 OF ALL PLANS. PLEASE NOTE THAT CHANGES ON THE
 RESUBMITTED PLANS OR THE SUBMITTAL OF INFORMATION/PLANS
 TO ADDRESS THE COMMENTS MAY RESULT IN ADDITIONAL
 COMMENTS.
  
 - THE PPRC COMMENTS SHALL BE SUFFICIENTLY ADDRESSED AND
 SUBMITTED TO THE PLANNING DIVISION BY 5 PM ON FRIDAY,
 DECEMBER 28, 2018, TO BE ELIGIBLE TO BE SCHEDULED FOR
 THE FEBRUARY 19, 2019 PLANNING BOARD MEETING.
  
 - PLEASE NOTE THAT IF THE PLANNING DIVISION HAS NOT
 RECEIVED A RESPONSE TO THE PPRC COMMENTS WITHIN 60 DAYS
 FROM THE DATE OF THESE COMMENTS/THE PPRC MEETING (BY
 FEBRUARY 12, 2019), THE CASE SHALL BE CONSIDERED
 WITHDRAWN. ANY FURTHER ACTION SHALL REQUIRE A NEW
 DEVELOPMENT APPLICATION.
  
 - PLEASE NOTE THAT YOU WILL BE PERMITTED ONE (1)
 RESUBMITTAL AT NO ADDITIONAL COST. IF PREVIOUSLY-ISSUED
 COMMENTS CONTINUE TO NOT BE SUFFICIENTLY ADDRESSED, THE
 APPLICANT WILL BE ASSESSED A RESUBMITTAL FEE. SUCH FEE
 WILL BE 20% OF THE ORIGINAL APPLICATION FEE ($600).
  
 - PUBLIC HEARING NOTIFICATION SIGNAGE SHALL BE POSTED
 IN ACCORDANCE WITH THE REQUIREMENTS OF SECTION
 94-39(I)(5) OF THE ZONING AND LAND DEVELOPMENT
 REGULATIONS.
  
 1) PROVIDE THE PALM BEACH COUNTY TRAFFIC APPROVAL
 LETTER.
  
 2) THE SURVEY PROVIDED WITH THE APPLICATION DOES NOT
 REFLECT THE BOUNDARY OF THE WEST PALM BEACH HOSPITAL
 COMMUNITY SERVICE PLANNED DEVELOPMENT AS DESCRIBED IN
 ORDINANCE NO. 4503-14.
  
 3) PROVIDE A SITE AND LANDSCAPE PLAN FOR THE ENTIRE
 CSPD WHICH SHOWS COMPLIANCE WITH ZONING AND LAND
 DEVELOPMENT REGULATION REQUIREMENTS.
  
 4) THE SITE AND LANDSCAPE PLANS NEED TO SHOW THE
 ACCURATE CSPD BOUNDARIES AND THIS NEEDS TO BE REFLECTED
 ON THE SITE DATA TABLES FOR BOTH THE SITE AND LANDSCAPE
 PLANS.
  
 5) EACH BUILDING ON SHEET C002 NEEDS TO BE LABELED. THE
 SITE DATA TABLE ON SHEET C002 NEEDS TO LIST THE SQUARE
 FOOTAGE FOR EACH BUILDING AND ITS USE. IF A BUILDING
 CONTAINS BEDS, THE NUMBER OF BEDS WITHIN THE BUILDING
 NEEDS TO BE LISTED AS WELL.
  
 6) PARKING TABLE:
  
 A. THE PARKING COUNT IS INCORRECT FOR THE CSPD. WITHIN
 THE CSPD THERE ARE 664 REGULAR AND 30 ADA PARKING
 SPACES FOR THE PROPOSED PROJECT. THE SITE DATA TABLE
 INCORRECTLY INCLUDES SPACES FOR THE MEDICAL OFFICE
 BUILDING THAT IS OUTSIDE OF THE CSPD AND EXCLUDES SOME
 SPACES WITHIN THE CSPD.
 B. THE PARKING TABLE NEEDS TO LIST ONLY THE PARKING
 COUNTS AFTER THE PROJECT IS COMPLETED. THE EXISTING
 CONDITIONS NEED TO BE STATED IN THE JUSTIFICATION
 STATEMENT.
 C. PLEASE CONFIRM THAT THERE WILL BE NO ADDITIONAL
 DOCTORS ADDED TO PROVIDE TREATMENT FOR THE ADDITIONAL
 13,000 SQUARE FEET OF BEHAVIORAL HEALTH TREATMENT
 FACILITIES.
  
 7) BOTH THE CSPD AND THE COMMUNITY SERVICE ZONED
 PORTIONS OF THE COMMUNITY HOSPITAL OF THE PB, INC.,
 OWNED PROPERTY HAVE A UNITY OF TITLE, WHICH PERMITTED
 THE INSTALLATION OF THE SIGNS ALONG 45TH STREET. SINCE
 THE PROPERTIES ARE UNIFIED, THIS IS A CORNER LOT WITH
 45TH STREET BEING THE FRONT PROPERTY LINE AND CONGRESS
 AVENUE BEING THE CORNER SIDE PROPERTY LINE. THE SETBACK
 TABLE MUST REFLECT THIS. THE DIMENSION LINES FOR THE
 CLOSEST BUILDING POINTS TO THE PROPERTY LINE NEED TO BE
 SHOWN. NOTE: THE DIMENSION TO THE CONGRESS AVENUE
 (CORNER SIDE) DO NOT NEED TO BE SHOWN FOR THE TWO
 PROPOSED BUILDINGS, SINCE THERE ARE INTERVENING
 BUILDINGS WHICH WILL SHOW SETBACK COMPLIANT DIMENSION
 LINES.
  
 8) SHEET C300
  
 A. REMOVE THE SITE DATA TABLE. IT IS NOT REQUIRED ON
 EACH INDIVIDUAL SHEET, JUST THE OVERALL SITE PLAN.
 B. WHERE DOES THE WALKWAY BY THE DOOR ON THE WEST SIDE
 OF THE BUILDING LEAD TO? IT IS DANGEROUS TO DISCHARGE
 FOLKS INTO THE DRIVEWAY.
 C. PLEASE VERIFY WITH THE CONSTRUCTION SERVICES
 DIVISION THAT NO ADA PARKING SPACES ARE REQUIRED IN THE
 PARKING FIELD TO THE NORTH OF THIS PROPOSED BUILDING.
  
 9) SHEET C301
  
 A. REMOVE THE SITE DATA TABLE. IT IS NOT REQUIRED ON
 EACH INDIVIDUAL SHEET, JUST THE OVERALL SITE PLAN.
 B. PROVIDE DIMENSION LINES FOR ALL THE NEWLY STRIPED
 PARKING ROWS, BOTH WIDTH AND DEPTH.
 C. ONE OF THE RE-STRIPED PARKING ROWS SHOWS A PARKING
 STALL DEPTH DIMENSION OF ONLY 15 FEET. THIS NEEDS TO BE
 A MINIMUM OF 18 FEET.
 D. STRIPING NOT "A" INCORRECTLY CALLS OUT 6 INCH WHITE
 STRIPES FOR THE PARKING SPACES - THEY ARE TO BE 4
 INCHES AS SHOWN ON THE PARKING DETAIL.
 E. WHAT IS THE 19 FOOT MOUNTABLE AREA?
 F. WHY DO SOME PARKING STALLS HAVE WHEEL STOPS AND
 OTHERS NOT HAVE WHEEL STOPS. IF CURBING IS TO BE USED,
 THEN HAVE THE SPACES 16 FEET DEEP WITH A TWO FOOT OVER
 HANG.
 G. WHY ARE SOME PARKING SPACES 9 FEET WIDE AND OTHERS
 10 FEET WIDE?
 H. CLEARLY LABEL THE D CURBING.
 I. INCLUDE AISLE WIDTH DIMENSION LINES IN THE WESTERN
 PARKING FIELD THAT IS BEING RECONSTRUCTED AND
 RE-STRIPED.
 J. PROVIDE THE DRIVE AISLE WITH BEHIND THE 90 DEGREE
 PARKING INTO THE MAIN CAMPUS ROADWAY TO VERIFY THAT IT
 IS A MINIMUM OF 24 FEET WIDE AT THIS POINT.
 K. PLEASE VERIFY WITH THE CONSTRUCTION SERVICES
 DIVISION THAT THE LOCATION OF THE ADA PARKING SPACES
 COMPLIES WITH THE REQUIREMENT THAT THE ADA PARKING
 SPACES ARE CLOSEST TO THE ENTRANCE. THE TWO PROPOSED
 ADA PARKING SPACES APPEAR TO SERVE NOT BUILDING
 ENTRANCE.
  
 10) NEITHER THE SITE PLANS NOT THE ELEVATIONS SHOW ANY
 MECHANICAL EQUIPMENT. PLEASE SHOW THE MECHANICAL
 EQUIPMENT ON THE PLANS. PLEASE REMEMBER THAT BOTH
 ROOFTOP AND GROUND MOUNTED MECHANICAL EQUIPMENT MUST BE
 SCREENED.
  
 11) THE SITE PLANS SHOW THAT THE SITE IS DEFICIENT IN
 BICYCLE PARKING SO CODE COMPLIANT BICYCLE PARKING MUST
 BE PROVIDED THROUGHOUT THE CAMPUS AND THE DATA TABLES
 AND SITE DETAILS UPDATED ACCORDINGLY.
  
 12) PROVIDE THE UTILITY RELEASE LETTERS.
  
 13) THE INFORMATION ON SHEET C101 REGARDING THE REMOVAL
 OF STRIPING IS NOT REFLECTED ON SHEET C301 WHERE THIS
