 OF ALL PLANS. PLEASE NOTE THAT CHANGES ON THE
 RESUBMITTED PLANS OR THE SUBMITTAL OF INFORMATION/PLANS
 TO ADDRESS THE COMMENTS MAY RESULT IN ADDITIONAL
 COMMENTS.
  
 - THE PPRC COMMENTS SHALL BE SUFFICIENTLY ADDRESSED AND
 SUBMITTED TO THE PLANNING DIVISION BY 5 PM ON FRIDAY,
 AUGUST 21, 2015, TO BE ELIGIBLE FOR SCHEDULING FOR THE
 SEPTEMBER 15, 2015 PLANNING BOARD MEETING.
  
 - PLEASE NOTE THAT IF THE PLANNING DIVISION HAS NOT
 RECEIVED A RESPONSE TO THE PPRC COMMENTS WITHIN 60 DAYS
 FROM THE DATE OF THE PPRC MEETING (BY OCTOBER 12,
 2015), THE CASE SHALL BE CONSIDERED WITHDRAWN. ANY
 FURTHER ACTION SHALL REQUIRE A NEW DEVELOPMENT
 APPLICATION.
  
 - PLEASE NOTE THAT YOU WILL BE PERMITTED ONE (1)
 RESUBMITTAL AT NO ADDITIONAL COST. IF PREVIOUSLY-ISSUED
 COMMENTS CONTINUE TO NOT BE SUFFICIENTLY ADDRESSED, THE
 APPLICANT WILL BE ACCESSED A RESUBMITTAL FEE. SUCH FEE
 WILL BE 20% OF THE ORIGINAL APPLICATION FEE ($600).
  
 - PUBLIC HEARING NOTIFICATION SIGNAGE SHALL BE POSTED
 IN ACCORDANCE WITH THE REQUIREMENTS OF SECTION
 94-39(I)(5) OF THE ZONING AND LAND DEVELOPMENT
 REGULATIONS.
  
 - IF THE VALUE OF THE BUILDING IMPROVEMENTS EXCEED
 $500,000, THE PROJECT WILL BE SUBJECT TO THE CITY'S
 ARTS IN PUBLIC PLACES REQUIREMENTS AT THE TIME OF
 BUILDING PERMIT SUBMITTAL.
  
 1) PLEASE PROVIDE THE LEGAL DESCRIPTION FOR THE ENTIRE
 GOOD SAMARITAN MEDICAL CENTER, INCLUDING THE NORTH
 PARKING LOT THAT IS THE SUBJECT OF THE FUTURE LAND USE
 MAP AMENDMENT AND THE REZONING AND THE PROPERTIES WEST
 OF DIXIE HIGHWAY. THE ENTIRE SITE SHALL BE INCLUDED ON
 A SURVEY.
  
 2) THE SIGN PROGRAM CANNOT MOVE FORWARD WITH THE SIGN
 SHOWN ON THE CORNER OF FLAGLER DRIVE AND PALM BEACH
 LAKES BOULEVARD UNLESS THE CORNER PARCEL REVERTS BACK
 TO THE GOOD SAMARITAN MEDICAL CENTER.
  
 3) GOOD SAMARITAN MEDICAL CENTER SHALL APPLY TO ABANDON
 THE 75 FOOT BY 13.5 FOOT ALLEY THAT EXTENDS FROM SOUTH
 DIXIE HIGHWAY. IF THE CITY COMMISSION DOES NOT ABANDON
 THE ALLEY, THEN THE GOOD SAMARITAN MEDICAL CENTER SHALL
 SECURE A REVOCABLE PERMIT FOR THE USE OF THE LAND OR
 DESIGN THE SITE TO EXCLUDE THE ALLEY. THIS REQUIRES THE
 RECONFIGURATION OF PARKING AND DRIVE AISLES AS WELL AS
 THE ADDITION OF BUFFER LANDSCAPE.
  
 4) PLEASE PROVIDE THE 12/15/14 PARKING REPORT CITED FOR
 THE PARKING DATA TABLE.
  
 5) PLANNING STAFF WAS UNABLE TO LOCATE THE BUILDING
 PERMIT FOR THE NORTHERN PARKING LOT. NOT ONLY DOES IT
 NOT MATCH THE APPROVED ZBA PLANS, IT HAS SUBSTANDARD
 SPACES AND DOES MEET THE REQUIREMENTS OF THE PARKING
 CODE ? EX. THE 11 PARKING SPACES ALONG THE NORTH
 BOUNDARY SHALL BE STRIPED AS 10 SPACES (WHICH IS ALSO A
 CODE MAXIMUM IN A ROW). DEFICIENCIES SHALL BE CORRECTED
 BOTH ON THE PLANS AS PART OF THIS AMENDMENT AND IN THE
 FIELD, AFTER SECURING A BUILDING PERMIT.
  
 6) THE "AT-GRADE PARKING (NORTH SIDE OF MAIN BUILDING)"
 INCLUDES AN ADA PARKING SPACE THAT IS NOT WIDE ENOUGH
 NOR DOES IT CONTAIN THE REQUIRED ACCESS AISLE. THIS
 SHALL BE CORRECTED AS PART OF THIS AMENDMENT. THE
 AERIALS SHOW A COVERED CANOPY IN THIS LOCATION THAT IS
 NOT ON THE SITE PLAN.
  
 7) THE SOUTH SIDE OF THE FERRIS GARAGE IS NOT SHOWN
 ACCURATELY ON THE SITE AND LANDSCAPE PLANS. THERE IS A
 DRIVEWAY ENTRANCE MISSING AS WELL AS WHAT APPEARS TO BE
 A COVERED WALKWAY ON THE SOUTH SIDE OF THE DRIVE AISLE.
  
 8) ALL PARKING SPACES SHALL BE CONFIRMED TO MEET
 MINIMUM CODE SIZE REQUIREMENTS OR THEY SHALL BE
 DESIGNATED AS COMPACT SPACES (IF THEY MEET THE COMPACT
 SPACE MINIMUMS). IF A ROW CAN BE RESTRIPED TO HAVE ALL
 SPACES MEET CODE, THAT IS THE OPTION THAT SHALL BE
 USED.
  
 9) PLEASE CLARIFY WHERE THE 25 ADA SPACES ARE. ONLY 23
 ARE SHOWN ON THE SITE PLAN.
  
 10) ANY CHANGES TO THE PARKING COUNT BASED ON THE
 PARKING COMMENTS SHALL BE REFLECTED ON THE PARKING DATA
 TABLE.
  
 11) PLEASE PROVIDE FLOOR PLANS TO VERIFY THAT THERE ARE
 NO NEW BEDS BEING ADDED.
  
 12) ALL NEW MECHANICAL EQUIPMENT (GROUND OR ROOFTOP)
 ADDED TO THE SITE SHALL BE SCREENED FROM VIEW OF THE
 PUBLIC.
  
 13) PROVIDE SIGHT LINES TO VERIFY THAT THE PUBLIC WILL
 NOT SEE THE NEW ROOFTOP MECHANICAL EQUIPMENT. PLEASE
 NOTE THAT THIS INCLUDES FROM THE PALM BEACH LAKES
 BOULEVARD BRIDGE OVER THE RAILROAD TRACKS.
  
 14) PROVIDE VERIFICATION THAT THE GARAGE PLANTERS ARE
 STILL PLANTED IN ACCORDANCE WITH THE ADOPTING ORDINANCE
 REQUIREMENTS.
  
 15) PROVIDE THE SIGN SETBACK DIMENSIONS TO THE
 RIGHT-OF-WAYS AND THE PROPERTY LINES.
  
 16) SIGNS 4 AND 27C IS SHOWN IN THE INCORRECT LOCATION.
  
 17) EXISTING GOOD SAMARITAN SIGN ABOVE THE O'KEEFE
 EXPANSION AREA (FACING WEST) IS NOT SHOWN ? WILL IT BE
 REMOVED OR RELOCATED?
  
 18) FUTURE SIGN 32 IS NOT SHOWN ON THE SIGN PLAN.
  
 19) ARE THERE ANY SIGNS DESIRED FOR THE WESTERN PARKING
 GARAGE?
  
 20) THE DISCUSSION OF THE SPECIFICS OF THE MASTER
 SIGNAGE PLAN REQUIRES A SEPARATE MEETING TO TRY TO
 ARRIVE AT CONSENSUS REGARDING THE REQUESTS AND WHAT
