 TO ADDRESS THE COMMENTS MAY RESULT IN ADDITIONAL
 COMMENTS.
  
 - PLEASE NOTE THAT IF THE PLANNING DIVISION HAS NOT
 RECEIVED A RESPONSE TO THESE COMMENTS WITHIN 60 DAYS
 FROM THE DATE ISSUANCE (BY JULY 7, 2015), THE CASE
 SHALL BE CONSIDERED WITHDRAWN. ANY FURTHER ACTION SHALL
 REQUIRE A NEW DEVELOPMENT APPLICATION.
  
 - PLEASE NOTE THAT YOU WILL BE PERMITTED ONE ADDITIONAL
 RESUBMITTAL AT NO ADDITIONAL COST. IF PREVIOUSLY-ISSUED
 COMMENTS CONTINUE TO NOT BE SUFFICIENTLY ADDRESSED, THE
 APPLICANT WILL BE ACCESSED A RESUBMITTAL FEE. SUCH FEE
 WILL BE 20% OF THE ORIGINAL APPLICATION FEE ($60).
  
 SITE PLAN
  
 1) REPEAT COMMENT: THE PARKING AREA TO THE WEST OF THE
 ER BUILDING SHALL EITHER INCLUDE WHEEL STOPS TWO FEET
 FROM THE END OF THE SPACE (REGARDLESS IF CURBING IS
 USED AT THE END OF THE SPACE) OR BE 16 FEET DEEP OF
 PAVEMENT WITH A CURB WHICH ALLOWS A TWO (2) FOOT
 OVERHANG OF THE ADJACENT SIDEWALK OR LANDSCAPE AREA.
 PLEASE NOTE THAT THE OVERHANG MAY NOT REDUCE THE WIDTH
 OF THE ADJACENT SIDEWALK, WHICH SHALL BE FIVE (5) FEET
 FOR THE ADA ROUTE. THE SURVEY INDICATES THAT THERE ARE
 EXISTING CURBS AT THE HEAD OF THE SPACES WITH 18 FEET
 OF PAVEMENT. WHEEL STOPS WILL BE REQUIRED ON THE
 EASTERN ROW OF SPACES TO PREVENT THE OVERHANG OF THE
 EXISTING SIDEWALK. THE LANDSCAPE PLAN SHOWS A HEDGE AT
 THE HEAD OF SOME OF THE PARKING SPACES ON THE WEST SIDE
 OF THE PARKING AREA (IN FRONT OF THE LIGHT POLE), WHICH
 PREVENT VEHICLE OVERHANG. WHEEL STOPS SHALL BE USED IN
 THESE TWO PARKING SPACES.
  
 2) REPEAT COMMENT: THE CURBLINE ON PALM BEACH LAKES
 BOULEVARD AT DIXIE HIGHWAY IS NOT SHOWN CORRECTLY ON
 THE SITE PLAN. THE LANDSCAPE PLAN HAS BEEN UPDATED.
  
 3) REPEAT COMMENT: THE NEW SIDEWALK LEADING TO DIXIE
 HIGHWAY IS SHOWN CORRECTLY ON THE LANDSCAPE PLAN, BUT
 NOT SHOWN ON THE SITE PLAN. ALL PLANS SHALL SHOW THE
 FINAL CONDITION.
  
 4) REVISED COMMENT: THERE ARE THREE PARKING AREAS BEING
 MODIFIED AS PART OF THE ADA UPGRADES ASSOCIATED WITH
 PERMIT NO. 15020678. SHOW CONSISTENCY BETWEEN THE
 AMENDMENT AND THE PERMIT.
  
 A. NORTH OF VICTOR FERRIS - THE CITY ONLY PERMITS TEN
 PARKING SPACES IN A ROW WITHOUT AN INTERIOR LANDSCAPE
 ISLAND. THE AREA NORTH OF THE VICTOR FERRIS BUILDING
 MAY NOT BE RESTRIPED TO INCLUDE 11 PARKING SPACES. A
 PERMIT FOR THE PARKING SPACE STRIPING ASSOCIATED WITH
 PERMIT NO. 15020678 HAS NOT YET BEEN SUBMITTED, SO
 STAFF HAD NOT PREVIOUSLY REVIEWED THE PARKING LAYOUT
 FOR CODE COMPLIANCE TO NOTICE THAT THERE ARE 11 SPACES
 SHOWN IN A ROW.
  
 B. NORTHEAST OF MAIN HOSPITAL ENTRANCE ? PARKING
 STRIPING, SIDEWALKS, WHEEL STOPS AND TERMINAL ISLANDS
 ON THE MINOR AMENDMENT PLANS DO NOT MATCH THE PROPOSED
 PARKING LOT WORK. STAFF IS HAVING A HARD TIME SEEING IF
 THE PERMIT GEOMETRY IS EVEN POSSIBLE WHEN COMPARED TO
 THE SURVEY CONDITIONS. PLEASE REVIEW AND CLARIFY.
  
 C. WEST OF ER - PARKING STRIPING, SIDEWALKS, WHEEL
 STOPS AND TERMINAL ISLANDS ON THE MINOR AMENDMENT PLANS
 DO NOT MATCH THE PROPOSED PARKING LOT WORK. PERMIT SET
 REQUIRES MAJOR CHANGES FROM WHAT IS SHOWN ON THE
 SURVEY. PLEASE REVIEW AND CLARIFY.
  
 5) REPEAT COMMENT: THE SUBMITTED PLANS INCORRECTLY SHOW
 THE PRESENCE OF OVERHEAD ELECTRIC LINES WEST OF THE
 LIGHT POLE WESTERNMOST LIGHT POLE ON THE NORTH SIDE OF
 PALM BEACH LAKES BOULEVARD. PLANS SHALL SHOW ACCURATE
 CONDITIONS.
  
 CONDITION 6) PRIOR TO THE SUBMITTAL OF BUILDING PERMITS
 FOR ANY WORK ASSOCIATED WITH THIS MINOR AMENDMENT, A
 MAJOR PD AMENDMENT SHALL BE SUBMITTED WHICH SHALL
 INCLUDE THE FOLLOWING:
  
 A. PLANS SHOWING COMPLIANCE WITH LANDSCAPE,
 PARKING/VEHICULAR CIRCULATION FOR THE SITE, AS WELL AS
 ALL SITE FEATURES, ESPECIALLY ANY WHICH REQUIRE WAIVERS
 FROM THE CURRENT ZONING AND LAND DEVELOPMENT
 REGULATIONS (PROVIDE LIST OF REQUESTED WAIVERS).
  
 B. A MASTER SIGN PLAN.
  
 C. CORRECTIONS TO ANY FEATURES CONSTRUCTED WITHIN CITY
 RIGHT-OF-WAY OR PROPERTY. EX. THE PARTIAL ALLEY ON WEST
 SIDE OF SITE AND THE SIGN IN THE PALM BEACH LAKES
 RIGHT-OF-WAY.
  
 D. EASEMENTS FOR ANY PUBLIC INFRASTRUCTURE WITHIN THE
 BOUNDARIES OF THE GOOD SAMARITAN CSPD.
  
 E. INCLUSION OF THE PARKING LOT ON THE NORTH END OF THE
 PROPERTY (15TH STREET PARKING LOT) WITHIN THE GOOD
 SAMARITAN CSPD BOUNDARY. THIS REQUIRES A FUTURE LAND
 USE AMENDMENT.
  
 CONDITION 7) PLEASE NOTE THAT STAFF WAS UNABLE TO
 LOCATE THE BUILDING PERMIT THAT APPROVED THE 15TH
 STREET PARKING LOT; HOWEVER, IT IS NOT STRIPED IN
 ACCORDANCE WITH THE PLAN APPROVED WITH THE CLASS B
 SPECIAL USE PERMIT FOR THE OFF-SITE PARKING. THIS WILL
 NEED TO BE RECTIFIED.
  
 CONDITION 8) THE LANDSCAPE FROM THE ZBA APPROVED PLAN
 (OR EQUIVALENT) FOR THE 15TH STREET PARKING LOT SHALL
 BE REPLACED. THE SUBMITTED LANDSCAPE PLAN DOES NOT SHOW
 COMPLIANCE WITH THE APPROVED PLAN.
