 PLEASE PROVIDE A WRITTEN RESPONSE TO THE FOLLOWING
 REVIEW COMMENTS:
  
 1. THE PUBLIC NOTICE MAILER AFFIDAVIT WILL NEED TO BE
 PROVIDED UPON MAILING THE NOTICE.
  
 2. PLEASE PROVIDE A LETTER FROM FLORIDA PUBLIC
 UTILITIES INDICATING NO OBJECTION TO INSTALLATION OF
 THE LANDSCAPING WITHIN CLOSE PROXIMITY OF THE EXISTING
 GAS LINE.
  
 3. IT IS UNDERSTOOD THAT THE TRASH RECEPTACLE WILL BE
 PLACED WITHIN THE BUILDING. A LETTER SHALL BE PROVIDED
 FROM THE CITY???S SANITATION DEPARTMENT INDICATING NO
 OBJECTION.
  
 4. CONDITION OF APPROVAL - RESTRICTIONS WILL BE PLACED
 ON THE HOURS FOR LOADING/UNLOADING TO REDUCE CONFLICT
 BETWEEN LOADING TRAFFIC AND BUSINESS TRAFFIC.
  
 5. DOCUMENTATION SHALL BE PROVIDED FROM THE FLORIDA
 DEPARTMENT OF TRANSPORTATION (FDOT) INDICATING THEIR
 REVIEW AND APPROVAL OF ALL IMPROVEMENTS TO THE SOUTHERN
 BOULEVARD RIGHT-OF-WAY, INCLUDING SIZE AND LOCATION OF
 INGRESS/EGRESS POINTS, ETC.
  
 6. CONDITION OF APPROVAL ??? NO CONVENIENCE ITEMS SHALL
 NOT BE SOLD WITHIN THE FACILITY OR ON THE PROPERTY AT
 ANY TIME. THIS CONDITION DOES NOT EXCLUDE THE PROVISION
 OF VENDING MACHINES WITHIN THE WAITING AREA.
  
 7. CONDITION OF APPROVAL ??? HOURS OF OPERATION WILL BE
 LIMITED TO ENSURE COMPATIBILITY WITH THE ADJACENT
 RESIDENTIAL PROPERTIES.
  
 8. THE DIMENSION LINE SHOWN ON THE ANGLED PARKING
 SPACES SHOWS 9 FEET IN WIDTH, WHILE THE SPACE ONLY
 MEASURES 8.5 FEET; DIMENSIONS SHALL ACCURATELY REFLECT
 WHAT IS PROVIDED.
  
 9. CONDITION OF APPROVAL ??? AS SITE LIGHTING IS BEING
 PROPOSED, A PHOTOMETRIC PLAN WILL BE REQUIRED PRIOR TO
 THE ISSUANCE OF A BUILDING PERMIT. SUCH LIGHTING SHALL
 NOT EXCEED 0.5 FOOTCANDLES WHEN MEASURED IN A
 RESIDENTIAL DISTRICT, AND SHALL BE REFLECTED AWAY FROM
 THE ADJACENT RESIDENTIAL PROPERTIES.
  
 10. THE PROPOSED FREESTANDING SIGN AT THE NORTHEAST
 CORNER OF THE PROPERTY DOES NOT MEET THE MINIMUM
 SETBACK REQUIREMENTS. LOW FREESTANDING SIGNS SHALL BE A
 MINIMUM OF FIVE (5) FEET FROM THE PUBLIC RIGHT-OF-WAY.
 ADDITIONALLY, STAFF DOES NOT SUPPORT MAINTAINING THE
 EXISTING NONCONFORMING HIGH FREESTANDING SIGN. IF THIS
 SIGN IS PROPOSED TO REMAIN, AN APPLICATION FOR A
 VARIANCE WOULD BE REQUIRED TO BE SUBMITTED, NOT A
 WAIVER WITHIN THE CLASS B SPECIAL USE PERMIT
 APPLICATION.
  
 11. CONDITION OF APPROVAL ??? ALL VACUUMS WILL BE
 REQUIRED TO BE WITHIN THE ENCLOSED BUILDING.
  
 12. CONDITION OF APPROVAL ??? NO STORAGE OF VEHICLES
 WILL BE PERMITTED ON-SITE. ALL SERVICES SHALL BE
 LIMITED TO WAITING CUSTOMERS.
  
 13. CONDITION OF APPROVAL ??? TRAFFIC SHALL NOT BLOCK
 THE PUBLIC SIDEWALK.
  
 PLEASE NOTE THE FOLLOWING WHEN RESUBMITTING:
  
 A) WRITTEN RESPONSES TO COMMENTS MADE BY ALL
 DEPARTMENTS SHALL BE PROVIDED UPON RESUBMISSION.
  
 B) 15 SETS OF REVISED PLANS AND/OR DOCUMENTS SHALL BE
 PROVIDED, ALONG WITH ONE (1) REDUCED COPY (11IN X
 17IN), AND AN ELECTRONIC COPY OF ALL PLANS.
  
 C) PLEASE NOTE THAT RE-SUBMITTALS MAY BE SUBJECT TO
 ADDITIONAL COMMENTS.
  
 D) IF NO RESPONSE IS RECEIVED WITHIN 60 DAYS OF THE
 ISSUANCE OF THESE COMMENTS, THE APPLICATION WILL BE
 CONSIDERED WITHDRAWN AND ANY FURTHER ACTION WILL
 REQUIRE SUBMISSION OF A NEW DEVELOPMENT APPLICATION.
  
 E) PLEASE NOTE THAT YOU WILL BE PERMITTED ONE (1)
 RESUBMITTAL AT NO ADDITIONAL COST. IF PREVIOUSLY-ISSUED
 COMMENTS CONTINUE TO NOT BE SUFFICIENTLY ADDRESSED, THE
 APPLICANT WILL BE ACCESSED A RESUBMITTAL FEE. SUCH FEE
 WILL BE 20% OF THE ORIGINAL APPLICATION FEE.
