 B. $600 FOR ONE (1) WAIVER REQUEST
 C. $216 ADVERTISING FEE
 D. $424 ENGINEERING FEE
  
 2. PLEASE PROVIDE THE PROPERTY LIST OF EVERYONE WITHIN
 400 FEET OF THE PROPERTY (AS PROVIDED BY THE PALM BEACH
 COUNTY PROPERTY APPRAISER?S OFFICE).
  
 3. PLEASE PROVIDE THE MAP SHOWING EVERYONE WITHIN 400
 FEET OF THE PROPERTY (AS PROVIDED BY THE PALM BEACH
 COUNTY PROPERTY APPRAISER?S OFFICE).
  
 4. PLEASE PROVIDE COPIES OF ALL REQUIRED UTILITY
 LETTERS.
  
 5. DEP HAD NO OBJECTION TO CONSTRUCTING THE
 DOCK/BOATLIFT OVER EXISTING SEAGRASSES? THE PLANS
 ATTACHED TO THE FDEP APPROVAL INDICATE THAT GRATED
 DECKING IS TO BE PROVIDED WITH A MINIMUM OF 43% OPEN
 SPACE REQUIRED; THE PLANS FROM CONSTRUCTION TECHNOLOGY,
 INC. DO NOT SHOW THAT. ADDITIONALLY, THE FDEP PLANS
 INDICATE THAT A 3FT. X 18FT. SECTION OF THE TERMINAL
 PLATFORM IS TO BE PLACED AT A LOWER ELEVATION; THIS IS
 NOT REFLECTED IN THE CONSTRUCTION TECHNOLOGY, INC.
 PLANS. PLEASE CLARIFY.
  
 6. PLEASE PROVIDE DETAILS OF PROPOSED GATE/FENCING.
  
 7. PLEASE NOTE THAT PERMITTING AND CONSTRUCTION OF THE
 DOCK MAY NOT OCCUR UNTIL PERMITTING/CONSTRUCTION HAS
 BEGUN ON THE UPLAND RESIDENCE. ADDITIONALLY, A
 CERTIFICATE OF COMPLETION FOR THE DOCK WILL NOT BE
 ISSUED UNTIL A CERTIFICATE OF OCCUPANCY HAS BEEN ISSUED
 FOR THE UPLAND RESIDENCE.
  
 8. PLEASE NOTE THAT ELECTRONIC COPIES OF ALL PLANS
 SHALL BE PROVIDED.

 PLEASE NOTE THE FOLLOWING WHEN RESUBMITTING:
  
 A) WRITTEN RESPONSES TO COMMENTS MADE BY ALL
 DEPARTMENTS SHALL BE PROVIDED UPON RESUBMISSION.
  
 B) 15 SETS OF REVISED PLANS AND/OR DOCUMENTS SHALL BE
 PROVIDED, ALONG WITH ONE (1) REDUCED COPY (11IN X
 17IN), AND AN ELECTRONIC COPY OF ALL PLANS.
  
 C) PLEASE NOTE THAT RE-SUBMITTALS MAY BE SUBJECT TO
 ADDITIONAL COMMENTS.
  
 D) IF NO RESPONSE IS RECEIVED WITHIN 60 DAYS OF THE
 ISSUANCE OF THESE COMMENTS, THE APPLICATION WILL BE
 CONSIDERED WITHDRAWN AND ANY FURTHER ACTION WILL
 REQUIRE SUBMISSION OF A NEW DEVELOPMENT APPLICATION.
  
 E) PLEASE NOTE THAT YOU WILL BE PERMITTED ONE (1)
 RESUBMITTAL AT NO ADDITIONAL COST. IF PREVIOUSLY-ISSUED
 COMMENTS CONTINUE TO NOT BE SUFFICIENTLY ADDRESSED, THE
 APPLICANT WILL BE ACCESSED A RESUBMITTAL FEE. SUCH FEE
 WILL BE 20% OF THE ORIGINAL APPLICATION FEE.
  
 F) IN ORDER TO BE CONSIDERED FOR THE APRIL 16, 2013
 PLANNING BOARD AGENDA, ALL COMMENTS SHALL BE
 SATISFACTORILY ADDRESSED. PLEASE SUBMIT ALL REVISED
 DOCUMENTS/PLANS NO LATER THAN FRIDAY, MARCH 15, 2013.
  
 G) TO BE PLACED ON THE APRIL 16, 2013 PLANNING BOARD
 AGENDA, SIGNS SHALL BE POSTED ON THE PROPERTY NO LATER
 THAN MARCH 31, 2013. SIGNAGE SHALL CONFORM AND BE
 INSTALLED WITH THE REQUIREMENTS OF THE CITY?S ZONING
 AND LAND DEVELOPMENT REGULATIONS, AS ALSO PROVIDED IN
 THE DEVELOPMENT APPLICATION. A SIGNED AND NOTARIZED
 AFFIDAVIT SHALL BE PROVIDED BY THE DEADLINE REFERENCED
 ABOVE.
