 Z17040017: 711 N. ROSEMARY AVE. - RESTAURANT/RETAIL
 REDEVELOPMENT (FSPR 17-10)
  
 THIS IS AN INITIAL LEVEL REVIEW. CONSTRUCTION LEVEL
 DETAILS, DESIGN CALCULATIONS, AND THE POLLUTION
 PREVENTION PLAN WILL BE REVIEWED AS PART OF THE
 ENGINEERING DEVELOPMENT APPROVAL AND BUILDING PERMIT
 APPLICATION PROCESSES.
  
 FOLLOWING COMMENTS MUST BE ADDRESSED PRIOR TO SITE PLAN
 APPROVAL:
  
 1. SHOW ON THE SITE PLANS ADA ACCESSIBLE RAMPS ON THE
 SIDEWALK AT THE ENTRANCE AND EXIT /DRIVEWAY APPROACH;
  
 2. PLEASE SUBMIT ESTIMATES FOR THE PEAK POTABLE WATER
 AND FIRE DEMAND; AND SANITARY SEWER FLOW FOR THE
 PROPOSED RESTAURANTS.
  
 3. PLANS DO NOT CLEARLY SHOW HOW THE BUILDING WILL BE
 SERVED FOR WATER, SEWER AND FIRE PROTECTION. PLEASE
 SUBMIT PLANS SHOWING ALL EXISTING UTILITIES IN THE
 VICINITY OF THE SITE AND LABEL THEIR SIZE AND
 MATERIALS. ALSO SHOW PROPOSED WATER, FIRE LINE AND
 SANITARY SEWER TO THE BUILDING FROM CONNECTION POINTS
 OF ANY AND ALL EXISTING PIPES.
  
 4. EXISTING WATER SERVICE CONNECTIONS/WATER METERS AND
 SANITARY SEWER SERVICES SERVING THE PROPERTY, IF NOT
 USED, ARE TO BE CUT AND CAPPED AT THE MAIN.
 DE-ACTIVATED/INACTIVE PIPELINES LARGER THAN 2-INCH
 DIAMETER SHALL BE REMOVED. PLEASE LABEL PIPE SIZE AND
 MATERIAL AND ADD NOTES TO THE UTILITY PLANS
 ACCORDINGLY.
  
 5. SHOW ON THE PLANS ANY EXISTING FIRE HYDRANT CLOSEST
 TO THE SITE AND ITS DISTANCE. ALL PROPOSED FIRE
 DEPARTMENT CONNECTIONS SHALL BE WITHIN 150 FEET OF A
 FIRE HYDRANT AND SHALL BE WITHIN AN EASEMENT WITHIN THE
 PROPERTY.
  
 6. ON THE LANDSCAPING PLAN, WHEN SUBMITTED IN THE NEXT
 REVIEW PHASE, SHOW LOCATIONS OF ALL WATER METERS,
 VALVES, CLEAN-OUTS, LIGHT POLES, ETC. ROOT BARRIERS MAY
 BE REQUIRED WHERE PLANTINGS ARE NEAR UNDERGROUND
 STORMWATER, SANITARY, OR POTABLE WATER LINES. ALSO
 CLEARLY INDICATE ALL EXISTING AND PROPOSED EASEMENTS ON
 THE LANDSCAPE PLAN;
  
 7. PLEASE ALSO SHOW ON THE LANDSCAPE PLAN, WHEN
 SUBMITTED IN THE NEXT REVIEW PHASE, THE SIGHT TRIANGLES
 FOR THE ALL VEHICULAR EXITS FROM THE SITE. (SECTION
 94-305E, CODE OF ORDINANCES);
  
 8. PLEASE SUBMIT ESTIMATES FOR THE SOLID WASTE
 COLLECTION YARDAGE AT THIS STAGE. AS PART OF THE FINAL
 DESIGN, THIS SOLID WASTE ESTIMATE WILL NEED TO BE
 VERIFIED TO VALIDATE APPROPRIATE NUMBER AND SIZE OF
 DUMPSTERS/GARBAGE RECEPTACLES.
  
 9. FOR SANITATION/GARBAGE COLLECTION CONTACT THE CITY'S
 PUBLIC WORKS DEPARTMENT. SUBMIT A LETTER OF APPROVAL
 FOR SANITATION PLANS, SIZE OF GARBAGE BINS AND LOCATION
 OF THE PROPOSED DUMPSTER ENCLOSURE FROM PETER BIENIEK,
 PUBLIC WORKS DIRECTOR PRIOR TO SITE PLAN APPROVAL.
 PLEASE INCLUDE THE CITY OF WEST PALM BEACH STANDARD
 DETAIL FOR THE DUMPSTER ENCLOSURE ON THE PLANS. MINIMUM
 DIMENSIONAL (E.G. THE GATE WIDTH MUST BE A MINIMUM OF
 12.5 FEET) REQUIREMENTS FOR THE ENCLOSURE ARE LISTED IN
 CODE OF ORDINANCES, SECTION 94-444(B) OF THE CODE OF
 ORDINANCES. CITY STANDARD DETAIL GC-8 MAY BE DOWNLOADED

 10. PROVIDE AN AUTOTURN ANALYSIS DEMONSTRATING THE
 SOLID WASTE COLLECTION VEHICLE CAN ACCESS AND MANEUVER
 WITHIN THE SITE/PROPOSED DEVELOPMENT.
  
 11. A 5-FEET WIDE UTILITY EASEMENT WILL BE REQUIRED
 ALONG THE SOUTH PROPERTY LINE.
  
 AFTER THE SITE PLAN IS APPROVED, IN THE NEXT PHASE, THE
 FOLLOWING GENERAL COMMENTS MUST BE ADDRESSED PRIOR TO
 ISSUANCE OF CONSTRUCTION AUTHORIZATION INCLUDING
 BUILDING PERMITS AND ENGINEERING DEVELOPMENT APPROVAL:
  
 12. AN ENGINEERING DEVELOPMENT APPROVAL APPLICATION
 WILL BE REQUIRED FROM THE ENGINEERING SERVICES
 DEPARTMENT. THE ENGINEERING DEVELOPMENT APPROVAL
 APPLICATION FORMS CAN BE DOWNLOADED FROM THE FOLLOWING

 13. PLEASE SUBMIT THE LATEST VERSION OF THE 'AUTO-CALC
 CERTIFIED COST ESTIMATE FORM' FOR SUBMISSION TO CITY OF
 WEST PALM BEACH PUBLIC WORKS DEPARTMENT FOR ADDITIONAL
 WORK PROPOSED. THIS FORM CAN BE DOWNLOADED FROM THE

 14. PAVEMENT RESTORATION WILL BE REQUIRED AND SHALL BE
 PER THE CITY STANDARD DETAILS (GC-9 AND P-4) IF ANY
 PROPOSED WATER AND SANITARY SEWER CONNECTION WORK IS
 PERFORMED WITHIN THE RIGHT-OF-WAY;
  
 15. FIRE HYDRANT FLOW TEST DATA FROM EXISTING FIRE
 HYDRANT WILL BE REQUIRED PART OF THE PLAN SUBMITTAL;
  
 16. ANY WATERLINE THAT HAS NOT BEEN USED FOR A WHILE
 SHALL REQUIRE BAC-T TESTING PRIOR TO CONNECTION FOR NEW
 SERVICE;
  
 17. IN THE FINAL WATER AND SEWER PLANS, INCLUDE A NOTE
 THAT: AS OF DECEMBER 20, 2015 THE CITY OF WEST PALM
 BEACH CROSS-CONNECTION CONTROL MANUAL HAS BEEN REVISED,
 MAKING KNOWN THAT "ALL BACKFLOW DEVICES MUST BE LEAD
