 CHURCH PARKING GARAGE (PB CASE NO. 1231H)
  
 THIS IS AN INITIAL LEVEL REVIEW. CONSTRUCTION LEVEL
 DETAILS, DESIGN CALCULATIONS, AND THE POLLUTION
 PREVENTION PLAN WILL BE REVIEWED AS PART OF THE
 ENGINEERING DEVELOPMENT APPROVAL AND BUILDING PERMIT
 APPLICATION PROCESSES.
  
 FOLLOWING COMMENTS MUST BE ADDRESSED PRIOR TO SITE PLAN
 APPROVAL:
  
 1. WHAT IS THE INTENT OF THE SANITARY SEWER CONNECTION
 TO THE PROPOSED PARKING STRUCTURE? PLEASE EXPLAIN.
  
 2. WHAT IS THE IRRIGATION PLANS FOR THE LANDSCAPED AREA
 AROUND THE PROPOSED PARKING STRUCTURE? IS THERE A
 REASON FOR NOT KEEPING/USING THE EXISTING WATER SERVICE
 CONNECTIONS AND/OR WATER METERS, THOSE ARE SHOWN ON THE
 DEMOLITION PLANS TO BE REMOVED AND ABANDONED UNDER
 CHADBOURNE CT.?
  
 3. EXISTING WATER AND SANITARY SEWER SERVICES SERVING
 THE PROPERTY, IF NOT USED, ARE TO BE CUT AND CAPPED
 BACK TO THE MAIN. DE-ACTIVATED/INACTIVE PIPELINES
 LARGER THAN 2" DIAMETER SHALL BE REMOVED.
  
 4. PROPOSED BACKFLOW PREVENTER FOR THE 6-INCH FIRE LINE
 SHALL BE PLACED IN AN ACCESSIBLE AREA AND NOT INSIDE
 THE DETENTION BASIN.
  
 5. IF A FIRE DEPARTMENT CONNECTIONS (FDC) IS PROPOSED,
 THAT SHALL BE PLACED WITHIN 150 FEET OF A FIRE HYDRANT
 AND SHALL BE WITHIN AN EASEMENT WITHIN THE PROPERTY.
  
 6. ON THE LANDSCAPING PLAN, WHEN SUBMITTED IN THE NEXT
 REVIEW PHASE, SHOW LOCATIONS OF ALL WATER METERS,
 VALVES, CLEAN-OUTS, LIGHT POLES, ETC. ROOT BARRIERS MAY
 BE REQUIRED WHERE PLANTINGS ARE NEAR UNDERGROUND
 STORMWATER, SANITARY, OR POTABLE WATER LINES. ALSO
 CLEARLY INDICATE ALL EXISTING AND PROPOSED EASEMENTS ON
 THE LANDSCAPE PLAN;
  
 7. CONTACT THE CITY'S PUBLIC WORKS DEPARTMENT FOR
 CANCELLATION OF CURRENT SERVICES FOR SANITATION/GARBAGE
 FOR THE EXISTING APARTMENT BUILDING THOSE WILL BE
 DEMOLISHED.
 AFTER THE SITE PLAN IS APPROVED, IN THE NEXT PHASE, THE
 FOLLOWING GENERAL COMMENTS MUST BE ADDRESSED PRIOR TO
 ISSUANCE OF CONSTRUCTION AUTHORIZATION INCLUDING
 BUILDING PERMITS AND ENGINEERING DEVELOPMENT APPROVAL:
  
 8. AN ENGINEERING DEVELOPMENT APPROVAL APPLICATION WILL
 BE REQUIRED FROM THE ENGINEERING SERVICES DEPARTMENT.
 THE ENGINEERING DEVELOPMENT APPROVAL APPLICATION FORMS
 CAN BE DOWNLOADED FROM THE FOLLOWING WEBSITE LINK:

 9. PLEASE SUBMIT THE LATEST VERSION OF THE 'AUTO-CALC
 CERTIFIED COST ESTIMATE FORM' FOR SUBMISSION TO CITY OF
 WEST PALM BEACH PUBLIC WORKS DEPARTMENT FOR ADDITIONAL
 WORK PROPOSED. THIS FORM CAN BE DOWNLOADED FROM THE
 FOLLOWING LINK:

 10. FIRE HYDRANT FLOW TEST DATA FROM THE CLOSEST
 EXISTING FIRE HYDRANT WILL BE REQUIRED PART OF THE PLAN
 SUBMITTAL;
  
 11. IN THE FINAL PLANS, SHOW CONFLICT INFORMATION
 (BOTTOM AND TOP OF CROSSING PIPES) AT THE CONFLICT
 POINTS OF ALL WATER, STORM, SANITARY SEWER, IRRIGATION
 PIPES AS WELL AS OTHER FRANCHISE UTILITIES. EXISTING
 UTILITIES WITHIN THE PROPOSED WATER, SANITARY SEWER AND
 FIRE LINES ARE TO BE VERIFIED VIA SOFTDIGS.
  
 12. IN THE FINAL WATER AND SEWER PLANS, INCLUDE A NOTE
 THAT: AS OF DECEMBER 20, 2015 THE CITY OF WEST PALM
 BEACH CROSS-CONNECTION CONTROL MANUAL HAS BEEN REVISED,
 MAKING KNOWN THAT "ALL BACKFLOW DEVICES MUST BE LEAD
 FREE." FOR MORE INFORMATION REFER TO THE LINK BELOW:
