 - PLEASE PROVIDE RESPONSES TO REVIEW COMMENTS IN
 WRITTEN FORMAT.
  
 - WHEN RESUBMITTING, PLEASE PROVIDE A MINIMUM OF TEN
 (10) COPIES AND AN ELECTRONIC COPY IN .PNG FORMAT OF
 ALL REVISED PLANS. PLEASE NOTE THAT RE-SUBMITTALS MAY
 BE SUBJECT TO ADDITIONAL COMMENTS.
  
 - PLEASE PROVIDE ALL TEXT DOCUMENTS IN WORD FORMAT.
  
 - THE PPRC RESUBMITTAL COMMENTS SHALL BE SUFFICIENTLY
 ADDRESSED AND SUBMITTED TO THE PLANNING AND ZONING
 DEPARTMENT BY NOON ON MONDAY, NOVEMBER 30, 2009, TO BE
 SCHEDULED FOR THE DECEMBER 15, 2009 PLANNING BOARD
 MEETING.
  
 - PLEASE NOTE THAT IF THE PLANNING AND ZONING
 DEPARTMENT HAS NOT RECEIVED A RESPONSE TO THE COMMENTS
 WITHIN 60 DAYS FROM COMMENT DATE STATED ABOVE, THE CASE
 SHALL BE CONSIDERED WITHDRAWN. ANY FURTHER ACTION SHALL
 REQUIRE RESUBMITTAL OF A NEW DEVELOPMENT APPLICATION.
  
 1) PROVIDE THE ANGLE FOR THE NEW ANGLED PARKING SPACES.
  
 2) THE NEW ANGLED PARKING SPACES APPEAR TO BE AT A 60
 DEGREE ANGLE. THE SPACES SHOWN ON THE PLAN ARE ONLY
 14.5 FEET DEEP (NOTE THAT THE MEASURE POINT IS THE
 SHORTEST SIDE).
  
 3) 60 DEGREE ANGLED PARKING REQUIRES AN 18 FOOT WIDE
 DRIVE AISLE. THE 12 FOOT WIDE AISLE AS PROVIDED WOULD
 ONLY SERVE 30 DEGREE ANGLED PARKING.
  
 4) PROVIDE THE RIGHT-OF-WAY CENTER LINE SETBACK TO THE
 BUILDING.
  
 5) PROVIDE THE SET BACK TO THE WALL FROM THE PROPERTY
 LINE.
  
 6) THE WEST ELEVATION IS MISLABELED.
  
 7) CORRECT THE NORTH, EAST AND SOUTH ELEVATIONS BASED
 ON THE NEW BUILDING LAYOUT.
  
 8) PROVIDE DETAILS OF THE PROPOSED BLOCK WALL AND A
 WEST ELEVATION SHOWING THE WALL IN FRONT OF THE
 BUILDING. STAFF HAS CONCERNS THAT THE PLACEMENT AND
 MATERIAL OF THE WALL DOES NOT MEET THE INTENT OF THE
 BUILDING ADDRESSING SOUTH OLIVE AVENUE.
  
 9) HOW WILL ACCESS BE PROVIDED TO THE A/C UNITS? NO
 OPENING IS SHOWN IN THE WALL AND THERE IS NO ACCESS
 FROM THE BUILDING.
  
 10) STAFF WAS UNDER THE IMPRESSION THAT THE INSIDE OF
 THE WALL WOULD BE SCREENED WITH LANDSCAPE TO PROVIDE A
 VIEW FOR THE PATIENTS. IF LANDSCAPE IS PROVIDED, HOW
 WILL IT BE MAINTAINED?
  
 11) THE DESIGN OF THE WEST SIDE OF THE BUILDING READS
 AS THE REAR OF A BUILDING AND THUS DOES NOT INTERACT
 WITH SOUTH OLIVE AVENUE. IN ORDER TO INTEGRATE WITH
 SOUTH OLIVE AVENUE AND THE MANGO PROMENADE
 NEIGHBORHOOD, PLEASE ADD A PORCH TO THE WEST FACADE OF
 THE BUILDING. STAFF RECOMMENDS THAT IT BE LOCATED IN
 FRONT OF THE ORTHO TREATMENT BAYS. THERE ARE SEVERAL
 OPTIONS FOR THE PORCH:
  
 A) A THREE FOOT DEEP ROOFED PORCH EXTENDING FROM THE
 BUILDING.
 B) A FIVE FOOT DEEP PORCH EXTENDING FROM THE BUILDING
 WITH A ROOF EXTENDING THREE FEET FROM THE BUILDING.
 C) MOVE THE BUILDING TWO FEET EAST OR REDUCE THE WEST
 FOOTPRINT TWO FEET AND ADD A FIVE FOOT DEEP, ROOFED
 PORCH.
  
 12) THE PORCH SHALL BE ACCESSED BY A DOOR FROM THE
 INTERIOR OF THE BUILDING - RECOMMEND FRENCH STYLE
 DOOR(S).
  
 13) STAFF WOULD LIKE TO SEE THE PORCH HAVE STEPS TO THE
 GROUND THAT LEAD TO A GATE IN THE WALL TO CREATE THE
 INTERACTION WITH SOUTH OLIVE AVENUE.
  
 14) IN ORDER TO ACHIEVE A MORE RESIDENTIAL APPEARANCE
 ON THE WEST ELEVATION STAFF SUGGESTS USING DIFFERENT
 ROOF SLOPES AND A RELOCATION OF THE CENTER GABLE. STAFF
 IS AVAILABLE TO DISCUSS OPTIONS.
  
 15) PLEASE CLEARLY DEFINE PAVED VS. LANDSCAPE AREAS ON
 THE PLANS. DIFFICULT TO DISCERN AROUND THE BIKE RACK.
  
 16) THE EASTERN PARKING ROW REQUIRES TREES IN THE
 TERMINAL ISLANDS.
  
 17) SHIFT THE TREE IN THE EASTERN TERMINAL ISLAND OF
 THE OFF-SITE PARKING AREA TO THE NORTH. THIS WILL ALLOW
 ITS ROOT SYSTEM MORE SPACE AND ALIGN IT WITH THE
 WESTERN TERMINAL ISLAND TREE.
  
 18) JIM WITMER OF THE CONSTRUCTION SERVICES DEPARTMENT
 REVIEWED THE LAYOUT OF THE INTERIOR OF THE BUILDING AND
 THE PROPOSED ACCESSIBLE ROUTE ON THE NORTH SIDE OF THE
 BUILDING FOR FLORIDA ACCESSIBILITY CODE COMPLIANCE. HE
 EXPRESSED CONCERNS THAT THE CURRENT DESIGN DOES NOT
 COMPLY WITH THE CODE. SOME OF HIS COMMENTS WILL AFFECT
 THE SITE AND LANDSCAPE PLANS AND ELEVATIONS.
  
 19) THE SANITATION DEPARTMENT HAS NOT YET AGREED TO THE
 ELIMINATION OF THE DUMPSTERS.
