 OF ALL PLANS. PLEASE NOTE THAT CHANGES ON THE
 RESUBMITTED PLANS OR THE SUBMITTAL OF INFORMATION/PLANS
 TO ADDRESS THE COMMENTS MAY RESULT IN ADDITIONAL
 COMMENTS.
  
 - PLEASE NOTE THAT IF THE PLANNING DIVISION HAS NOT
 RECEIVED A RESPONSE TO THE PPRC COMMENTS WITHIN 60 DAYS
 FROM THE DATE OF THE PPRC MEETING (BY APRIL 11, 2015),
 THE CASE SHALL BE CONSIDERED WITHDRAWN. ANY FURTHER
 ACTION SHALL REQUIRE A NEW DEVELOPMENT APPLICATION.
  
 - PLEASE NOTE THAT YOU WILL BE PERMITTED ONE ADDITIONAL
 RESUBMITTAL AT NO ADDITIONAL COST. IF PREVIOUSLY-ISSUED
 COMMENTS CONTINUE TO NOT BE SUFFICIENTLY ADDRESSED, THE
 APPLICANT WILL BE ACCESSED A RESUBMITTAL FEE. SUCH FEE
 WILL BE 20% OF THE ORIGINAL APPLICATION FEE ($60).
  
 SITE PLAN
  
 1) REVISED COMMENT: THE PARKING AREA TO THE WEST OF THE
 ER BUILDING IS SHOWN AS BEING MODIFIED AND COMPLIES
 WITH THE PARKING AISLE AND PARKING SPACE DEPTH AS
 REQUIRED BY THE ZONING REGULATIONS. PLEASE NOTE THAT
 THE PREVIOUS COMMENT ABOUT REMOVING THE WHEEL STOPS
 ONLY APPLIED IF THE FRONT OF THE CARS WERE TO OVERHANG
 THE CURB (16 FOOT DEEP PAVEMENT), WHICH REQUIRES AN
 ADDITIONAL TWO (2) FEET OF SIDEWALK OR LANDSCAPE AREA
 FOR THE OVERHANG. SINCE NO OVERHANG AREA IS BEING
 PROVIDED, THE WHEEL STOPS SHALL BE USED AND SHOWN ON
 THE PLANS.
  
 2) REPEAT COMMENT: THE CURBLINE ON PALM BEACH LAKES
 BOULEVARD AT DIXIE HIGHWAY IS NOT SHOWN CORRECTLY ON
 THE SITE OR LANDSCAPE PLANS.
  
 3) REVISED COMMENT: THE NEW SIDEWALK TO DIXIE HIGHWAY
 SHALL CROSS THE DRIVEWAY ENTRANCE TO THE NEW PARKING
 LOT, TRAVERSE THE TERMINAL ISLAND, CROSS THE DRIVEWAY
 TO THE SOUTH AND LEAD TO THE EXISTING SIDEWALK WEST OF
 THE EMERGENCY ROOM BUILDING.
  
 4) NEW COMMENT: PLEASE VERIFY THAT YOUR PLANS MATCH THE
 ADA PARKING CHANGES SHOWN ON SUBMITTED PERMIT 15020678.
  
 LANDSCAPE
  
 5) REPEAT COMMENT: TERMINAL ISLAND TREES ARE REQUIRED
 ALONG THE PARKING SPACES WEST OF THE ER. THERE IS NO
 TERMINAL ISLAND TREE SHOWN SOUTH OF THE THREE SPACE
 PARKING BAY.
  
 6) REPEAT COMMENT: THE PERIMETER BUFFER TREES, ONE
 EVERY 30 FEET, ARE REQUIRED ALONG PALM BEACH LAKES
 BOULEVARD SOUTH OF THE ER. STAFF DOES NOT ACCEPT THE
 APPLICANT'S JUSTIFICATION FOR NOT HAVING THE REQUIRED
 TREES. THE TWO OTHER HOSPITALS WITHIN WEST PALM BEACH,
 ST. MARY'S AND WEST PALM HOSPITAL, DO NOT HAVE THEIR
 EMERGENCY ROOMS IMMEDIATELY VISIBLE FROM THE PERIMETER
 ROAD NETWORK. ST. MARY'S IS BLOCKED BY TREES AND WEST
 PALM HOSPITAL IS BEHIND A BUILDING. GOOD SAMARITAN HAS
 A DIRECTIONAL SIGN WITH THE EMERGENCY ROOM INFORMATION
 AT THE CORNER OF PALM BEACH LAKES AND DIXIE HIGHWAY AND
 ONE AT THE ENTRANCE TO THE EMERGENCY ROOM DRIVEWAY
 ALONG PALM BEACH LAKES TO DIRECT MOTORISTS.
 ADDITIONALLY, THE SUBMITTED PLANS INCORRECTLY SHOW THE
 PRESENCE OF OVERHEAD ELECTRIC LINES WEST OF THE LIGHT
 POLE. THERE IS NO OVERHEAD INTERFERENCE FOR THIS
 INSTALLATION OF TREES OR PALMS WEST OF THIS LOCATION.
  
 ELEVATION
  
 7) REVISED COMMENT: PROVIDE DRAWINGS TO SHOW THE
 SCREENING OF THE ROOFTOP MECHANICAL EQUIPMENT.
  
 CONDITION 8) PRIOR TO THE SUBMITTAL OF BUILDING PERMITS
 FOR ANY WORK ASSOCIATED WITH THIS MINOR AMENDMENT, A
 MAJOR PD AMENDMENT SHALL BE SUBMITTED WHICH SHALL
 INCLUDE THE FOLLOWING:
  
 A. A RECENT SURVEY, LESS THAN ONE YEAR OLD. THIS MAY BE
 AN UPDATE OF THE SUBMITTED SURVEY FROM 2001.
  
 B. PLANS SHOWING COMPLIANCE WITH LANDSCAPE,
 PARKING/VEHICULAR CIRCULATION FOR THE SITE, AS WELL AS
 ALL SITE FEATURES, ESPECIALLY ANY WHICH REQUIRE WAIVERS
 FROM THE CURRENT ZONING AND LAND DEVELOPMENT
 REGULATIONS (PROVIDE LIST OF REQUESTED WAIVERS).
  
 C. A MASTER SIGN PLAN.
  
 D. CORRECTIONS TO ANY FEATURES CONSTRUCTED WITHIN CITY
 RIGHT-OF-WAY OR PROPERTY. EX. THE PARTIAL ALLEY ON WEST
 SIDE OF SITE AND THE SIGN IN THE PALM BEACH LAKES
 RIGHT-OF-WAY.
  
 E. EASEMENTS FOR ANY PUBLIC INFRASTRUCTURE WITHIN THE
 BOUNDARIES OF THE GOOD SAMARITAN CSPD.
  
 F. INCLUSION OF THE PARKING LOT ON THE NORTH END OF THE
 PROPERTY (15TH STREET PARKING LOT) WITHIN THE GOOD
 SAMARITAN CSPD BOUNDARY. THIS REQUIRES A FUTURE LAND
 USE AMENDMENT.
  
 CONDITION 9) PLEASE NOTE THAT STAFF WAS UNABLE TO
 LOCATE THE BUILDING PERMIT THAT APPROVED THE 15TH
 STREET PARKING LOT; HOWEVER, IT IS NOT STRIPED IN
 ACCORDANCE WITH THE PLAN APPROVED WITH THE CLASS B
 SPECIAL USE PERMIT FOR THE OFF SITE PARKING. THIS WILL
 NEED TO BE RECTIFIED.
  
 CONDITION 10) THE LANDSCAPE FROM THE ZBA APPROVED PLAN
 (OR EQUIVALENT) FOR THE 15TH STREET PARKING LOT SHALL
 BE REPLACED. THE SUBMITTED LANDSCAPE PLAN DOES NOT SHOW
 COMPLIANCE WITH THE APPROVED PLAN.
