 PLEASE PROVIDE A WRITTEN RESPONSE TO THE FOLLOWING
 REVIEW COMMENTS:
  
 1. WHAT IS THE DEPTH FROM MEAN LOW WATERLINE AND THE
 BOTTOM? PLEASE JUSTIFY THE NECESSITY OF THE ADDITIONAL
 LENGTH BY PROVIDING MORE DETAILED INFORMATION ON THE
 DEPTH NECESSARY TO PROVIDE FOR A FUNCTIONAL DOCK.
  
 2. LABELS NEED TO BE PLACED ON STAMPED ENVELOPES. AS
 MORE THAN 50 PROPERTY OWNERS NEED TO BE NOTIFIED, THE
 CITY WILL PROVIDE THE APPLICANT WITH THE MAILER AND THE
 DATE UPON WHICH THE MAILER NEEDS TO BE SENT OUT. IT IS
 THE APPLICANT?S RESPONSIBILITY TO PLACE THE MAILERS IN
 THE ENVELOPES AND HAVE THE POSTMARKED BEFORE THE DATE
 PROVIDED. UPON COMPLETION OF THE MAILER, A
 SIGNED/NOTARIZED AFFIDAVIT (PROVIDED IN THE DEVELOPMENT
 APPLICATION), SHALL BE PROVIDED TO THE CITY.
  
 3. PLEASE PROVIDE A COPY OF THE SEAGRASS STUDY.
  
 4. PLEASE DELINEATE THE DISTANCE BETWEEN THE
 EASTERNMOST POINT OF THE PROPOSED DOCK AND THE
 NAVIGABLE CHANNEL.
  
 5. POTABLE WATER AND ELECTRIC SHALL BE PROVIDED TO THE
 DOCK. PLEASE PROVIDE A NOTE ON THE PLANS INDICATING
 SUCH.
  
 6. CITY STAFF WOULD LIKE TO WORK ON POSSIBLE
 ALTERNATIVES TO CONFORM WITH THE REQUIREMENTS OF THE
 MIDDLE 1/3, OR OTHERWISE REDUCE THE IMPACT ON ADJACENT
 PROPERTIES. THE LARGER BOAT LIFT COULD BE PLACED AT THE
 EAST END OF THE PROPOSED DOCK IN A NORTH/SOUTH
 DIRECTION.
  
 7. NO PORTION OF THE PROPOSED DOCK SHALL EXTEND ABOVE
 THE TOP SURFACE OF THE DOCK. ALL PILINGS, WITH THE
 EXCEPTION OF THOSE USED FOR MOORING PURPOSES, SHALL BE
 CUT DOWN TO THE LEVEL OF THE DECK.
  
 8. AT LEAST ONE (1) LADDER SHALL BE PROVIDED EXTENDING
 FROM THE DOCK SURFACE TO TWO (2) FEET BELOW THE MEAN
 LOW WATERLINE.
  
 9. ALL DOCKS SHALL HAVE WATER-RESISTANT REFLECTIVE
 NUMERALS CORRESPONDING TO THE STREET ADDRESS OF THE
 UPLAND PROPERTY AND SHALL BE VISIBLE FROM THE WATERWAY.
 PLEASE PROVIDE A NOTE ON THE PLANS INDICATING SUCH.
  
 10. ALL DOLPHINS OR FREE-STANDING PILINGS SHALL HAVE A
 REFLECTORIZED BAND OF AT LEAST FIVE (5) INCHES IN WIDTH
 LOCATED APPROXIMATELY 24 INCHES FROM THE TOP OF THE
 DOLPHIN. REFLECTOR TAPE SHALL BE FORMULATED FOR MARINE
 USE AND BE OF AN IRIDESCENT SILVER COLOR.

 PLEASE NOTE THE FOLLOWING WHEN RESUBMITTING:
  
 A) WRITTEN RESPONSES TO COMMENTS MADE BY ALL
 DEPARTMENTS SHALL BE PROVIDED UPON RESUBMISSION.
  
 B) 15 SETS OF REVISED PLANS AND/OR DOCUMENTS SHALL BE
 PROVIDED, ALONG WITH ONE (1) REDUCED COPY (11IN X
 17IN), AND AN ELECTRONIC COPY OF ALL PLANS.
  
 C) PLEASE NOTE THAT RE-SUBMITTALS MAY BE SUBJECT TO
 ADDITIONAL COMMENTS.
  
 D) IF NO RESPONSE IS RECEIVED WITHIN 60 DAYS OF THE
 ISSUANCE OF THESE COMMENTS, THE APPLICATION WILL BE
 CONSIDERED WITHDRAWN AND ANY FURTHER ACTION WILL
 REQUIRE SUBMISSION OF A NEW DEVELOPMENT APPLICATION.
  
 E) PLEASE NOTE THAT YOU WILL BE PERMITTED ONE (1)
 RESUBMITTAL AT NO ADDITIONAL COST. IF PREVIOUSLY-ISSUED
 COMMENTS CONTINUE TO NOT BE SUFFICIENTLY ADDRESSED, THE
 APPLICANT WILL BE ACCESSED A RESUBMITTAL FEE. SUCH FEE
 WILL BE 20% OF THE ORIGINAL APPLICATION FEE.
  
 F) IN ORDER TO BE CONSIDERED FOR THE MARCH 18, 2014
 PLANNING BOARD AGENDA, ALL COMMENTS SHALL BE
 SATISFACTORILY ADDRESSED. PLEASE SUBMIT ALL REVISED
 DOCUMENTS/PLANS NO LATER THAN MONDAY, MARCH 3, 2014.
