 ENTIRE EXTENT OF THE LIMITS OF YOUR PROJECT. IDENTIFY
 TYPE OF UTILITY, LOCATION, PIPE MATERIAL, AGE AND SIZE.
 SUBMIT A FORMAL RECORDS REQUEST (WITH SUNSHINE DESIGN
 TICKET NUMBER) TO JANICE TARBILL IN THE WPB ENGINEERING
 SERVICES DEPARTMENT AT [email protected]. SUBMIT PROOF
 OF THE FORMAL RECORD REQUEST TO THE CITY. NOTE: RECORD
 INFORMATION PROVIDED BY THE CITY IS FOR REFERENCE ONLY.
 PERFORM FIELD VERIFICATION OF EXACT LOCATION AND DEPTH
 BY SOFT-DIGS, POT-HOLING, ETC.
  
 PROVIDE SCHEMATIC UTILITY ENGINEERING DRAWINGS (24IN X
 36IN FORMAT) AT A LEGIBLE ENGINEERING SCALE AND
 PREPARED BY A LICENSED PROFESSIONAL ENGINEER.
  
 PROVIDE STATEMENT, CONFIRMED BY ENGINEERING ANALYSIS,
 THAT THE EXISTING UTILITY MAINS SERVING THE PROPOSED
 PROJECT ARE ADEQUATE. PROVIDE CALCULATIONS OF EXISTING
 AND PROPOSED WATER & SEWER DEMANDS (AVERAGE, MAXIMUM,
 PEAK) AND FIRE FLOW DEMAND. CALCULATIONS SHALL BE PER
 FAC 64E-6 FOR SEWER AND PER FAC 64-E.008 FOR WATER.
  
 LOCATIONS, SIZES AND OTHER APPROPRIATE DESCRIPTIONS OF
 THE FOLLOWING EXISTING OR PROPOSED UTILITY FACILITIES:
