 AS TO ANDREW ROTNERMEL'S (OR BRYN WESCH'S) RELATIONSHIP
 TO THE PROPERTY OWNER - ORIGINS BEHAVIORAL HEALTHCARE
 OF FLORIDA, LLC, AND AUTHORIZATION TO SIGN THE
 APPLICATION.
  
 2. [PREVIOUS COMMENT #3] TRAFFIC CONCURRENCY LETTER WAS
 NOT PROVIDED. PLEASE PROVIDE. THE PREVIOUSLY ISSUED
 TRAFFIC CONCURRENCY LETTER FOR THIS SITE HAS EXPIRED.
  
 3. [PREVIOUS COMMENT #4] GRAPHIC DEPICTING CHANGE IN
 BUILDING FOOTPRINT NEEDS TO INCLUDE A CALCULATION
 DEMONSTRATING THAT THERE IS NOT GREATER THAN A 5%
 CHANGE IN THE BUILDING FOOTPRINT TO STAY WITHIN THE
 MINOR PD AMENDMENT CATEGORY.
  
 4. [PREVIOUS COMMENT #5] REVISE THE DETAIL ON THE
 BICYCLE PAD SHOWING DIMENSIONS. THE WIDTH IS BASED ON
 THE MINIMUM REQUIRED SPACING SHOWN BETWEEN THE RACK AND
 DEPTH IS A MINIMUM OF 6 FEET. IF TWO ROWS OF PARKING
 ARE PROVIDED, THEN 4 FEET OF ADDITIONAL SPACE IS
 REQUIRED BETWEEN THE ROWS OF BICYCLE PARKING. IT IS
 RECOMMENDED THE DETAIL IS REVISED TO PROVIDE 4 BICYCLE
 PARKING SPACES (TWO 'U'-SHAPED RACKS) REQUIRED FOR THIS
 PHASE OF DEVELOPMENT AND ADDITIONAL BICYCLE PARKING
 PROVIDED PROPORTIONAL TO PHASE II THAT WILL BE ADDED TO
 THE SITE WHEN PHASE II IS CONSTRUCTED. ADJUST BICYCLE
 PARKING CALCULATION TO SHOW HOW THE MINIMUM BICYCLE
 PARKING WAS CALCULATED AND WHAT IS REQUIRED AND BEING
 INSTALLED FOR PHASE I AND PHASE II - FINAL BUILD-OUT.
  
 5. STALL DEPTH FOR 60 DEGREE ANGLE PARKING NEEDS TO BE
 A MINIMUM OF 19'-10". PLEASE DIMENSION ON PLAN. GRAPHIC
 IN ZLDR SEC. 94-485(J) SHOWS HOW THE STALL DEPTH
 DIMENSION IS CALCULATED.
  
 6. [PREVIOUS COMMENT #14] INFORMATION ON HEIGHT OF
 PROPOSED FENCE WAS REQUESTED. RESPONSE LETTER DID NOT
 INDICATE A HEIGHT OF THE FENCE. PLEASE PROVIDE HEIGHT
 OF FENCE TO DEMONSTRATE COMPLIANCE WITH ZLDR SEC.
 94-302.
  
 7. PARKING WAS BROKEN DOWN INTO PHASES, BUT ADA SPACES
 ALSO NEED TO BE BROKEN DOWN INTO PHASES TO DEMONSTRATE
 THAT ADA PARKING SPACES MEETS MINIMUM REQUIRED FOR
 PHASE I AND PHASE II - FINAL BUILD-OUT.
  
 8. IT IS UNCLEAR IF THE PROPOSED SITE DATA IS
 REPRESENTATIVE OF THE PHASE I CONSTRUCTION OR FINAL
 BUILD-OUT. PLEASE CLARIFY.
  
 9. [PREVIOUS COMMENT #17(F)] MAXIMUM LOT COVERAGE BY
 IMPERVIOUS AREA IS 75 PERCENT [ZLDR SEC.
 94-203(B)(2(B)], PLEASE INCLUDE THE CALCULATED
 PERCENTAGE ON THE PLAN INDICATING COMPLIANCE WITH THIS
 REQUIREMENT.
  
 10. SHEET SP-1 SITE DATA SHOWS THE LANDSCAPE AREA
 PERCENTAGE AS 43%, HOWEVER, BASED ON THE SQUARE FOOTAGE
 OF LANDSCAPE AREA PROVIDED, THE PERCENTAGE WOULD BE
 36%. PLEASE REVIEW AND REVISE ACCORDINGLY.
  
 11. INDICATE THE HEIGHT OF THE PROPOSED LIGHT POLES.
 LIGHT POLES SHALL NOT EXCEED THE MAXIMUM BUILDING
 HEIGHT FOR THE DISTRICT - 80 FEET [ZLDR SEC.
 94-203(B)(6)].

 A. A PLAT SHALL BE RECORDED FOR THE SUBJECT PROPERTY
 PRIOR TO C.O., AS LISTED IN THE CONDITIONS OF APPROVAL
