 THIS IS AN INITIAL LEVEL REVIEW. CONSTRUCTION LEVEL
 DETAILS, DESIGN CALCULATIONS, AND THE POLLUTION
 PREVENTION PLAN WILL BE REVIEWED AS PART OF THE
 ENGINEERING DEVELOPMENT APPROVAL AND BUILDING PERMIT
 APPLICATION PROCESSES.
  
 THE FOLLOWING GENERAL COMMENTS MUST BE ADDRESSED PRIOR
 TO ISSUANCE OF CONSTRUCTION AUTHORIZATION INCLUDING
 BUILDING PERMITS AND ENGINEERING DEVELOPMENT APPROVAL:
  
 1. PLAN REVIEW FOR ADDITIONAL IMPROVEMENT WILL REQUIRE
 A SEPARATE ENGINEERING DEVELOPMENT APPROVAL APPLICATION
 FROM THE ENGINEERING DIVISION OF THE PUBLIC WORKS
 DEPARTMENT. THE ENGINEERING DEVELOPMENT APPROVAL
 APPLICATION FORMS CAN BE DOWNLOADED AT:

 2. PLEASE SUBMIT THE LATEST VERSION OF THE 'AUTO-CALC

 WEST PALM BEACH PUBLIC WORKS DEPARTMENT FOR ADDITIONAL
 WORK PROPOSED. THIS FORM CAN BE DOWNLOADED FROM THE
 FOLLOWING LINK:

 THE FOLLOWING WATER AND SEWER COMMENTS:
  
 3. SUBMIT WITH THE ENGINEERING DEVELOPMENT APPROVAL
 APPLICATION SIGNED & SEALED ENGINEER/MEP WATER AND
 SANITARY SEWER CALCULATION (AVERAGE AND PEAK WITH
 PEAKING FACTOR) FOR THE PROPOSED BUILDING/GYMNASIUM.
 SUBMIT CALCULATIONS TO VALIDATE METER SIZE SELECTION.
 SHOW THESE CALCULATIONS ON THE UTILITY PLAN SHEET AS
 WELL;
  
 4. PLAN DOES NOT CLEARLY SHOW WHAT ARE THE UTILITY
 SERVICES TO THE PROPOSED GYMNASIUM BUILDING;
  
 5. HOW DO YOU PROPOSE TO ISOLATE THE EXISTING WATER
 MAIN IN THE ALLEY WAY BETWEEN BISCAYNE DRIVE AND
 FLAMINGO DRIVE? REVIEW WATERLINE CONFIGURATION ON
 BISCAYNE DRIVE;
  
 6. A FIRE HYDRANT FLOW TEST DATA FROM EXISTING FIRE
 HYDRANT WILL BE REQUIRED PART OF THE PLAN SUBMITTAL;
  
 7. PLEASE INCLUDE THE LATEST VERSION OF THE APPLICABLE
 CITY OF WEST PALM BEACH STANDARD UTILITY DETAILS TO THE
 DETAILS SHEET. CONTACT THE ENGINEERING SERVICES
 DEPARTMENT FOR THE LATEST REVISED DETAILS IN CAD
 FORMAT. CITY'S STANDARD DETAILS (IN PDF FILE FORMAT)
 CAN BE DOWNLOADED FROM THE FOLLOWING SITE:

 8. PLACEMENT OF THE PROPOSED METER, BACKFLOW PREVENTER,
 DCDA ETC. SHALL BE INSTALLED PER THE CITY'S STANDARD
 DETAILS. WATER METER SHOULD BE INSIDE AN 'UTILITY
 EASEMENT' JUST INSIDE THE PROPERTY NEAR THE
 RIGHT-OF-WAY AND ACCESSIBLE;
  
 9. PROVIDE ALL UTILITY CROSSING INFORMATION. SHOW
 CONFLICT INFORMATION (BOTTOM AND TOP OF CROSSING PIPES)
 AT THE CONFLICT POINTS OF ALL WATER, STORM AND SANITARY
 SEWER PIPES AS WELL AS OTHER FRANCHISE UTILITIES;
  
 10. PAVEMENT RESTORATION WILL BE REQUIRED FOR THE
 PROPOSED SANITARY SEWER CONNECTION TO THE EXISTING
 MANHOLE WITHIN THE RIGHT-OF-WAY. REFER TO THE CITY
 STANDARD DETAILS FOR RESTORATION GUIDELINES AND SHOW
 THE EXTENT OF PAVEMENT RESTORATION ON THE PLANS;
  
 11. EXISTING WATER SERVICE CONNECTIONS/WATER METERS
 SERVING THE PROPERTY, IF NOT USED, ARE TO BE CUT AND
 CAPPED AT THE MAIN. DE-ACTIVATED/INACTIVE PIPELINES
 LARGER THAN 2" DIAMETER WILL BE REMOVED. PLEASE LABEL
 PIPE SIZE AND MATERIAL AND ADD NOTES TO THE UTILITY
 PLANS ACCORDINGLY;
  
 12. EXISTING SEWER SERVICE CONNECTIONS SERVING THE
 PROPERTY, IF NOT USED, ARE TO BE CUT AND CAPPED AT THE
 MAIN;
  
 13. PLEASE NOTE THAT: AS OF DECEMBER 20, 2015 THE CITY
 OF WEST PALM BEACH CROSS-CONNECTION CONTROL MANUAL HAS
 BEEN REVISED, MAKING KNOWN THAT "ALL BACKFLOW DEVICES
 MUST BE LEAD FREE." FOR MORE INFORMATION REFER TO THE

 14. THE PROPOSED FIRE DEPARTMENT CONNECTION SHALL BE
 WITHIN 150 FEET OF A FIRE HYDRANT AND SHALL BE WITHIN
 AN EASEMENT WITHIN THE PROPERTY. LANDSCAPE PLAN SHALL
 SHOW LOCATIONS OF ALL WATER METERS, VALVES, CLEAN-OUTS,
 LIGHT POLES, ETC. ROOT BARRIERS MAY BE REQUIRED WHERE
 PLANTINGS ARE NEAR UNDERGROUND STORMWATER, SANITARY, OR
 POTABLE WATER LINES.
  
 15. SUBMITTAL MAINTENANCE OF TRAFFIC (MOT) WILL BE
 REQUIRED AT THE PRE-CONSTRUCTION MEETING.
