 THIS IS A CONCEPTUAL LEVEL REVIEW. CONSTRUCTION LEVEL
 DETAILS, DESIGN CALCULATIONS AND THE POLLUTION
 PREVENTION PLAN WILL BE REVIEWED AS PART OF THE
 ENGINEERING DEVELOPMENT APPROVAL AND BUILDING PERMIT
 APPLICATION PROCESSES. THE ENGINEERING DEVELOPMENT
 APPROVAL APPLICATION FORMS CAN BE DOWNLOADED AT
 HTTP://WPB.ORG/ENGINEERING/FORMS-AND-PUBLICATIONS/
  
 THE FOLLOWING COMMENTS MUST BE ADDRESSED PRIOR TO SITE
 PLAN APPROVAL:
  
 1. SOLID WASTE YARDAGE CALCULATION WILL NEED TO BE
 SUBMITTED AS PART OF THE FINAL DESIGN TO VERIFY THAT
 THE DESIGN INCLUDES APPROPRIATE NUMBER AND SIZE OF
 DUMPSTERS/GARBAGE RECEPTACLES. PLEASE SHOW ON THE PLAN
 IF A COMPACTOR IS PROPOSED TO REDUCE THE YARDAGE OF
 WASTE THAT WILL NEED TO BE COLLECTED;
  
 2. PLEASE PROVIDE AN AUTOTURN ANALYSIS DEMONSTRATING
 THE SOLID WASTE COLLECTION VEHICLE CAN ACCESS THE
 PROPOSED DUMPSTER AND COMPACTOR LOCATIONS. THE TRASH
 DUMPSTER/COMPACTORS MUST LOCATED IN AREAS EASILY
 SERVICED BY THE CITY SANITATION EQUIPMENT AND MANPOWER
 PER SECTION 74-35(A)(5) OF THE CODE OF ORDINANCES. IF A
 ROLL-OFF COMPACTOR IS PROPOSED, PLEASE PROVIDE A LETTER
 FROM THE PRIVATE PROVIDER VERIFYING THAT THEY CAN
 PROVIDE SERVICE WITH THE CONFIGURATION SHOWN ON THE
 PLANS;
  
 3. PLEASE INCLUDE THE CITY OF WEST PALM BEACH STANDARD
 DETAIL FOR THE DUMPSTER ENCLOSURE ON THE PLANS. MINIMUM
 DIMENSIONAL (E.G. THE GATE WIDTH MUST BE A MINIMUM OF
 12.5 FEET) REQUIREMENTS FOR THE ENCLOSURE ARE LISTED IN
 CODE OF ORDINANCES, SECTION 94-444(B) OF THE CODE OF
 ORDINANCES. CITY STANDARD DETAIL GC-8 MAY BE DOWNLOADED

 4. IF ROLL OUT TYPE DUMPSTER IS PROPOSED, PLEASE NOTE
 ON THE PLANS THAT: 'OWNER IS RESPONSIBLE TO ROLL OUT
 THE DUMPSTER OUTSIDE THE ENCLOSURE PRIOR TO CITY?S
 SANITATION CREW PICK UP GARBAGE'.
  
 5. PLEASE REVISE THE LANDSCAPE PLAN TO SHOW LOCATIONS
 OF ALL WATER METERS, VALVES, CLEAN-OUTS, LIGHT POLES,
 ETC. ROOT BARRIERS MAY BE REQUIRED WHERE PLANTINGS ARE
 NEAR UNDERGROUND STORMWATER, SANITARY, OR POTABLE WATER
 LINES;
  
 6. CITY'S GARBAGE COLLECTOR TRUCK NEEDS TO HAVE NO LESS
 THAN 14' HIGH VERTICAL CLEARANCE AND 12' HORIZONTAL
 CLEARANCES FOR THE TRUCK TO DRIVE THROUGH;
  
 THE FOLLOWING COMMENTS MUST BE ADDRESSED PRIOR TO
 ISSUANCE OF CONSTRUCTION AUTHORIZATION INCLUDING
 BUILDING PERMITS AND ENGINEERING DEVELOPMENT APPROVAL:
  
 7. AN ENGINEERING DEVELOPMENT APPROVAL APPLICATION WILL
 BE REQUIRED FROM THE ENGINEERING DIVISION OF THE PUBLIC
 WORKS DEPARTMENT. THE ENGINEERING DEVELOPMENT APPROVAL
 APPLICATION FORMS CAN BE DOWNLOADED AT:

 8. PLEASE SUBMIT THE LATEST VERSION OF THE 'AUTO-CALC
 CERTIFIED COST ESTIMATE FORM' FOR SUBMISSION TO CITY OF
 WEST PALM BEACH PUBLIC WORKS DEPARTMENT. THIS FORM CAN
 BE DOWNLOADED FROM THE FOLLOWING LINK:

 9. SUBMIT WITH THE ENGINEERING DEVELOPMENT APPROVAL
 APPLICATION SIGNED & SEALED ENGINEER/MEP WATER AND
 SANITARY SEWER CALCULATION (AVERAGE AND PEAK WITH
 PEAKING FACTOR). SUBMIT CALCULATIONS TO VALIDATE METER
 SIZE SELECTION. SHOW THESE CALCULATIONS ON THE UTILITY
 PLAN SHEET AS WELL;
  
 10. PLEASE INCLUDE APPLICABLE CITY OF WEST PALM BEACH
 STANDARD DETAILS TO THE DETAILS SHEET. CONTACT THE
 ENGINEERING SERVICES DEPARTMENT FOR THE LATEST REVISED
 DETAILS IN CAD FORMAT. CITY'S STANDARD DETAILS (IN PDF
 FILE FORMAT) CAN BE DOWNLOADED FROM THE FOLLOWING SITE:

 THE FOLLOWING COMMENTS ARE FOR WATER AND SEWER:
  
 11. PLACEMENT OF THE PROPOSED METER, BACKFLOW
 PREVENTER, DCDA ETC. SHALL BE INSTALLED PER THE CITY'S
 STANDARD DETAILS. REVISE THE LOCATION OF THE FIRE
 HYDRANT. WATER METER SHOULD BE INSIDE AN 'UTILITY
 EASEMENT' JUST INSIDE THE PROPERTY NEAR THE
 RIGHT-OF-WAY AND ACCESSIBLE;
  
 12. ENLARGE THE AREA (PREFERABLY TO A SCALE OF 1"=10')
 TO SHOW DETAILS OF THE PROPOSED 4-INCH WATER METER;
  
 13. EXISTING WATER SERVICE CONNECTIONS SERVING THE
 PROPERTY, IF NOT USED, ARE TO BE CUT AND CAPPED AT THE
 MAIN. DE-ACTIVATED/INACTIVE PIPELINES LARGER THAN 2"
 DIAMETER WILL BE REMOVED. PLEASE LABEL PIPE SIZE AND
 MATERIAL AND ADD NOTES TO THE UTILITY PLANS
 ACCORDINGLY;
  
 14. INCLUDE A PROFILE FOR THE PROPOSED SERVICE LATERAL
 WITH GROUND ELEVATIONS ABOVE THE PIPE;
  
 15. PROPOSED SANITARY SEWER MANHOLES SHALL NOT BE
 INSTALLED ON THE EXISTING SIDEWALK. INCLUDE IN THE
 PLANS CITY'S STANDARD DETAIL FOR MANHOLE ON THE DETAILS
 SHEET;
  
 16. PROVIDE UTILITY CROSSING INFORMATION. SHOW CONFLICT
 INFORMATION (BOTTOM AND TOP OF CROSSING PIPES) AT THE
 CONFLICT POINTS OF ALL WATER, STORM AND SANITARY SEWER
 PIPES AS WELL AS OTHER FRANCHISE UTILITIES;
  
 17. CCTV RECORD (VIDEO AND TV LOG) OF THE EXISTING AND
 NEW SANITARY SEWER (FULL LENGTH FROM MH TO MH) WILL BE
 REQUIRED AS PART OF THE PROJECT CLOSE-OUT SUBMITTALS;
  
 18. PAVEMENT RESTORATION WILL BE REQUIRED FOR THE
 PROPOSED SANITARY SEWER CONNECTION TO THE EXISTING
 MANHOLE WITHIN THE RIGHT-OF-WAY;
  
 19. PLEASE NOTE THAT: AS OF DECEMBER 20, 2015 THE CITY
 OF WEST PALM BEACH CROSS-CONNECTION CONTROL MANUAL HAS
 BEEN REVISED, MAKING KNOWN THAT "ALL BACKFLOW DEVICES
 MUST BE LEAD FREE." FOR MORE INFORMATION REFER TO THE

 LIGHT POLES, ETC. ROOT BARRIERS MAY BE REQUIRED WHERE
 PLANTINGS ARE NEAR UNDERGROUND STORMWATER, SANITARY, OR
