SHALL BE SUFFICIENTLY ADDRESSED AND

 MAY 24, 2019 TO BE ELIGIBLE TO BE SCHEDULED FOR THE
 JULY 16, 2019 PLANNING BOARD MEETING.
  
 - PLEASE NOTE THAT IF THE PLANNING DIVISION HAS NOT
 RECEIVED A RESPONSE TO THE COMMENTS WITHIN 60 DAYS FROM
 THE DATE OF THESE COMMENTS (BY JUNE 30, 2019), THE CASE
 SHALL BE CONSIDERED WITHDRAWN. ANY FURTHER ACTION SHALL
 REQUIRE A NEW DEVELOPMENT APPLICATION.
  
 - PLEASE NOTE THAT PREVIOUSLY-ISSUED COMMENTS WERE NOT
 SUFFICIENTLY ADDRESSED, SO THE APPLICATION IS ASSESSED
 A RESUBMITTAL FEE. SUCH FEE IS 20% OF THE ORIGINAL
 APPLICATION FEE ($600).
  
 - PUBLIC HEARING NOTIFICATION SIGNAGE SHALL BE POSTED
 IN ACCORDANCE WITH THE REQUIREMENTS OF SECTION
 94-39(I)(5) OF THE ZONING AND LAND DEVELOPMENT
 REGULATIONS.
  
 1) REVISED COMMENT: THANK YOU FOR PROVIDING THE PALM
 BEACH COUNTY TRAFFIC APPROVAL LETTER; HOWEVER, THE
 FOLLOWING ITEMS NEED TO BE EXPLAINED/CORRECTED:
  
 A. THE LETTER (AND THE TRAFFIC STUDY THAT WAS
 SUBMITTED) INCLUDES THE SQUARE FOOTAGE/VEHICLE TRIPS
 FOR PCN 74434306000001070, WHICH IS NOT PART OF THE JFK
 NORTH CAMPUS CSPD. PLEASE EXPLAIN WHY THIS PARCEL WAS
 INCLUDED IN THE TRAFFIC STATEMENT AND THUS THE TRAFFIC
 APPROVAL LETTER FOR THE JFK NORTH CAMPUS CSPD. THIS
 ADDS TWO MEDICAL OFFICE BUILDINGS TOTALING 36,750
 SQUARE FEET (PAPA) TO THE TRAFFIC IMPACT OF THE JFK
 NORTH CSPD.
 B. THE TRAFFIC STUDY AND THE TRAFFIC LETTER LIST THE
 EXISTING HOSPITAL AS HAVING 124,817 SQUARE FEET, WHICH
 IS SHOWN BY PAPA AS THE SQUARE FOOTAGE OF BUILDING 1 OF
 THE JFK NORTH CAMPUS CSPD. PAPA SHOWS MULTIPLE HOSPITAL
 BUILDINGS TOTALING 194,630 SQUARE FEET. THE BUILDING
 SQUARE FOOTAGE LISTED ON SHEET C002 TOTALS 206,703
 SQUARE FEET OF EXISTING HOSPITAL. YOU NEED TO RECTIFY
 THE EXISTING HOSPITAL SQUARE FOOTAGE ON ALL PLANS AND
 DOCUMENTS. THEN THE TRAFFIC STATEMENT AND THE
 CORRESPONDING TRAFFIC LETTER NEED TO BE UPDATED TO SHOW
 ACCURATE CONDITIONS. CURRENTLY, THE EXISTING HOSPITAL
 SQUARE FOOTAGE TRAFFIC IMPACT IS SIGNIFICANTLY LESS
 THAN ACTUAL CONDITIONS ACCORDING TO THE TRAFFIC
 STATEMENT.
  
 2) NEW COMMENT: PLEASE PROVIDE THE LEGAL DESCRIPTION
 FOR THE ENTIRE NEW JFK NORTH CAMPUS CSPD BOUNDARY IN
 WORD. ADDITIONALLY, PLEASE EXPLAIN WHY THE LEGAL
 DESCRIPTION PROVIDED FOR THE UNITY OF TITLE (WHICH
 COVERS THE ENTIRE AREA FOR THE EXPANDED JFK NORTH
 CAMPUS CSPD) LISTS THE ENTIRE PARCEL AS HAVING 26.82
 ACRES, YET THE NEW SURVEY LISTS THE ENTIRE PARCEL AS
 HAVING ONLY 25.39 ACRES. THE ACREAGE AND LEGAL
 DESCRIPTIONS ON THE UNITY OF TITLE AND THE SURVEY NEED
 TO DESCRIBE THE SAME PROPERTY.
  
 3) NEW COMMENT: ALL OF THE SITE FEATURES NEED TO HAVE A
 UNIFORMLY DARK LINE WIDTH SO THAT THEY ARE LEGIBLE ON
 THE PLANS.
  
 4) REPEAT COMMENT: PROVIDE THE SETBACK DIMENSION TO THE
 EXISTING FREE STANDING MONUMENT SIGNS. PLEASE NOTE THAT
 THE SURVEY SHOWS THE TWO SIGNS ALONG 45TH STREET IN THE
 FPL EASEMENT WHICH IS NOT WHERE THEY WERE APPROVED ON
 THE BUILDING PERMIT. THE ACTUAL SIGN SETBACKS NEED TO
 BE VERIFIED.
  
 5) REVISED COMMENT: THE MARCH 22, 2019 JUSTIFICATION
 STATEMENT DOES NOT ADDRESS THE FUTURE SUBDIVISION OF
 THE PROPERTY. AT SUCH TIME AS JFK NORTH APPLIES TO
 SUBDIVIDE THE PROPERTY, A MAJOR AMENDMENT TO ADDRESS
 ALL OF THE DEVELOPMENT REGULATIONS AND ANY ASSOCIATED
 WAIVERS WILL BE REQUIRED. PLEASE NOTE THAT MITIGATION
 FOR NEWLY CREATED WAIVERS MAY BE REQUIRED WITH THE
 FUTURE MAJOR AMENDMENT.
  
 6) REVISED COMMENT: SINCE THERE IS NO SUBDIVISION, NO
 CLARIFICATION ON PARKING USE IS REQUIRED WITH THIS
 APPLICATION.
  
 7) REVISED COMMENT: THE DATA TABLES ON THE SITE AND
 LANDSCAPE PLANS NEED TO BASE THE DEVELOPMENT
 CALCULATIONS ON THE ACCURATE PARCEL ACREAGE. THE SITE
 AND LANDSCAPE PLANS DO NOT LIST THE IDENTICAL AMOUNT OF
 ACREAGE, NOR DOES EITHER ONE OF THESE MATCH THE ACREAGE
 LISTED ON EITHER OF THE LEGAL DESCRIPTIONS REFERENCED
 IN COMMENT #2.
  
 8) REPEAT COMMENT: THE SITE DATA TABLE ON SHEETS C002
 AND C003 NEEDS TO LIST THE SQUARE FOOTAGE FOR EACH
 BUILDING AND ITS USE. IF A BUILDING CONTAINS BEDS, THE
 NUMBER OF BEDS WITHIN THE BUILDING NEEDS TO BE LISTED
 AS WELL. THE TWO MEDICAL OFFICE BUILDINGS WILL NEED TO
 PROVIDE THE TOTAL SQUARE FOOTAGE OF THE BUILDINGS.
 ----- THE DATA TABLES ON THE REVISED PLANS DO NOT
 ACCURATELY REFLECT THE SQUARE FOOTAGE OF THE TWO STORY
 MEDICAL OFFICE BUILDINGS, BUT RATHER IS ONLY PROVIDED
 AS IF THEY ARE ONE STORY BUILDINGS. SEE COMMENT #1 FOR
 ADDITIONAL DISCREPANCIES WHICH NEED TO BE RECTIFIED.
 THE NUMBER OF BEDS IN EACH BUILDING WAS NOT PROVIDED SO
 THAT STAFF CAN VERIFY THE REQUIRED PARKING COUNTS.
  
 9) PARKING TABLE:
  
 A. REVISED COMMENT: STAFF HAS COUNTED 793 REGULAR
 PARKING SPACES FOR THE ENTIRE SITE, NOT 792 AS LISTED.
 PLEASE VERIFY THE COUNTS.
  
 B. NEW COMMENT: THE PARKING REQUIREMENT FOR THE TWO
 MEDICAL OFFICE BUILDINGS WAS NOT PROVIDED ON THE
 PARKING TABLE. MEDICAL OFFICE REQUIRES 1 PARKING SPACE
 PER 200 SQUARE FEET. PROVIDE A LINE ITEM AND UPDATE THE
 TOTAL REQUIRED. STAFF'S INITIAL CALCULATION SHOWS THAT
 YOU REQUIRE 846 PARKING SPACES (EXCLUDES THE AMBULANCE
 SPACES), WHILE THE PARKING TABLE ONLY SHOWS THAT THE
 PROPOSED SITE PLAN PROVIDES 826 PARKING SPACES. AN
 ADDITIONAL 20 PARKING SPACES ARE REQUIRED.
  
 10) SHEET C300 - REVISED COMMENT: I SPOKE WITH THE
 SAMANTHA HILL IN THE BUILDING DIVISION REGARDING ADA
 PARKING SPACES IN THE PARKING FIELD TO THE NORTH OF
 THIS PROPOSED BUILDING. YOU NEED TO SHOW THE LOCATIONS
 OF THE ENTRANCES TO THE BUILDING FOR BOTH PUBLIC AND
 EMPLOYEES AND THE ROUTE OF TRAVEL TO THE ENTRANCES. THE
 CLOSEST PARKING SPACES ALONG THE ROUTE NEED TO BE THE
 ADA PARKING SPACES. ---- THE REVISED SITE PLAN SHOWS A
 DOOR ON THE NORTH SIDE OF THE EXISTING BUILDING AS
 "RESTRICTED ENTRY". WHO USES THIS ENTRANCE? IF
 EMPLOYEES USE THE ENTRANCE, IT STILL REQUIRES ADA
 PARKING SPACES IN THE NEARBY PARKING FIELD AND AN
 ACCESSIBLE ROUTE FROM THE PARKING AREA TO THE BUILDING.
  
 11) SHEET C301
  
 A. REVISE COMMENT: THE NORTHERN OF THE TWO NEW WESTERN
 PARKING FIELDS STILL HAS A 22 FOOT WIDE AISLE WIDTH
 DIMENSION (SHOWN CLEARLY ON SHEET C003) AND ONE OF THE
 AISLES SHOWS 24 FEET ON SHEET C301 YET ONLY 22 FEET ON
 SHEET C003. ALL PLANS NEED TO SHOW SAME, CORRECT 24
 FOOT CONDITION.
  
 B. REVISED COMMENT: I SPOKE WITH THE SAMANTHA HILL IN
 THE BUILDING DIVISION REGARDING ADA PARKING SPACES FOR
 THE REVISED PARKING FIELD. YOU NEED TO SHOW THE
 LOCATIONS OF THE ENTRANCES TO THE BUILDING FOR BOTH
 PUBLIC AND EMPLOYEES AND THE ROUTE OF TRAVEL TO THE
 ENTRANCES. THE CLOSEST PARKING SPACES ALONG THE ROUTE
 NEED TO BE THE ADA PARKING SPACES. THIS MIRRORS HOW TO
 VERIFY THE SIMILAR BUILDING DIVISION COMMENT FROM PPRC.
 ------- NO ENTRANCES ARE SHOWN NEAR THESES PARKING
 SPACES. WHO DO THEY SERVE? YOU HAVE FAILED TO LABEL
 WHAT APPEAR TO BE DOORS IN THIS AREA. THEY NEED TO BE
 LABELED. IF THERE ARE MULTIPLE ACCESSIBLE ENTRANCES,
 THEN THE SPACES NEED TO BE DISTRIBUTED TO SERVICE THEM.
 CONVERSELY, DEPENDING ON WHAT THESE DOORS ARE USED OR,
 YOU MAY NOT NEED THE ADA PARKING SPACES AT THIS
 LOCATION.
  
 12) BICYCLE PARKING REVISED COMMENTS
  
 A. THE DATA TABLE FOR THE NUMBER OF REQUIRED BICYCLE
 PARKING SPACES NEEDS TO BE UPDATED TO REFLECT THE
 ACTUAL NUMBER OF REQUIRED PARKING SPACES FROM COMMENT
 #9B. PLEASE NOTE THAT THE CITY ONLY REQUIRES ONE
 PARKING SPACE PER 20 REQUIRED PARKING SPACES, SO YOU
 WILL REQUIRE LESS BICYCLE PARKING SPACES THAN THE 55
 LISTED.
  
 B. EVERY PRINCIPAL ENTRANCE MUST HAVE BICYCLE PARKING
 TO COMPLY WITH THE CODE. GROUPING THE MAJORITY OF THE
 BICYCLE PARKING SPACES AS SHOWN ON SHEET C301 IS NOT
 ACCEPTABLE AS NO BUILDING ENTRANCES WERE LISTED AS
 BEING AT THIS LOCATION.
  
 C. ALL NEW AND ENLARGED BICYCLE RACKS MUST INCLUDE THE
 DIMENSIONS ON THE PLANS (SHEETS C002 AND C003).
  
 D. MEDICAL OFFICE BUILDING 1 REQUIRES BICYCLE PARKING
 AT THE ENTRANCE.
  
 E. MEDICAL OFFICE BUILDING 2 HAS A WAVE STYLE BICYCLE
 RACK THAT IS NOT CALLED OUT ON THE PLANS. PROVIDE THE
 DIMENSIONS OF THE RACK AND HOW IT SITS ON THE PAD SO
 THAT STAFF CAN DETERMINE HOW MANY BICYCLES IT CAN PARK.
 THIS WILL THEN BE REFLECTED ON THE NUMBER OF SPACES
 PROVIDED TABLE.
  
 13) REPEAT COMMENT: PROVIDE THE UTILITY RELEASE
 LETTERS. THERE ARE TREES AND MODIFIED PARKING BEING
 PROPOSED FOR THE FPL EASEMENT ALONG THE EAST PROPERTY
 LINE. THIS NEEDS TO BE ADDRESSED IN THE RELEASE LETTER.
  
 14) NEW COMMENT: THE LANDSCAPE PLANS SHOW A LINE
 PATTERN THAT IS DESIGNATED AS THE "PROPERTY LINE"? AND
 A SEPARATE LINE PATTERN THAT IS DESIGNATED THE "LIMITS
 OF THE WEST PALM HOSPITAL CSPD". THESE TWO LINES MUST
 BE THE SAME.
  
 15) NEW COMMENT: THERE ARE AREAS ALONG THE CSPD
 BOUNDARY THAT DO NOT CONTAIN THE MINIMUM REQUIRED FIVE
 FOOT WIDE LANDSCAPE BUFFER. PROVIDE A FIVE FOOT OFFSET
 LINE TO SHOW THE REQUIRED BUFFER LOCATION ON THE
 LANDSCAPE PLAN. WHERE THE BUFFER IS NOT MET, PROVIDE
 THE ACTUAL DIMENSION OF THE BUFFER. STAFF NEEDS THESE
 DIMENSIONS TO ADDRESS THE EXISTING NON-CONFORMITIES IN
 THE DEVELOPMENT REGULATIONS.
  
 16) NEW COMMENT: THE LANDSCAPE PLAN FAILS TO CLEARLY
 SHOW THE 12 FOOT WIDE FPL EASEMENT ON THE NORTH, SOUTH
 AND WEST PROPERTY LINES.
  
 17) NEW COMMENT: SHEET C003 SHOWS PARKING STALLS HAVING
 A 17 FOOT DEPTH WITH A LANDSCAPE DIVIDER MEDIAN IN
 FRONT OF THEM. SOME OF THESE HAVE A 25 FOOT AND SOME
 HAVE A 24 FOOT DRIVE AISLE BEHIND THEM (SOME SHARE A 25
 FOOT DRIVE AISLE). THE ZONING AND LAND DEVELOPMENT
 REGULATIONS PERMIT AN OVERHANG INTO THE DIVIDER MEDIAN;
 HOWEVER, THE TOTAL SPACE DEPTH PLUS A TWO FOOT OVERHANG
 MUST TOTAL 18 FEET. FIELD VERIFY THE DIMENSIONS OF THE
 SPACES, INCLUDING THE WHEEL STOP LOCATION AND PROVIDE A
 DETAIL OF EACH OF THE ROWS SHOWING 17 FOOT DEEP PARKING
 SPACES. STAFF NEEDS THESE DIMENSIONS TO ADDRESS ANY
 EXISTING NON-CONFORMITIES IN THE DEVELOPMENT
 REGULATIONS.
  
 18) NO LANDSCAPE SHEET WAS PROVIDED FOR THE NORTHWEST
 QUADRANT OF THE SITE. THIS NEEDS TO BE PROVIDED AND
 CORRECTLY REFLECTED ON ALL THE LANDSCAPE DATA TABLES.
  
 19) WAIVERS REQUIRED BASED ON EXISTING CONDITIONS
 IDENTIFIED BY STAFF:
  
 A. INSTANCES OF PARKING ROWS WITH MORE THAN 10 SPACES.
 B. AREAS WHERE THE MINIMUM FIVE FOOT LANDSCAPE BUFFER
 IS NOT PROVIDED.
 C. THE REQUIRED LANDSCAPE BUFFER MATERIAL IS NOT
 PROVIDED ALONG THE NORTH, SOUTH AND WEST PROPERTY LINES
 WHERE THE FPL EASEMENT IS LOCATED.
 D. THE REQUIRED LANDSCAPE BUFFER MATERIAL OR WIDTH
