 STATUS: FAILED
  
 THIS IS AN INITIAL LEVEL REVIEW. CONSTRUCTION LEVEL
 DETAILS, DESIGN CALCULATIONS, AND THE POLLUTION
 PREVENTION PLAN WILL BE REVIEWED AS PART OF THE
 ENGINEERING DEVELOPMENT APPROVAL AND BUILDING PERMIT
 APPLICATION PROCESSES. FOLLOWING COMMENTS NEED TO BE
 ADDRESSED PRIOR TO SITE PLAN APPROVAL:
  
 1. THE ALLEYWAY ON THE WEST SIDE OF THE PROPERTY HAS AN
 EXISTING SANITARY SEWER LATERAL (APPROXIMATELY 33-FEET
 NORTH OF MANHOLE) WHICH WILL NEED TO BE CUT AND CAPPED
 AT THE MAIN;
  
 2. PLEASE ALSO SHOW ON THE LANDSCAPE PLAN, WHEN
 SUBMITTED IN THE NEXT REVIEW PHASE, SHOW THE SIGHT
 TRIANGLES FOR THE ALL VEHICULAR EXITS FROM THE SITE.
 (SECTION 94-305E, CODE OF ORDINANCES);
  
 3. ONSITE WATER AND SEWER SHALL BE PRIVATE. WATER WILL
 NEED TO BE MASTER METERED AND PROPOSED SANITARY SEWER
 WILL NEED A MANHOLE AT RIGHT-OF-WAY;
  
 4. PLEASE PROVIDE AN AUTOTURN ANALYSIS DEMONSTRATING
 THE SOLID WASTE COLLECTION VEHICLE CAN ACCESS AND
 MANEUVER WITHIN THE PROPOSED DEVELOPMENT;
  
 AFTER THE SITE PLAN IS APPROVED, IN THE NEXT PHASE, THE
 FOLLOWING GENERAL COMMENTS MUST BE ADDRESSED PRIOR TO
 ISSUANCE OF CONSTRUCTION AUTHORIZATION INCLUDING
 BUILDING PERMITS AND ENGINEERING DEVELOPMENT APPROVAL:
  
 1. AN ENGINEERING DEVELOPMENT APPROVAL APPLICATION WILL
 BE REQUIRED FROM THE ENGINEERING SERVICES DEPARTMENT.
 THE ENGINEERING DEVELOPMENT APPROVAL APPLICATION FORMS
 CAN BE DOWNLOADED FROM THE FOLLOWING WEBSITE LINK:

 MS,-PERMITS-APPLICATIONS
  
 2. PLEASE SUBMIT THE LATEST VERSION OF THE 'AUTO-CALC
 CERTIFIED COST ESTIMATE FORM' FOR SUBMISSION TO CITY OF
 WEST PALM BEACH PUBLIC WORKS DEPARTMENT FOR ADDITIONAL
 WORK PROPOSED. THIS FORM CAN BE DOWNLOADED FROM THE
 FOLLOWING LINK:

 MS,-PERMITS-APPLICATIONS
  
 3. SUBMIT WITH THE ENGINEERING DEVELOPMENT APPROVAL
 APPLICATION SIGNED & SEALED ENGINEER/MEP WATER AND
 SANITARY SEWER CALCULATION (IN DETAIL I.E. AVERAGE AND
 PEAK WITH PEAKING FACTOR). SUBMIT CALCULATIONS TO
 VALIDATE METER SIZE SELECTION. SHOW THESE CALCULATIONS
 ON THE UTILITY PLAN SHEET AS WELL;
  
 4. CCTV MUST BE PROVIDED FROM MANHOLE TO MANHOLE WHERE
 THE PROPOSED SANITARY SEWER WILL BE LOCATED. CONTACT ME
 FOR THE MANHOLE NUMBER AND SUBMIT CD WITH THE
 ENGINEERING DEVELOPMENT APPROVAL APPLICATION.
 COORDINATE WITH UTILITIES O&M SANITARY SEWER
 SUPERINTENDENT CHARLES PHOENIX PROVIDE 48-HOUR NOTICE
 AND OBTAIN A RIGHT-OF-WAY PERMIT;
  
 5. PLEASE INCLUDE THE LATEST VERSION OF THE APPLICABLE
 CITY OF WEST PALM BEACH STANDARD UTILITY DETAILS TO THE
 DETAIL SHEET/S DURING SUBMISSION OF THE FINAL
 ENGINEERING PLANS. CONTACT THE ENGINEERING SERVICES
 DEPARTMENT FOR THE LATEST REVISED DETAILS IN CAD
 FORMAT. CITY'S STANDARD DETAILS (IN PDF FILE FORMAT)
 CAN BE DOWNLOADED FROM THE FOLLOWING SITE: HTTP://WPB.O
 RG/DEPARTMENTS/ENGINEERING-PUBLIC-WORKS/FORMS,-PERMITS-
 APPLICATIONS
  
 6. PLACEMENT OF THE ANY PROPOSED METER, BACKFLOW
 PREVENTER, DCDA ETC. SHALL BE INSTALLED PER THE CITY'S
 STANDARD DETAILS. WATER METER SHOULD BE WITHIN THE
 RIGHT-OF-WAY OR INSIDE AN 'UTILITY EASEMENT' JUST
 INSIDE THE PROPERTY NEAR THE RIGHT-OF-WAY AND
 ACCESSIBLE;
  
 7. IN THE FINAL PLANS, MUST PROVIDE UTILITY CONFLICT
 TABLE (BOTTOM AND TOP OF CROSSING PIPES) AT THE
 CONFLICT POINTS OF ALL WATER, STORM AND SANITARY SEWER
 PIPES AS WELL AS OTHER FRANCHISE UTILITIES. EXISTING
 UTILITIES WITHIN THE PROPOSED SANITARY SEWER LATERAL
 ARE TO BE VERIFIED VIA SOFTDIGS, PROVIDE ALL UTILITY
 CROSSING INFORMATION;
  
 8. IN THE FINAL PLANS, SHOW DIRECTION OF FLOW FOR THE
 EXISTING SANITARY SEWER. SHOW INVERT AT CONNECTION
 POINTS AND INCLUDE A PROFILE FOR THE PROPOSED SERVICE
 LATERAL WITH GROUND ELEVATIONS ABOVE THE PIPE;
  
 9. PAVEMENT RESTORATION FOR OPEN CUT/TRENCH FOR UTILITY
 CONNECTION WORKS WILL NEED TO BE IN ACCORDANCE WITH
 2016 CITY STANDARD DETAIL GC-9. THE ROAD SHALL BE
 MILLED/RESURFACED FOR FULL LANE WIDTH OF THE TRAVEL
 LANE(S) ENCROACHED BY THE TRENCH AREA, INCLUDING A
 TRANSITION AREA OF 50 FEET ON EACH SIDE OF THE TRENCH.
 PLEASE SHOW ON THE FINAL PLANS THE EXTENT OF THE
 MILLING AND RESURFACING;
  
 10. SHOW ON THE FINAL PLANS SIZE AND MATERIAL OF ALL
 EXISTING UTILITIES ON MERCER AVENUE, ALPHA STREET AND
 ALLEWAYS INCLUDING BUT NOT LIMITED TO WATER, SEWER,
 GAS, UNDERGROUND ELECTRIC CONDUITS ETC. PLEASE IDENTIFY
 THE SOURCE OF THE AS-BUILT INFORMATION UTILIZED FOR THE
 POTABLE WATER, SANITARY SEWER, AND STORM SEWER DESIGN;
  
 11. FOR THE FINAL WATER AND SEWER PLANS, INCLUDE A NOTE
 THAT: AS OF DECEMBER 20, 2015 THE CITY OF WEST PALM
 BEACH CROSS-CONNECTION CONTROL MANUAL HAS BEEN REVISED,
 MAKING KNOWN THAT "ALL BACKFLOW DEVICES MUST BE LEAD
