 THIS IS AN INITIAL LEVEL REVIEW. CONSTRUCTION LEVEL
 DETAILS, DESIGN CALCULATIONS, AND THE POLLUTION
 PREVENTION PLAN WILL BE REVIEWED AS PART OF THE
 ENGINEERING DEVELOPMENT APPROVAL AND BUILDING PERMIT
 APPLICATION PROCESSES.
  
 THE FOLLOWING GENERAL COMMENTS MUST BE ADDRESSED PRIOR
 TO ISSUANCE OF CONSTRUCTION AUTHORIZATION INCLUDING
 BUILDING PERMITS AND ENGINEERING DEVELOPMENT APPROVAL:
  
 1. AN ENGINEERING DEVELOPMENT APPROVAL APPLICATION WILL
 BE REQUIRED FROM THE ENGINEERING SERVICES DEPARTMENT.
 THE ENGINEERING DEVELOPMENT APPROVAL APPLICATION FORMS
 CAN BE DOWNLOADED FROM THE FOLLOWING WEBSITE LINK:

 2. PLEASE SUBMIT THE LATEST VERSION OF THE 'AUTO-CALC
 CERTIFIED COST ESTIMATE FORM' FOR SUBMISSION TO CITY OF
 WEST PALM BEACH PUBLIC WORKS DEPARTMENT FOR ADDITIONAL
 WORK PROPOSED. THIS FORM CAN BE DOWNLOADED FROM THE
 FOLLOWING LINK:

 FOLLOWING GENERAL SITE COMMENTS:
  
 3. FOR SANITATION/GARBAGE COLLECTION CONTACT THE CITY'S
 PUBLIC WORKS DEPARTMENT. FINAL APPROVED SITE PLANS NEED
 TO SHOW PROPOSED SANITATION COLLECTION PLANS AND
 DETAILS; AND WILL NEED APPROVAL FROM PETER BIENIEK,
 PUBLIC WORKS DIRECTOR. PLEASE INCLUDE THE CITY OF WEST
 PALM BEACH STANDARD DETAIL FOR THE DUMPSTER ENCLOSURE
 ON THE PLANS. MINIMUM DIMENSIONAL (E.G. THE GATE WIDTH
 MUST BE A MINIMUM OF 12.5 FEET) REQUIREMENTS FOR THE
 ENCLOSURE ARE LISTED IN CODE OF ORDINANCES, SECTION
 94-444(B) OF THE CODE OF ORDINANCES. CITY STANDARD
 DETAIL GC-8 MAY BE DOWNLOADED AT:

 4. FOR PROPOSED BRICK PAVERS WITHIN THE SIDEWALK WILL
 NEED MAINTENANCE AGREEMENT; PROPOSED SIDEWALK ON
 FLAGLER DRIVE NEED TO BE IN AN EASEMENT AS IT APPEARS
 TO BE WITHIN THE PROPERTY LINE (REFER TO ENGINEERING
 PLANS SHEET 4 OF 5);
  
 FOLLOWING ARE WATER AND SEWER COMMENTS AND NEED TO BE
 ADDRESSED PRIOR TO ENGINEERING DEPARTMENT APPROVAL:
  
 5. SUBMIT WITH THE ENGINEERING DEVELOPMENT APPROVAL
 APPLICATION SIGNED & SEALED ENGINEER/MEP WATER AND
 SANITARY SEWER CALCULATION (AVERAGE AND PEAK WITH
 PEAKING FACTOR). SUBMIT CALCULATIONS TO VALIDATE METER
 SIZE SELECTION. SHOW THESE CALCULATIONS ON THE UTILITY
 PLAN SHEET AS WELL;
  
 6. A FIRE HYDRANT FLOW TEST DATA FROM EXISTING FIRE
 HYDRANT WILL BE REQUIRED PART OF THE PLAN SUBMITTAL;
  
 7. PLEASE INCLUDE THE LATEST VERSION OF THE APPLICABLE
 CITY OF WEST PALM BEACH STANDARD UTILITY DETAILS TO THE
 DETAILS SHEET DURING SUBMISSION OF THE FINAL
 ENGINEERING PLANS. CONTACT THE ENGINEERING SERVICES
 DEPARTMENT FOR THE LATEST REVISED DETAILS IN CAD
 FORMAT. CITY?S STANDARD DETAILS (IN PDF FILE FORMAT)
 CAN BE DOWNLOADED FROM THE FOLLOWING SITE:

 8. PLACEMENT OF THE PROPOSED METER, BACKFLOW PREVENTER,
 DCDA ETC. SHALL BE INSTALLED PER THE CITY'S STANDARD
 DETAILS. THEY SHALL NOT BE INSIDE THE BUILDING
 ENCLOSURE. WATER METER SHOULD BE WITHIN THE
 RIGHT-OF-WAY OR INSIDE AN 'UTILITY EASEMENT' JUST
 INSIDE THE PROPERTY NEAR THE RIGHT-OF-WAY AND
 ACCESSIBLE;
  
 9. IN THE FINAL PLANS, SHOW CONFLICT INFORMATION
 (BOTTOM AND TOP OF CROSSING PIPES) AT THE CONFLICT
 POINTS OF ALL WATER, STORM AND SANITARY SEWER PIPES AS
 WELL AS OTHER FRANCHISE UTILITIES. EXISTING UTILITIES
 WITHIN THE PROPOSED SANITARY SEWER LATERAL ARE TO BE
 VERIFIED VIA SOFTDIGS AND PROVIDE ALL UTILITY CROSSING
 INFORMATION;
  
 10. IN THE FINAL PLANS, SHOW DIRECTION OF FLOW FOR THE
 EXISTING SANITARY SEWER. SHOW INVERT AT CONNECTION
 POINTS AND INCLUDE A PROFILE FOR THE PROPOSED SERVICE
 LATERAL WITH GROUND ELEVATIONS ABOVE THE PIPE;
  
 11. CCTV RECORD (VIDEO AND TV LOG) OF THE EXISTING
 SANITARY SEWER (FULL LENGTH FROM MH TO MH) AND NEW
 SANITARY SEWER LATERAL WILL BE REQUIRED AS PART OF THE
 PROJECT CLOSE-OUT SUBMITTALS;
  
 12. PAVEMENT RESTORATION WILL BE REQUIRED FOR THE
 PROPOSED SANITARY SEWER CONNECTION TO THE EXISTING
 MANHOLE WITHIN THE RIGHT-OF-WAY. REFER TO THE CITY
 STANDARD DETAILS FOR RESTORATION GUIDELINES;
  
 13. THERE ARE EXISTING WATER METER, WATER AND SANITARY
 SEWER SERVICE AVAILABLE FOR THE SITES. IT IS
 RECOMMENDED THAT THEY ARE UTILIZED;
  
 14. EXISTING WATER SERVICE CONNECTIONS/WATER METERS
 SERVING THE PROPERTY, IF NOT USED, ARE TO BE CUT AND
 CAPPED AT THE MAIN. DE-ACTIVATED/INACTIVE PIPELINES
 LARGER THAN 2" DIAMETER WILL BE REMOVED. PLEASE LABEL
 PIPE SIZE AND MATERIAL AND ADD NOTES TO THE UTILITY
 PLANS ACCORDINGLY;
  
 15. EXISTING SEWER SERVICE CONNECTIONS SERVING THE
 PROPERTY, IF NOT USED, ARE TO BE CUT AND CAPPED AT THE
 MAIN;
  
 16. EXISTING SANITARY SEWER MH TO BE REHABBED PER
 CITY'S REQUIREMENTS;
  
 17. SHOW SIZE AND MATERIAL OF THE PROPOSED SANITARY
 SEWER SERVICE. INCLUDE A PROFILE FOR THE PROPOSED
 SERVICE LATERAL WITH GROUND ELEVATIONS ABOVE THE PIPE;
  
 18. FOR THE FINAL UTILITY PLANS, INCLUDE A NOTE THAT:
 AS OF DECEMBER 20, 2015 THE CITY OF WEST PALM BEACH
 CROSS-CONNECTION CONTROL MANUAL HAS BEEN REVISED,
 MAKING KNOWN THAT "ALL BACKFLOW DEVICES MUST BE LEAD
 FREE." FOR MORE INFORMATION REFER TO THE LINK BELOW:

 19. ALL PROPOSED FIRE DEPARTMENT CONNECTIONS SHALL BE
 WITHIN 150 FEET OF A FIRE HYDRANT AND SHALL BE WITHIN
 AN EASEMENT WITHIN THE PROPERTY. LANDSCAPE PLAN SHALL
 SHOW LOCATIONS OF ALL WATER METERS, VALVES, CLEAN-OUTS,
 LIGHT POLES, ETC. ROOT BARRIERS MAY BE REQUIRED WHERE
 PLANTINGS ARE NEAR UNDERGROUND STORMWATER, SANITARY, OR
 POTABLE WATER LINES.
  
 20. SUBMITTAL OF MAINTENANCE OF TRAFFIC (MOT) FOR WORK
 WITHIN THE RIGHT-OF-WAY WILL BE REQUIRED AT THE
 PRE-CONSTRUCTION MEETING.
