 (5) PAPER COPIES AND AN ELECTRONIC COPY IN .PDF FORMAT
 OF ALL PLANS. PLEASE NOTE THAT CHANGES ON THE
 RESUBMITTED PLANS OR THE SUBMITTAL OF INFORMATION/PLANS
 TO ADDRESS THE COMMENTS MAY RESULT IN ADDITIONAL
 COMMENTS.
  
 - PLEASE NOTE THAT IF THE PLANNING DIVISION HAS NOT
 RECEIVED A RESPONSE TO THE PPRC COMMENTS WITHIN 60 DAYS
 FROM THE DATE OF THE PPRC MEETING (BY APRIL 11, 2015),
 THE CASE SHALL BE CONSIDERED WITHDRAWN. ANY FURTHER
 ACTION SHALL REQUIRE A NEW DEVELOPMENT APPLICATION.
  
 - PLEASE NOTE THAT YOU WILL BE PERMITTED ONE (1)
 RESUBMITTAL AT NO ADDITIONAL COST. IF PREVIOUSLY-ISSUED
 COMMENTS CONTINUE TO NOT BE SUFFICIENTLY ADDRESSED, THE
 APPLICANT WILL BE ACCESSED A RESUBMITTAL FEE. SUCH FEE
 WILL BE 20% OF THE ORIGINAL APPLICATION FEE ($60).
  
 1) PRIOR TO THE SUBMITTAL OF BUILDING PERMITS FOR ANY
 WORK ASSOCIATED WITH THIS MINOR AMENDMENT, A MAJOR PD
 AMENDMENT SHALL BE SUBMITTED WHICH SHALL INCLUDE THE
 FOLLOWING:
  
 A. A RECENT SURVEY, LESS THAN ONE YEAR OLD. THIS MAY BE
 AN UPDATE OF THE SUBMITTED SURVEY FROM 2001.
  
 B. PLANS SHOWING COMPLIANCE WITH LANDSCAPE,
 PARKING/VEHICULAR CIRCULATION FOR THE SITE, AS WELL AS
 ALL SITE FEATURES, ESPECIALLY ANY WHICH REQUIRE WAIVERS
 FROM THE CURRENT ZONING AND LAND DEVELOPMENT
 REGULATIONS (PROVIDE LIST OF REQUESTED WAIVERS).
  
 C. A MASTER SIGN PLAN.
  
 D. CORRECTIONS TO ANY FEATURES CONSTRUCTED WITHIN CITY
 RIGHT-OF-WAY OR PROPERTY. EX. THE PARTIAL ALLEY ON WEST
 SIDE OF SITE AND THE SIGN IN THE PALM BEACH LAKES
 RIGHT-OF-WAY.
  
 E. EASEMENTS FOR ANY PUBLIC INFRASTRUCTURE WITHIN THE
 BOUNDARIES OF THE GOOD SAMARITAN CSPD.
  
 F. INCLUSION OF THE PARKING LOT ON THE NORTH END OF THE
 PROPERTY (15TH STREET PARKING LOT) WITHIN THE GOOD
 SAMARITAN CSPD BOUNDARY. THIS REQUIRES A FUTURE LAND
 USE AMENDMENT.
  
 2) PLEASE NOTE THAT STAFF WAS UNABLE TO LOCATE THE
 BUILDING PERMIT THAT APPROVED THE 15TH STREET PARKING
 LOT; HOWEVER, IT IS NOT STRIPED IN ACCORDANCE WITH THE
 PLAN APPROVED WITH THE CLASS B SPECIAL USE PERMIT FOR
 THE OFF SITE PARKING. THIS WILL NEED TO BE RECTIFIED.
  
 3) THE BUILDING ADDITION WILL REQUIRE COMPLIANCE WITH
 THE CITY?S ARTS IN PUBLIC PLACES REQUIREMENTS.
  
 SITE PLAN
  
 4) THE SITE DATA TABLE SHALL BE AMENDED TO SHOW THE
 AREA THAT IS WITHIN THE CSPD AS ONE LINE ITEM AND THE
 AREA WITHIN THE POR AS ANOTHER.
  
 5) PLEASE EXPLAIN THE "GROSS" AND "NET" ACREAGE.
  
 6) THE PARKING GARAGES DO NOT COUNT TOWARD THE FLOOR
 AREA RATIO NUMBERS; HOWEVER, THEY ARE PART OF THE LOT
 COVERAGE. PLEASE NOTE THAT THE FAR, LOT COVERAGE AND
 IMPERVIOUS NUMBERS APPLY FOR THE ENTIRE CSPD AND THE
 POR PORTIONS OF THE LOT SEPARATELY.
  
 7) PLEASE CLARIFY IF THE CANCER CENTER SQUARE FOOTAGE
 IS INCLUDED WITHIN THE OFFICE POOL FOR THE PARKING
 COUNTS. UNLESS THIS AREA INCLUDES BEDS, IT SHALL BE
 COUNTED AS OFFICE, NOT HOSPITAL.
  
 8) PLEASE VERIFY YOUR PARKING COUNT TABLE. IT DOES NOT
 SEEM TO LABEL THE PARKING AREAS CORRECTLY, NOR DOES IT
 INCLUDE THE PARKING SPACES ADJACENT TO THE VICTOR
 FARRIS BUILDING. NOTE: THESE SPACES SHALL BE SHOWN AS
 APPROVED BY PERMIT NO. 12070790, NOT THE 2001 SURVEY.
  
 9) THE NEW PARKING LOT THAT REPLACED THE AUDITORIUM
 SHALL HAVE A DESIGNATED TURN AROUND SPOT.
  
 10) THE LOADING DOCK CONTAINS A LABEL FOR 10 PARKING
 SPACES THAT DO NOT EXIST WITHIN THE LOADING DOCK.
  
 11) THE PARKING AREA TO THE WEST OF THE ER BUILDING IS
 SHOWN AS BEING MODIFIED. THIS AREA SHALL HAVE THE WHEEL
 STOPS REMOVED AND OVERHANG THE CURB SO THAT THE DRIVE
 AISLE IS WIDENED TO MEET THE 24 FOOT MINIMUM STANDARD
 (NOT 23.6 AS SHOWN). THE NEW TERMINAL ISLAND SHALL BE
 DESIGNED TO MEET THIS REQUIREMENT.
  
 12) THE CURBLINE ON PALM BEACH LAKES BOULEVARD AT DIXIE
 HIGHWAY IS NOT SHOWN CORRECTLY ON THE SITE OR LANDSCAPE
 PLANS.
  
 13) THE SITE PLAN AND THE LANDSCAPE PLAN CURBING DO NOT
 MATCH FOR THE DIVIDER MEDIAN SOUTH OF THE ENTRY DRIVE.
  
 14) PLEASE EXPLAIN THE CURVED SIDEWALK THAT GOES
 NOWHERE TO THE WEST OF THE NEW PARKING SPACES WHERE THE
 AUDITORIUM WAS REMOVED.
  
 15) PLEASE EXPLAIN THE ENORMOUS WIDTH OF THE TWO ADA
 SPACES SOUTH OF THE ER. WHY DO THESE NOT HAVE
 ACCESSIBLE AISLES?
  
 LANDSCAPE
  
 16) THE NEW PARKING LOT WHERE THE AUDITORIUM WAS
 REMOVED IS REQUIRED TO HAVE TERMINAL ISLAND TREES. NONE
 ARE SHOWN ON THE NORTH SIDE OR THE SOUTH EAST SIDE. THE
 TWO COCONUT PALMS ON IN THE SOUTH WEST ISLAND DO
 QUALIFY AS THE TERMINAL TREE AS A GROUP OF THREE IS
 REQUIRED.
  
 17) TERMINAL ISLAND TREES ARE REQUIRED ALONG THE
 PARKING SPACES WEST OF THE ER.
  
 18) THE PERIMETER BUFFER TREES, ONE EVERY 30 FEET, ARE
 REQUIRED ALONG PALM BEACH LAKES BOULEVARD SOUTH OF THE
 ER.
  
 19) THE LANDSCAPE FROM THE ZBA APPROVED PLAN (OR
 EQUIVALENT) FOR THE 15TH STREET PARKING LOT SHALL BE
 REPLACED. THE SUBMITTED LANDSCAPE PLAN DOES NOT SHOW
 COMPLIANCE WITH THE APPROVED PLAN.

 20) HOW WILL THE ROOFTOP MECHANICAL EQUIPMENT BE
 SCREENED ON THE NEW ER ADDITION?
