 THIS IS AN INITIAL LEVEL REVIEW. CONSTRUCTION LEVEL
 DETAILS, DESIGN CALCULATIONS, AND THE POLLUTION
 PREVENTION PLAN WILL BE REVIEWED AS PART OF THE
 ENGINEERING DEVELOPMENT APPROVAL AND BUILDING PERMIT
 APPLICATION PROCESSES.
  
 THE FOLLOWING GENERAL COMMENTS MUST BE ADDRESSED PRIOR
 TO ISSUANCE OF CONSTRUCTION AUTHORIZATION INCLUDING
 BUILDING PERMITS AND ENGINEERING DEVELOPMENT APPROVAL:
  
 1. PLEASE PROVIDE AN ALTA/ACSM SURVEY WHICH HAS BEEN
 SIGNED AND SEALED BY PROFESSIONAL SURVEY & MAPPER
 (PSM).
  
 2. PLEASE COORDINATE WITH PALM BEACH COUNTY TO OBTAIN
 PERMIT FOR WORK TO BE PERFORMED WITHIN CONGRESS AVENUE
 RIGHT-OF-WAY INCLUDING SIDEWALK, CURBING AND CURB RAMPS
 ETC. PLEASE SUBMIT A COPY OF THE COUNTY APPROVAL OF THE
 DRIVEWAY ENTRANCE ON NORTH CONGRESS PERMIT WITH THE
 DEVELOPMENT APPLICATION TO THE CITY?S ENGINEERING
 DEPARTMENT;
  
 3. PLEASE SHOW THE SIGHT TRIANGLES ON THE PLANS FOR THE
 VEHICULAR EXITS. (SECTION 94-305E, CODE OF ORDINANCES).
  
 4. PLEASE PROVIDE AN AUTOTURN ANALYSIS DEMONSTRATING
 THE SOLID WASTE COLLECTION VEHICLE CAN ACCESS THE
 PROPOSED DUMPSTER AND COMPACTOR LOCATIONS. THE TRASH
 DUMPSTER/COMPACTORS MUST LOCATED IN AREAS EASILY
 SERVICED BY THE CITY SANITATION EQUIPMENT AND MANPOWER
 PER SECTION 74-35(A)(5) OF THE CODE OF ORDINANCES. IF A
 ROLL?OFF COMPACTOR IS PROPOSED, PLEASE PROVIDE A LETTER
 FROM THE PRIVATE PROVIDER VERIFYING THAT THEY CAN
 PROVIDE SERVICE WITH THE CONFIGURATION SHOWN ON THE
 PLANS;
  
 5. AN ENGINEERING DEVELOPMENT APPROVAL APPLICATION WILL
 BE REQUIRED FROM THE ENGINEERING DIVISION OF THE PUBLIC
 WORKS DEPARTMENT. THE ENGINEERING DEVELOPMENT APPROVAL
 APPLICATION FORMS CAN BE DOWNLOADED AT:

 6. PLEASE SUBMIT THE LATEST VERSION OF THE ?AUTO-CALC
 CERTIFIED COST ESTIMATE FORM? FOR SUBMISSION TO CITY OF
 WEST PALM BEACH PUBLIC WORKS DEPARTMENT. THIS FORM CAN
 BE DOWNLOADED FROM THE FOLLOWING LINK:

 THE FOLLOWING WATER AND SEWER COMMENTS:
  
 7. ADDRESS PREVIOUS PPRC COMMENTS FOR UTILITIES;
  
 8. CALL OUT FORCED MAIN (FM) SIZE, MATERIAL ON THE
 PLANS;
  
 9. SUBMIT WITH THE ENGINEERING DEVELOPMENT APPROVAL
 APPLICATION SIGNED & SEALED ENGINEER/MEP WATER AND
 SANITARY SEWER CALCULATION (AVERAGE AND PEAK WITH
 PEAKING FACTOR). SUBMIT CALCULATIONS TO VALIDATE METER
 SIZE SELECTION. SHOW THESE CALCULATIONS ON THE UTILITY
 PLAN SHEET AS WELL;
  
 10. A FIRE HYDRANT FLOW TEST DATA FROM EXISTING FIRE
 HYDRANT WILL BE REQUIRED PART OF THE PLAN SUBMITTAL;
  
 11. RECORD INFO SHOWS AN EXISTING 6-INCH GAS MAIN
 WITHIN THE RIGHT-OF-WAY DEDICATION. SHOW THIS ON THE
 PLANS;
  
 12. SHOW ALL EXISTING UTILITIES WITHIN THE VICINITY OF
 THE PROPOSED SITE AND PROVIDE ALL UTILITY CROSSING
 INFORMATION. SHOW CONFLICT INFORMATION (BOTTOM AND TOP
 OF CROSSING PIPES) AT THE CONFLICT POINTS OF ALL WATER,
 STORM AND SANITARY SEWER PIPES AS WELL AS OTHER
 FRANCHISE UTILITIES;
  
 13. IT IS NOT CLEAR FROM THE PLANS WHERE THE PROPOSED
 WATER LINE IS CONNECTED TO AS SHOWN ON THE PLAN (AT THE
 NORTHWEST PROPERTY LINE). THE LABEL SAYS ?CONNECT TO
 EXISTING 8? WATER MAIN? BUT RECORD INFORMATION SHOWS
 EXISTING WATER LINE IS FURTHER WEST OF THERE;
  
 14. SHOW SIZE AND MATERIAL OF THE PROPOSED WATER LINE;
  
 15. PLEASE INCLUDE THE LATEST VERSION OF THE APPLICABLE
 CITY OF WEST PALM BEACH STANDARD UTILITY DETAILS TO THE
 DETAILS SHEET. CONTACT THE ENGINEERING SERVICES
 DEPARTMENT FOR THE LATEST REVISED DETAILS IN CAD
 FORMAT. CITY?S STANDARD DETAILS (IN PDF FILE FORMAT)

 16. PLACEMENT OF THE PROPOSED METER, BACKFLOW
 PREVENTER, DCDA ETC. SHALL BE INSTALLED PER THE CITY?S
 STANDARD DETAILS. WATER METER SHOULD BE INSIDE AN
 'UTILITY EASEMENT' JUST INSIDE THE PROPERTY NEAR THE
 RIGHT-OF-WAY AND ACCESSIBLE;
  
 17. SHOW INFORMATION OF THE EXISTING MANHOLES AND
 SANITARY SEWER I.E. INVERTS, RIM ELEVATION ETC. ON THE
 UTILITY PLANS;
  
 18. EXISTING SANITARY SEWER MH TO BE REHABBED PER
 CITY?S REQUIREMENTS;
  
 19. SHOW SIZE AND MATERIAL OF THE PROPOSED SANITARY
 SEWER SERVICE. INCLUDE A PROFILE FOR THE PROPOSED
 SERVICE LATERAL WITH GROUND ELEVATIONS ABOVE THE PIPE;
  
 20. CCTV RECORD (VIDEO AND TV LOG) OF THE EXISTING
 SANITARY SEWER (FULL LENGTH FROM MH TO MH) AND NEW
 SANITARY SEWER LATERAL WILL BE REQUIRED AS PART OF THE
 PROJECT CLOSE-OUT SUBMITTALS;
  
 21. PAVEMENT RESTORATION WILL BE REQUIRED FOR THE
 PROPOSED SANITARY SEWER CONNECTION TO THE EXISTING
 MANHOLE WITHIN THE RIGHT-OF-WAY. REFER TO THE CITY
 STANDARD DETAILS FOR RESTORATION GUIDELINES;
  
 22. EXISTING WATER SERVICE CONNECTIONS/WATER METERS
 SERVING THE PROPERTY, IF NOT USED, ARE TO BE CUT AND
 CAPPED AT THE MAIN. DE-ACTIVATED/INACTIVE PIPELINES
 LARGER THAN 2" DIAMETER WILL BE REMOVED. PLEASE LABEL
 PIPE SIZE AND MATERIAL AND ADD NOTES TO THE UTILITY
 PLANS ACCORDINGLY;
  
 23. PLEASE NOTE THAT: AS OF DECEMBER 20, 2015 THE CITY
 OF WEST PALM BEACH CROSS-CONNECTION CONTROL MANUAL HAS
 BEEN REVISED, MAKING KNOWN THAT "ALL BACKFLOW DEVICES
 MUST BE LEAD FREE." FOR MORE INFORMATION REFER TO THE
 LINK BELOW:

 24. THE PROPOSED FIRE DEPARTMENT CONNECTION SHALL BE
 WITHIN 150 FEET OF A FIRE HYDRANT AND SHALL BE WITHIN
 AN EASEMENT WITHIN THE PROPERTY. LANDSCAPE PLAN SHALL
 SHOW LOCATIONS OF ALL WATER METERS, VALVES, CLEAN-OUTS,
 LIGHT POLES, ETC. ROOT BARRIERS MAY BE REQUIRED WHERE
 PLANTINGS ARE NEAR UNDERGROUND STORMWATER, SANITARY, OR
 POTABLE WATER LINES.
  
 25. SUBMITTAL MAINTENANCE OF TRAFFIC (MOT) WILL BE
 REQUIRED AT THE PRE-CONSTRUCTION MEETING.
